Reverse total shoulder versus angular stable plate treatment for proximal humeral fractures in over 65 years old patients.

BACKGROUND: Treatment for displaced proximal humeral fractures is still under debate. Poor rotator cuff status and non-union of the tubercles in elderly patients has caused reversed total shoulder prosthesis growing popularity and showed promising results, even in comparison to angular stable plates fixation. The purpose of this study is to report clinical and radiological results of proximal humerus fractures treated with rTSA or ORIF in elderly. MATERIALS AND METHODS: The study has investigated retrospectively a consecutive series of 73 patients over 65 years old (range 65-91) with proximal

Does grafting of the tuberosities improve the functional outcomes of proximal humeral fractures treated with reverse shoulder arthroplasty?

BACKGROUND: Functional outcomes of reverse total shoulder arthroplasty (rTSA) can be improved by fixation of the tuberosities. This study compares clinical and radiologic results of patients with comminuted proximal humeral fractures treated with rTSA, with and without autologous grafting. METHODS: Thirty-three patients with proximal humeral fractures were treated with rTSA and tuberosity fixation. In 18 patients (group I; mean age, 75 years), tuberosity fixation was augmented with autografting; in 15 patients (group II; mean age, 71 years), graft augmentation was not used. The mean follow-up

Reverse shoulder prosthesis to treat complex proximal humeral fractures in the elderly patients: results after 10-year experience.

PURPOSE: The aim of this study is to report the clinical and radiological results of reverse total shoulder arthroplasty (rTSA) in elderly patients who have been treated for complex humeral fractures. MATERIALS AND METHODS: From January 2005 to December 2014, we have implanted rTSA for proximal humeral fractures in 95 patients (80 women, 15 men) about 75 years old on average (range 62-95 years). All rates and results on intraoperative and postoperative complications have been collected in a specific database. In all cases we have used a modular implant prosthesis (Lima Corporate, San Daniele

Periprosthetic fractures associated with primary total shoulder arthroplasty and primary humeral head replacement: a thirty-three-year study.

BACKGROUND: The objective of this study was to assess the frequency of, and risk factors for, periprosthetic fractures during and after shoulder arthroplasty. METHODS: All adults treated with a primary total shoulder arthroplasty or humeral head replacement at the Mayo Clinic Medical Center from 1976 to 2008 were identified. Periprosthetic fractures were validated by medical record review. Univariate and multivariable-adjusted logistic regression analyses were used to assess the association of demographic factors (age, sex, and body mass index [BMI]), underlying diagnosis, implant fixation (c

Reverse shoulder arthroplasty for proximal humeral fractures: outcomes comparing primary reverse arthroplasty for fracture versus reverse arthroplasty after failed osteosynthesis.

BACKGROUND: Surgical treatment of proximal humeral fractures in the elderly pose challenges in decision making. Reverse total shoulder arthroplasty (RTSA) has been established as a reliable option for salvage of failed hemiarthroplasty, although few studies have analyzed RTSA after failed open reduction with internal fixation (ORIF). This study evaluated the outcomes of patients with failed osteosynthesis who undergo salvage RTSA compared with patients undergoing primary RTSA for proximal humeral fractures. METHODS: We retrospectively reviewed 18 patients who underwent primary RTSA for acute

Additive fiber-cerclages in proximal humeral fractures stabilized by locking plates: no effect on fracture stabilization and rotator cuff function in human shoulder specimens.

BACKGROUND AND PURPOSE: The effect of additive fiber-cerclages in proximal humeral fractures stabilized by locking plates on fracture stabilization and rotator cuff function is unclear. Here it was assessed in a human cadaver study. METHODS: 24 paired human shoulder specimens were harvested from median 77-year-old (range 66-85) female donors. An unstable 3-part fracture model with an intact rotator cuff was developed. 1 specimen of each pair received an additive fiber-cerclage of the rotator cuff after plate fixation, and the other one received a plate fixation without an additive fiber-cercl

Primary stability of an uncemented humeral stem in reverse shoulder arthroplasty for acute proximal humeral fractures: a prospective study.

PURPOSE: The objective of this study was to determine the primary stability of an uncemented humeral implant when used for acute proximal humeral fractures. METHODS: A prospective study that includes consecutive proximal humeral fractures treated with an uncemented rTSA. Three measures were obtained to assess the stability of the implant: the retroversion, the humeral stem height and the varus/valgus alignment of the humeral component. The three measures were done in both the CT scan obtained 24 h after surgery, and the CT scan obtained at the 6 month follow-up. Among the patient sample inclu

Prognostic value of the Neer and Mayo-Fundacion Jimenez Diaz classifications in reverse shoulder arthroplasty for proximal humerus fractures.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is the preferred surgical treatment for non-reconstructible 7 proximal humerus fractures in the elderly population. The most common classification for 8 these fractures is the Neer classification. A more recent system is the Mayo-Fundación Jiménez 9 Díaz (Mayo-FJD) classification, which has demonstrated higher intraobserver and interobserver 10 reliability compared to the Neer system. However, their predictive utility remains unclear. METHODS: The study consisted of a retrospective analysis of prospectively collected data from 40 patients

MRI-Defined Osteonecrosis Extent After Locked Plate Fixation of Proximal Humeral Fractures: Correlation with Shoulder Motion and Radiographic Severity.

