Nodular Posterior Scleritis Masquerading as a Subretinal Mass.

This is the case report of a 50-year-old female with no systemic comorbidities who presented to the eye clinic with a 1-month history of rig … ion rate. Infective and autoimmune markers were negative. A diagnosis was made of nodular posterior scleritis and the patient was treated with intravenous corticosteroids initially, and subsequently switched to oral corticosteroids. There was complete resolution of the mass with optic atrophy as a result. Posterior nodular scleritis is an extremely rare pot …

Molecular pathobiology of scleritis and its therapeutic implications.

Scleritis and other autoimmune diseases are characterized by an imbalance in the levels of pro-inflammatory and anti-inflammatory molecules with the balance tilted more towards the former due to the failure of recognition of self. The triggering of inflammatory process could be ascribed to the presence of cytoplasmic DNA/chromatin that leads to activation of cytosolic DNA-sensing cGAS-STING (cyclic GMP-AMP synthase linked to stimulator of interferon genes) pathway and enhanced expression of NF-κB that results in an increase in the production of pro-inflammatory bioactive lipids. Bioactive lipi…

Adalimumab-associated Acquired Hemophilia in a Patient with Scleritis.

Acquired hemophilia A (AHA) is a rare condition that may be drug-induced. In this case report, we describe a patient who presented with extensive subcutaneous bleeding three years after beginning treatment with adalimumab for necrotizing scleritis. His workup was compatible with drug-induced AHA. He was treated with high-dose corticosteroids, cyclophosphamide, and rituximab. Adalimumab was discontinued. We present this case as an example of a rare, but potentially life-threatening, complication of adalimumab.

Multimodal imaging in a child with severe posterior scleritis.

Objective: Posterior scleritis in a child is a rare condition. High-resolution imaging techniques in the course of posterior scleritis have not been published extensively in literature. The authors reported a case of posterior scleritis in a 12-year-old child to demonstrate multimodal imaging techniques in the course of development and improvement of the disease. Methods: Case report that included fundus photography, spectral domain optical coherence tomography with enhanced depth imaging, blue-peak autofluorescence, multicolor imaging, fluorescein angiography, indocyanine green angiography, a…

Treatment of chronic non-infectious uveitis and scleritis.

Ocular inflammations such as uveitis and scleritis can lead to significant visual impairment if not treated properly. To limit potentially sight-threatening complications, good control of the inflammation in the acute phase is necessary. Corticosteroids have been the mainstay of ocular therapies for many years, but high doses of corticosteroids, which are required to maintain quiescence in severe uveitis, can be associated with many systemic and ocular complications. In order to limit steroid side-effects, classic immunosuppressant and immunobiologic agents have been widely used as steroid-spa…

Poststreptococcal syndrome presenting as posterior scleritis in a child.

Posterior scleritis in children is very rare. In contrast to the adult form, pediatric posterior scleritis has not previously been associated with any systemic disorder. We describe a case of an 11-year-old girl who presented with left eye pain and redness and was found to have posterior scleritis on ultrasonography. Her laboratory work-up revealed a highly elevated antistreptolysin O titer; the rest of her serologic and radiologic evaluation was unremarkable. She was diagnosed with presumed poststreptococcal posterior scleritis and improved with a slow taper of oral steroids.

Clinical Features and Complications of Scleritis in Chinese Patients.

PURPOSE: To characterize the clinical features of scleritis in Chinese patients. METHODS: The history, demographics, ocular findings, auxiliary examination findings, complications, systemic diseases and therapeutic effects were analyzed retrospectively. RESULTS: The study included 124 male and 169 female patients with scleritis. Anterior and posterior scleritis were, respectively, found in 243 and 42 patients. The other eight patients had both anterior and posterior involvement. The mean age of scleritis onset was 39.4 years. Systemic diseases associated with scleritis mainly included rheuma…

Optical coherence tomography in the diagnosis of scleritis and episcleritis.

PURPOSE: To describe the optical coherence tomography (OCT) findings in eyes with active anterior scleritis and compare the findings to those with episcleritis and normal controls. DESIGN: Prospective evaluation of a diagnostic test. METHODS: We included a total of 30 eyes of 30 patients with unilateral anterior scleral or episcleral inflammation. The contralateral 30 eyes with no active ocular disease served as controls. OCT was performed over the anterior sclera in the inflamed area on all cases. The OCT images were analyzed to determine the thickness of the sclera and the presence or abse…

Postoperative necrotizing scleritis: a report of four cases.

Postoperative necrotizing scleritis should be considered in cases of persistent localized postoperative inflammation following all forms of surgical trauma. We present the history, clinical findings, and follow-up data of four patients with postoperative necrotizing scleritis. The clinical records of four patients who developed scleritis following ocular surgery were retrospectively reviewed. The first step in managing necrotizing scleritis is to rule out infectious etiology. Surgically induced necrotizing scleritis is an immune-mediated condition that can coexist with concomitant infectious c…

Pharmacotherapy of scleritis: current paradigms and future directions.

INTRODUCTION: Scleritis is an inflammatory condition affecting the eye wall that may be associated with a number of systemic inflammatory diseases. Because scleritis can be refractory to standard treatment, knowledge of the body of available and emerging therapies is paramount and is reviewed here. AREAS COVERED: This review focuses on both traditional and emerging therapies for noninfectious scleritis. The authors cover the mechanisms of action and potential adverse effects of each of the treatment modalities. In addition, a summary of the significant MEDLINE indexed literature under the sub…
