Scaphoid nonunion treated by open reduction, anterior inlay bone grafting, and Kirschner-wire fixation.

Forty-three patients with ununited fractures of the scaphoid were treated by open reduction, anterior inlay bone grafting and Kirschner wire fixation. All but one achieved bone union. The one failure was successfully treated with a second anterior inlay bone grafting procedure. The average period of cast immobilization was 7 weeks. The presence of an unstable nonunion did not adversely affect the results. In cases with carpal instability, we used a longer corticocancellous graft to restore a more normal alignment of the scaphoid by opening up the volar cortex. Preoperatively, carpal instabilit

Outcomes of volar locking plate fixation and bone grafting for scaphoid fracture non-unions.

BACKGROUND: Scaphoid fractures are the most common carpal bone fractures and are particularly prone to non-union due to their distinctive blood supply1. If left untreated, scaphoid non-union may result in carpal collapse and severe degenerative arthritis, leading to significant impairment of hand function. Delayed or missed diagnoses increase the risk of non-union and subsequent osteoarthritis. Open reduction and internal fixation (ORIF) with bone grafting have become the standard approach to prevent the progression of arthritis resulting from non-union2. However, the optimal fixation method r

Scaphoid Nonunion Arthroscopic-Assisted Reduction and Percutaneous Fixation With Distal Radius Autologous Bone Graft.

Scaphoid fractures are common and often progress to nonunion due to the unique vascularity. When nonunion occurs, patients are at risk of developing carpal collapse and arthritis if not managed appropriately with traditional surgical management. However, open surgical management puts the fragile vascular supply and extrinsic soft tissue stabilizers of the carpus at risk, compromising healing and clinical outcomes. Arthroscopy allows for the preservation of these structures and improved visualization of fracture debridement, reduction, and alignment. We describe an arthroscopic-assisted reducti

Surgical Treatment of Scaphoid Non-Union in Adolescents: A Modified Vascularized Bone Graft Technique.

Background: Although adolescents are at a lower risk of developing scaphoid non-union than adults, this complication is not uncommon in youn … cases and carries no risk of donor site morbidity. It enables preservation of severely compromised scaphoid bones, prevents early-onset wrist osteoarthritis, and facilitates the return to sports activities for young patients.

Treatment of failed scaphoid nonunion fixation using free medial femoral condyle vascularized bone grafting.

BACKGROUND: Nonunion in scaphoid fractures may be considered a devastating problem. Union failure results in scaphoid deformity, resorption, and bone loss. Failed previous fixation decreases remaining bone stock and makes it more difficult to achieve union. Free vascularized graft represents a good option to achieve scaphoid union with revision fixation. Our study aims at the assessment of the management of scaphoid fractures non-union after failed previous fixation with the use of a free vascularized graft from the medial femoral condyle. METHODS: This is a retrospective study including 16 c

Scaphoid nonunion: does open reduction, bone grafting and Herbert screw fixation justify the treatment?

PURPOSE: Conventional bone grafting and Herbert screw fixation give satisfactory results for scaphoid nonunion; however, vascularized bone grafting has superior results, especially in the case of avascular necrosis of proximal fragment. Vascularized bone grafting is technically more demanding with small error of margin, problems of getting the appropriate graft, fixation and incorporation, and requires longer duration for wrist immobilization. METHODS: Forty-five patients of scaphoid nonunion were treated by cancellous bone grafting, cortex containing graft if required and Herbert screw fixat

Management of scaphoid nonunion with avascular necrosis using 1,2 intercompartmental supraretinacular arterial bone graft.

BACKGROUND: 1,2 ICSRA, introduced by Aidembery et al., is a well-established technique, with up to 100% union rate among different studies. The purpose of our study was to evaluate the outcome of scaphoid nonunion undergoing 1,2 ICSRA bone graft in Iran. MATERIALS AND METHODS: All participants who presented sequentially over a period of 24 months between 2010 and 2013 with nonunion scaphoid fracture with AVN in proximal pole were included in the study. Anteroposterior and lateral view plain radiographs of carpal bones were obtained for diagnosis of nonunion, and a diagnosis of avascular necro

Examination tests predictive of bone injury in patients with clinically suspected occult scaphoid fracture.