Background/Objectives: Humeral head osteonecrosis is a recognised complication of locked plate fixation for proximal humeral fractures, yet the significance of necrotic lesion extent is poorly characterised. This study quantified MRI-defined osteonecrosis extent and its associations with radiographic severity, shoulder motion, and functional outcomes. Methods: Twenty-five patients with Neer three- or four-part proximal humeral fractures treated by locked plate fixation between 2010 and 2022 were retrospectively reviewed. Prediction of acromial and scapular spine fractures after reverse total shoulder arthroplasty using machine learning: a retrospective cohort study.

BACKGROUND: Acromial and scapular spine fractures (ASSF) are an uncommon but significant complication following reverse total shoulder arthroplasty (rTSA). The study aims were to (1) determine the incidence and type of ASSF captured within a local shoulder arthroplasty registry, (2) develop a novel, interpretable machine learning model to predict individual risk of fracture after rTSA, and (3) assess model performance variation based on implant design. METHODS: We conducted a retrospective cohort study including 2,256 registry-documented patients who underwent primary rTSA between 2006 and 20

Does ORIF of rare scapular spine fractures sustained after reverse shoulder arthroplasty benefit elderly patients? A case-series appraisal.

PURPOSE: Scapular spine fractures sustained after reverse shoulder arthroplasty (RSA) are debilitating for elderly patients with osteoporosis. We examined the 1-year postoperative outcomes of open reduction and internal fixation (ORIF) in a small case-series, and hypothesised that patients undergoing surgical treatment for post-RSA scapular spine fractures would improve in function and pain. METHODS: Five consecutive RSA patients within our shoulder arthroplasty register who sustained a scapular spine fracture underwent ORIF using a double plating technique. Standard radiographs and clinical/

Nonoperative management versus reverse shoulder arthroplasty for treatment of 3- and 4-part proximal humeral fractures in older adults.

BACKGROUND: The treatment of 3- and 4-part proximal humeral fractures in the older adult is controversial. No study has directly compared reverse shoulder arthroplasty (RSA) with nonoperative treatment for these fractures. The purpose of this study was to compare clinical and patient-reported outcomes between RSA and nonoperative treatment groups. METHODS: A retrospective review was performed on all 3- and 4-part proximal humeral fractures treated with either RSA or nonoperative treatment with minimum 1-year follow-up. All patients in the nonoperative cohort were offered RSA but declined. Obj

Pediatric lumbar Chance fractures in British Columbia: chart review and analysis of the use of shoulder restraints in MVAs.

Chance fractures of the skeletally immature spine classically occur in frontal motor vehicle accidents (MVAs) when the occupants are restrained by a lap belt only and undergo traumatic hyperflexion of the torso during the impact. We retrospectively examined all MVA-related Chance fractures at British Columbia's Children's Hospital since 1986, by collecting injury and seat-belt use information from chart data and imaging studies. Twenty-six patients were included in the study, 14 wore a lap belt only, seven wore a three-point restraint properly, and five were reportedly misusing the shoulder po

Western Ontario Osteoarthritis of the Shoulder Index (WOOS) - a validation for use in proximal humerus fractures treated with arthroplasty.

BACKGROUND: The Swedish shoulder and Arthroplasty Registry (SSAR) use the Western Ontario Osteoarthritis of the Shoulder Index (WOOS) as their shoulder-specific score in the follow-up. WOOS is not yet validated for use as the Patient Reported Outcome Measurement (PROM) for proximal humerus fractures (PHF) treated with shoulder hemiarthroplasty (SHA) in the Swedish registry. The aim of this study was to examine the validity, the reliability and the responsiveness of WOOS as a PROM for proximal humerus fractures treated with shoulder arthroplasty. METHODS: Data was collected from the SSAR from

Tuberosity union in patients with proximal humerus fractures treated with reverse shoulder arthroplasty: a technical note and exploratory analysis.

PURPOSE: The use of reverse shoulder arthroplasty (RSA) to treat complex humerus fractures is increasing, especially in older, osteoporotic patients. Refixation and tuberosity healing are needed to achieve an optimal range of motion (ROM), external rotation, active forward elevation, and patient satisfaction. Proper healing has been reported, however, in only 40-84% of cases. Our study's aim was to describe a simple, reproducible fixation technique designed to improve tuberosity healing. METHODS: We included 30 patients with acute proximal humerus fractures undergoing RSA (Global Unite Revers
