Characteristic MRI findings of the shoulder, elbow, and wrist joints in elite wheelchair basketball players.

BACKGROUND: The health of wheelchair users' upper limbs is directly related to their quality of life. Moreover, para-sport athletes are subjected to a dual load on their upper extremities from competition and daily life, making it even more critical to maintain upper extremity health. This study aimed to investigate the characteristics of joint disorders in elite wheelchair basketball players using magnetic resonance imaging (MRI). METHODS: We scanned MRI images of the bilateral shoulders, elbows, and wrist joints of ten elite wheelchair basketball players and ten general wheelchair users. Th

Effect of Blue Light and Photosensitizers on Cutibacterium acnes on Shoulder Periprosthetic Joint Infection Isolates.

Introduction: Cutibacterium acnes is gaining recognition as a leading pathogen after orthopaedic shoulder procedures. Photodynamic therapy, a combination of light and a photosensitizer, has demonstrated antimicrobial activity against C. acnes in the treatment of acne vulgaris. We sought to evaluate the effect of photodynamic therapy using blue light and photosensitizers on C. acnes isolates from shoulder prosthetic joint infections. Methods: C. acnes strains isolated from 19 patients with shoulder PJI were exposed to blue light alone (415 nm) or in combination with photosensitizers (fluorescei

Anatomic analysis of the whole articular capsule of the shoulder joint, with reference to the capsular attachment and thickness.

BACKGROUND: Although conventional Bankart repair has been the accepted procedure for traumatic anterior glenohumeral instability, the humeral avulsion of the glenohumeral ligament or an elongation of the capsule remains challenging to decide the appropriate treatment. The anatomical knowledge regarding the whole capsule of glenohumeral joint is necessary to accurately treat for the capsular disorders. The aims of the current study were to investigate the anatomical features of capsular attachment and thickness in a whole capsule of glenohumeral joint. METHODS: We used 13 shoulders in the curr

Arthroscopic debridement for septic arthritis of the shoulder joint: post-infectious arthritis is an inevitable consequence?

PURPOSE: The purpose of this study was to compare clinical outcomes and development of post-infectious arthritis of the shoulder joint after single (Group S) or repeated (Group R) arthroscopic debridement surgeries. We hypothesized that (1) repeated surgeries might be attributable to initial subchondral involvement resulting from septic arthritis and (2) the repeated surgery group would exhibit a higher incidence of post-infectious arthritic changes and inferior clinical outcomes due to prolonged resolution of the infection. METHODS: This study included 42 patients (single surgery, n = 29; re…

Neer Award 2015: Analysis of cytokine profiles in the diagnosis of periprosthetic joint infections of the shoulder.

BACKGROUND: Periprosthetic joint infection (PJI) after shoulder arthroplasty can present a diagnostic and therapeutic challenge. This study evaluated the diagnostic utility of broader synovial fluid cytokine analysis for identifying PJI in patients undergoing revision shoulder arthroplasty. METHODS: Synovial fluid levels of 9 cytokines (interleukin [IL] 6, granulocyte-macrophage colony-stimulating factor, IL-1β, IL-12, IL-2, IL-8, interferon-γ, IL-10, and tumor necrosis factor-α) were measured in 75 cases of revision shoulder arthroplasty with a multiplex immunoassay. Cases were classified in

Effect of isolated hyperglycemia on native mechanical and biologic shoulder joint properties in a rat model.

Recently, diabetes has been linked to rotator cuff disease and adhesive capsulitis, conditions with increased stiffness and inflammation. Unfortunately, limited research exists examining how hyperglycemia affects the native shoulder (tendon and capsule) properties. Therefore, the objectives of this study were to compare shoulder joint mechanics, tendon properties (mechanics and immunohistochemistry), and capsule of healthy control and hyperglycemic rats 8 weeks following induction of hyperglycemia with a submaximal dose of streptozotocin (STZ). Eighteen rats were injected with STZ to induce hy…

Kinematic impact of size on the existing glenohumeral joint in patients undergoing reverse shoulder arthroplasty.

BACKGROUND: Glenohumeral relationships in reverse shoulder arthroplasty patients have not been previously reported. The purpose of this study was to quantify and compare the shoulder spatial relationships and moment arms. Measurements were used to define general size categories and determine if sizes scale linearly for all metrics. METHODS: Ninety-two shoulders of patients undergoing primary reverse shoulder arthroplasty for functionally-deficient massive rotator cuff tear without bony deformity or deficiency were evaluated using three-dimensional CT reconstructions and computer-aided design

Lack of effect of corticosteroid injection at the shoulder joint on blood glucose levels in diabetic patients.