BACKGROUND: This study aims to identify the physical examination tests most indicative of bone injury in patients with clinically suspected occult scaphoid fractures. METHODS: Ten physical examination manoeuvres were performed on 41 patients with a history of a fall on an outstretched hand and tenderness at the anatomical snuffbox and scaphoid tubercle without a radiographically visible fracture line. The results of wrist examination and subsequent magnetic resonance imaging (MRI) were recorded. The sensitivity, specificity, positive and negative predictive values, accuracy and likelihood rat

Vascularized second metacarpal-base bone graft in scaphoid non-union by the palmar approach.

Vascularized second metacarpal-base bone grafting from the palmar side for scaphoid non-union was performed in seven patients (six males and one female, with a mean age of 26 years). In all patients, bone union was obtained at a mean of 10.2 weeks. In six of the seven patients, preoperative dorsal intercalated segment instability (DISI) was corrected. The results were excellent in three patients, good in two patients, and fair in two patients, showing that more than 80 percent of the patients had satisfactory results. In this procedure, the second dorsal metacarpal artery, with a large vascula

Scaphoid nonunion treated with bone graft and Herbert screw. 23 of 28 fractures healed.

We retrospectively reviewed 28 patients with symptomatic, established nonunion of the waist of the scaphoid who were treated with bone grafting and Herbert screw fixation. A volar approach was used in 20 patients, and a dorsal in 8. Grafting was performed with cancellous bone from the radius in 16 patients and a iliac wedge graft in 12 patients. At a mean follow-up of 3 (1-6) years. 5 of the 28 patients had not united. The surgical approach, the type of graft applied, and the duration of postoperative immobilization played a role for the final outcome. The volar approach appeared to be associa

Bone density comparison of selected carpal and tarsal bones: validation for their use in compression fracture fixation studies of scaphoid screws.

INTRODUCTION: To determine if trabecular, total and cortical bone densities of the capitate, navicular, cuboid, and first cuneiform were equivalent to those of the scaphoid, such that these bones could be used in place of the scaphoid in evaluating new headless scaphoid compression screws. METHODS: Fifty scaphoids, capitates, naviculars, cuboids, and first cuneiforms were harvested from fresh frozen cadavers. The trabecular, total and cortical bone densities were measured using pQCT technology and statistically compared. RESULTS: A paired t comparison between paired scaphoids and capitates s

Association between bone shape and the presence of a fracture in patients with a clinically suspected scaphoid fracture.

Scaphoid fractures are difficult to diagnose with current imaging modalities. It is unknown whether the shape of the scaphoid bone, assessed by statistical shape modeling, can be used to differentiate between fractured and non-fractured bones. Therefore, the aim of this study was to investigate whether the presence of a scaphoid fracture is associated with shape modes of a statistical shape model (SSM). Forty-one high-resolution peripheral quantitative computed tomography (HR-pQCT) scans were available from patients with a clinically suspected scaphoid fracture of whom 15 patients had a scapho…

Treatment of scaphoid nonunions with a vascularized bone graft based on the first dorsal metacarpal artery.

Four patients with chronic nonunion of the scaphoid were treated by a vascularized bone graft based on first dorsal metacarpal artery. The mean duration of the nonunion was 28.5 months (range 12-48 months). There was avascular necrosis in all patients confirmed by magnetic resonance imaging (MRI). None of the patients had previous attempts at surgical reconstruction. Two fractures were localized at the waist one in the distal part and one at the proximal pole. Osseous union of the scaphoid was confirmed by X-ray in all patients in an average of 2.1 months. We recommend this technique for the t
