We describe a 13-year-old girl with juvenile rheumatoid arthritis who developed obstructive uropathy and renal failure. Retroperitoneal fibrosis (RPF) was confirmed by surgery. Although the renal failure and hydronephrosis resolved after surgery, the symptoms of vascular occlusion persisted. We consider that early diagnosis and treatment are essential. In cases of autoimmune disease, RPF should be considered when there is acquired obstructive uropathy accompanied by vascular occlusion syndrome.

We describe the eight case of idiopathic retroperitoneal fibrosis infiltrating the ureter to cause intrinsic obstruction. Discovery of this situation intraoperatively necessitates a change in treatment plan, as ureterolysis is not sufficient.

It is generally recognized that in many patients the ureteric obstruction and other manifestations of non-malignant retroperitoneal fibrosis will respond to treatment with corticosteroids. However, most surgeons are reluctant to use steroids as the primary treatment for patients with this condition, mainly because of the risk of mismanagement of malignant retroperitoneal fibrosis. Our experience in the care of 17 patients with non-malignant retroperitoneal fibrosis has led us to believe that an initial non-surgical approach is both safe and preferable.

The carcinoid syndrome and fibrosis are unusual but identifiable disease processes. We report a rare case of retroperitoneal fibrosis associated with an ileal carcinoid in the absence of metastatic disease. The literature is reviewed.

Idiopathic retroperitoneal fibrosis is uncommon enough to be placed low down on a list of differential diagnoses of any problem facing a general surgeon in a district hospital. In this paper, 4 cases of retroperitoneal fibrosis are described, all of whom presented within a 5-year period to the same surgeon; each described a different symptomatology. This paper serves to reiterate the diversity of presenting features in retroperitoneal fibrosis and to demonstrate that this condition may not be as rare as is widely believed.

Three patients with lumbar anterior spinal fusion (two with Dwyer instrumentation) developed retroperitoneal fibrosis in relation to the operative site. This complication is possibly a reaction to the retroperitoneal insult. Haematoma, low-grade infection and metallic implant are other likely contributory factors. Its occurrence is affected by the laterality of surgical approach and extent of dissection. Radiologists should be aware of such a complication when investigating post-fusion patients. Early diagnosis and treatment prevent further renal damage.

A case of retroperitoneal fibrosis causing pancreatitis is reported; another case is reported as well, where chronic pancreatitis caused secondary symptomatic retroperitoneal fibrosis. Differential diagnosis of the 2 diseases is discussed. Surgical therapy has to eliminate complications and reduce retroperitoneal fibrous tissue structures; this has to be followed by continuous drug therapy aimed at slowing proliferation of fibrous tissue.
