Microsurgical reconstruction of extensive scalp defects.

Large, full-thickness scalp defects represent a reconstructive challenge that has benefitted greatly from the introduction of microsurgical techniques. The authors review their experience with 16 patients with acquired defects of the scalp for which local or regional reconstructive options were unavailable. The mean age at the time of operation was 44.8 years. Nine patients underwent resection of malignant scalp lesions, followed immediately by free-flap coverage. Six patients required revision procedures for unstable scar as a result of prior trauma (2), old scalp avulsions (2), and multiple …

Cicatricial pemphigoid rarely involves the scalp.

We present a case of cicatricial pemphigoid with significant scarring alopecia as a major manifestation and review our series of cicatricial pemphigoid patients in Oxford (UK). Only four of 54 patients had scarring alopecia of the scalp. While rare, this manifestation poses a difficult management problem; patients find this cosmetically distressing and there are no successful treatments available to date.

Technical aspects of prolonged scalp expansion.

Tissue expansion offers the reconstructive surgeon a better option than the conventional methods of skin grafts or the transfer for non-hair-bearing, regional or microsurgical flaps for reconstructing large defects that cannot be repaired with adjacent hair-bearing flaps. Aspects of preoperative planning and technical factors of scalp expansion are presented to assist the clinician in managing these difficult reconstructive challenges.

Scalp-ear-nipple syndrome: additional manifestations.

Scalp-ear-nipple (SEN) syndrome is a rare, autosomal dominant condition that causes aplasia cutis congenita of the scalp, alteration of the shape of the external ear, and hypoplasia of the nipple. Women in a new family, the fifth to be described, had virtually complete aplasia of the breast and a small skin dimple without any pigmentation instead of a normal nipple, although other affected women had normal breast and nipple development. Dental changes included widely spaced or missing secondary teeth; the ears were cupped or folded and stood out from the head, axillary apocrine secretion and a…

Anterior fontanelle scalp cysts in infancy.

Seven infant patients with anterior fontanelle scalp cysts who were managed at the Paediatric Surgical Department of Muhimbili Medical Centre, Dar-es-Salaam between January 1986 and December, 1992 were studied. They were all Africans. There were 2 boys and 5 girls. Their ages ranged from 4 months to 24 months. The onset was at birth in 6 infants while in one case it was at 3 months of age. Their mental as well as motor development was normal. They were all easily and completely excised and in none was there intracranial communication. The histology was that of dermoid cyst in 6 patients and se…

Surgical management of advanced scalp cancer.

A series of 53 advanced scalp cancer patients treated with surgery is presented. A review of these patients concludes: 1) local tumor growth--not distant metastasis--is the primary problem; 2) management is based on the tendency of a tumor to spread laterally early in its development with a deep invasion occurring later on; 3) when deep invasion occurs, radical excision and reconstruction is indicated to control the disease; 4) recurrence following adequate local therapy requires a change in treatment modality.

Scalp hair patterns in mental subnormality.

Scalp hair patterns have been examined in 1901 healthy subjects and 1789 severely subnormal patients. Patients with Down's syndrome had a highly significant excess of midline occipital whorls and a deficit of right-sided occipital whorls. Five out of 44 patients with microcephaly had a distinct 'up-sweep' of the frontal hair, a much lower proportion than found previously. Patients with unspecified mental subnormality had a highly significant deficit of multiple occipital whorls. Cutis verticis gyrata was noted incidentally in 15 subnormal patients, 13 of whom were males. Observation of hair pa…

Artificial intelligence-based prescription of personalized scalp cosmetics improved the scalp condition: efficacy results from 100 participants.

Background: Scalp-related symptoms such as dandruff and itching are common with diverse underlying etiologies. We previously proposed a novel classification and scoring system for scalp conditions, called the scalp photographic index (SPI); it grades five scalp features using trichoscopic images with good reliability. However, it requires trained evaluators.Aim: To develop artificial intelligence (AI) algorithms for assessment of scalp conditions and to assess the feasibility of AI-based recommendations on personalized scalp cosmetics. Methods: Using EfficientNet, convolutional neural network (…

Introducing the Rectus Fascia Scalp Augmentation Technique: A New Method for Improving Scalp Durability in Cranioplasty Reconstruction.

OBJECTIVE: Patients with cranioplasty in need of skull reconstruction are at increased risk of complications when presenting with abnormally thin scalps. As such, the authors sought to develop and investigate a new technique for scalp augmentation using autologous rectus fascia grafts (ARFGs) for prevention of implant extrusion and long-term scalp durability. METHODS: A retrospective review of our database, consisting of 450 consecutive cranial reconstructions from 2012 to 2017, was performed under institutional review board approval. Selection criteria included all adult patients requiring i…

Scalp infestation by Pthirus pubis in a 91-year-old woman: A rare cause of chronic scalp pruritus.

Pthirus pubis is an ectoparasite that typically infests pubic hair and is primarily transmitted through close sexual contact. Although extragenital involvement may occur, scalp infestation remains exceptionally uncommon. We report the case of a 91-year-old woman who presented with a two-month history of persistent scalp pruritus that failed to respond to oral terbinafine and ketoconazole shampoo. Dermatological examination revealed numerous adult lice and nits attached to scalp hairs, with additional involvement of the eyelashes, eyebrows, and pubic region. Microscopic examination confirmed Pt…

Scalp and serum profiling of frontal fibrosing alopecia reveals scalp immune and fibrosis dysregulation with no systemic involvement.

BACKGROUND: Frontal fibrosing alopecia (FFA) is a progressive, scarring alopecia of the frontotemporal scalp that poses a substantial burden on quality of life. Large-scale global profiling of FFA is lacking, preventing the development of effective therapeutics. OBJECTIVE: To characterize FFA compared to normal and alopecia areata using broad molecular profiling and to identify biomarkers linked to disease severity. METHODS: This cross-sectional study assessed 33,118 genes in scalp using RNA sequencing and 350 proteins in serum using OLINK high-throughput proteomics. Disease biomarkers were …

Flavor Scalping in Packaged Foods: A Review.

Over the past decades, plastics have been increasingly employed to package foods and beverages. Furthermore, foods, nowadays, are kept in co … tential interaction of food with the packaging container includes: permeation, migration and flavor scalping. Most food and beverage containers are lined with plastics mainly polyolefins, which due to their low polarity tend to absorb volatile compounds of similar polarity. Absorption of flavor compounds by polymers involves both partitioning and diffus …

Understanding the Scalp: Dandruff and Seborrheic Dermatitis.

Dandruff (DF) and seborrheic dermatitis (SD) are prevalent, chronic scalp disorders that affect a large portion of the global population, often leading to significant psychological distress and quality-of-life impairment. While DF is a milder form characterized by mild scaling, itching, and no visible inflammation, SD presents as a more severe condition with red, scaly, and often inflamed lesions. Despite the high prevalence of these conditions, research specific to their epidemiology and management in Canada remains limited. This review examines the etiology, epidemiology, and management of D…