The effect of systemic glucocorticosteroids on the metabolism of glucose is well known; however, there are no reports on the effect of intraarticular steroids on the metabolism of glucose in diabetic patients. Controlled or near controlled diabetic patients who have self-monitoring devices for home monitoring of blood glucose with shoulder pain were offered an intraarticular crystalloid steroid injection of 35 mg of methylprednisolone acetate (MPA) at the shoulder joint after failure of pharmacological and physical therapy. Patients were asked to record blood glucose levels before and 2 h afte…

[Value of diagnostic measures in soft tissue diseases and soft tissue lesions of the shoulder joint].

Calcifying tendinitis, acute tendinitis, frozen shoulder, rotator cuff rupture, subluxation of the gleno-humeral joint and injury of the biceps tendon are commonly classed under the blanket term "peri-arthropathy of the shoulder". It is now possible to make a precise diagnosis of these shoulder disorders by means of a clinical examination supported by X-ray examination, ultrasonography, arthrography, computed tomography (CT), magnetic resonance imaging (MRI) and arthroscopy. Calcifying tendinitis is diagnosed by consideration of the patient's history, followed by clinical examination and X-ray

Carbon fiber replacement of the ligaments of the shoulder girdle and the treatment of lateral instability of the ankle joint.

A normal active human being will not accept chronic instability or persistent luxation of any joint. Many attempts have been made to stabilize a damaged joint with autogeneic and allogeneic materials and by various surgical techniques. In animal and clinical trials, carbon fibers are reported to be a promising material for ligament replacement in chronic joint instability. The authors are investigating carbon fibers not only for repair of chronic instability of knee joints but also for chronic luxations and subluxations of the sternoclavicular, acromioclavicular, and ankle joints. For long-ter

The effect of pre- and postmastectomy radiotherapy on the degree of edema, shoulder-joint mobility, and gripping force.

Women undergoing mastectomy for cancer were assigned at random to three groups, each with about 310 patients. In one group, radiotherapy was given before the operation, and in the second group, it was given after the operation. In the third group, no radiotherapy was given. About 160 patients from each group were examined for the degree of edema of the arm, mobility of the shoulder--represented by 5 indices--and gripping force. Except for gripping force, the findings were significantly better for the group not receiving radiotherapy than for the groups given irradiation. There was no statistic

Effect of thoracic expansion restriction on scapulothoracic and glenohumeral joint motion during shoulder external rotation.

BACKGROUND: Shoulder external rotation in the throwing motion involves movement of the scapulothoracic and glenohumeral joints, thoracic spine, and the thorax. Restriction of thoracic expansion may decrease scapulothoracic joint motion and compensate by excessive glenohumeral joint motion. However, it is unclear how restricting the expansion of the thorax alters shoulder motion. OBJECTIVE: To elucidate changes in scapulothoracic and glenohumeral joint movements caused by restricted thoracic expansion. METHODS: Kinematic data were obtained using an electromagnetic tracking device (Liberty; Po

The association between anterior shoulder joint capsule thickening and glenoid deformity in primary glenohumeral osteoarthritis.

BACKGROUND: Anterior shoulder joint capsule thickening is typically present in osteoarthritic shoulders, but its association with specific patterns of glenoid wear is incompletely understood. We sought to determine the relationship between anterior capsular thickening and glenoid deformity in primary glenohumeral osteoarthritis. METHODS: We retrospectively identified 134 consecutive osteoarthritic shoulders with magnetic resonance imaging and computed tomography scans performed. Axial fat-suppressed magnetic resonance imaging slices were used to quantify the anterior capsular thickness in mil…

Implant sonication versus intraoperative tissue sample cultures for Periprosthetic Joint Infection (PJI) of Shoulder Arthroplasty.

Introduction Periprosthetic joint infection (PJI) is the most problematic complications after shoulder arthroplasty. Many diagnostic tools have been identified to find infection, such as hystopatologic examination of tissue sections or cultures of intraoperative tissue. Implant sonication fluid culture showed good results in order to enhance diagnostic accuracy, but literature results are still controversial. Aim of our study is to compare the results of sonication with intraoperative tissue sample cultures. Patients and Methods From February 2016 to January 2018 we performed 102 revisions of
