Metallosis is an unusual but consequential complication arising from orthopedic hardware implantation, characterized by the deposition of metallic particles in the periprosthetic soft tissues. The incidence of metallosis associated with shoulder arthroplasties is exceptionally rare since the shoulder is not a weight-bearing joint, making it less susceptible to mechanical wear and, consequently, to conditions like particle disease and metallosis. Nevertheless, anomalous metal-on-metal interactions can develop in total shoulder arthroplasties if the polyethylene component fails due to wear, frac

Avascular necrosis (AVN) is a relatively uncommon cause of glenohumeral arthritis. Previous retrospective reviews of shoulder arthroplasty for AVN have shown very good results in small numbers of patients. This study prospectively evaluated a consecutive series of 21 shoulders in 19 patients treated with the same modular prosthesis at a single institution. Of the shoulders, 8 developed AVN after a proximal humeral fracture, 1 was associated with a massive rotator cuff tear, 10 developed after corticosteroid therapy, and 2 were idiopathic. The patients, 14 women and 5 men, with a mean age of 54

To evaluate movement quality of upper limb (UL) prosthesis users, performance-based outcome measures have been developed that examine the normalcy of movement as compared to a person with a sound, intact hand. However, the broad definition of "normal movement" and the subjective nature of scoring can make it difficult to know which areas of the body to evaluate, and the expected magnitude of deviation from normative movement. To provide a more robust approach to characterizing movement differences, the goals of this work are to identify degrees of freedom (DOFs) that will inform abnormal movem

UNLABELLED: Many glenohumeral studies have demonstrated how anatomy varies across the population. Third and fourth generation shoulder prosthesis designs were developed to provide greater modularity and size ranges to better reproduce this anatomy and thus achieve better results in terms of shoulder function. This study quantifies the quality of anatomic reconstruction and compares that to long-term clinical outcomes using one fourth generation platform shoulder system.
METHODOLOGY: One hundred and forty primary total shoulder arthroplasties were performed by one experienced single surgeon be…

BACKGROUND: It remains unclear whether altered bone density may be a stability issue in stemless reverse shoulder arthroplasty (RSA).
METHODS: This was a retrospective multicentric study. Patients with stemless RSA with two-year postoperative shoulder X-ray and clinical data were included. Measurement of proximal humeral cortical bone thickness (CBT avg) on postoperative radiography was used to assess bone density.
RESULTS: 68 patients (mean age, 67.0 years) were included. CBT avg < 6 mm (osteoporosis) at two years was reported in 22.2% (2/9) of patients with humeral loosening and 35.6% (21/

BACKGROUND: Comminuted radial head fractures are challenging to treat with open reduction and internal fixation. Radial head arthroplasty is a favourable technique for the treatment of complex radial head fractures. The purpose of this study was to evaluate the functional and radiological outcomes of radial head arthroplasty using modular pyrocarbon radial head prosthesis.
METHODS: We retrospectively reviewed 21 consecutive patients requiring radial head arthroplasty for unreconstructible radial head fractures between July 2003 and July 2009. Patients completed a Short Form-36 (SF-36), the Di

BACKGROUND: Patient-reported outcomes (PROs) can be used to evaluate perceived capacity of an individual in executing tasks in a natural environment with their prosthetic device. According to the World Health Organization International Classification of Health, Functioning, and Disability (ICF) models, there may be specific factors of a person, factors of assistive prosthetic technology, or factors related to the health condition or body function that affect their functioning and disability. However, an understanding of factors affecting an upper limb prosthesis user's perception of their abil

Pyrocarbon hemiarthroplasty is a novel procedure that has shown promise for the complex cases of young patients with severe shoulder arthritis. This prosthesis aims to overcome challenges faced by conventional hemiarthroplasty designs, particularly the issue of significant glenoid arthrosis over time. Results of the pyrocarbon hemiarthroplasty have generally shown good promise, but there has been a scarcity in studies and reports that explore the long-term outcomes of this procedure. In this report, we describe the case of a young male patient who underwent a pyrocarbon hemiarthroplasty for si

The increased utilization of shoulder arthroplasty, including revision procedures, combined with rises in life expectancy, is expected to translate into a substantial increase in periprosthetic humeral fractures. The evaluation and management of these fractures needs to be updated to consider fractures that complicate anatomic and reverse arthroplasties and contemporary short-stem and stemless implants. Although conservative treatment is successful in a large proportion of these fractures, several surgical reconstructive techniques are required for the management of all fracture types. Surgica

INTRODUCTION: This study documents 2-year clinical and radiographic results following reversed total shoulder arthroplasty using a novel prosthesis with inverted bearing materials (polyethylene glenoid; metal humeral component). This design was intended to avoid massive PE abrasion on the humeral side. Therefore, we predicted a lack of subsequent osteolysis-induced exacerbation of scapular notching, and because of other design features and modified operating technique a reduced notching rate.
MATERIALS AND METHODS: An ongoing, prospective, international, multicenter study of patients implante

Prosthetic joint infection is a devastating complication following shoulder arthroplasty that can lead to pain, poor function, and poor quality of life. With the increasing number of shoulder arthroplasties performed annually, recognition of prosthetic infection and treatment is necessary. The skin surrounding the shoulder has a unique microbiome, and Cutibacterium acnes is the most commonly encountered bacteria causing prosthetic joint infection. C acnes is a low-virulence organism that resides in the subcutaneous layer of the skin. As a result, the clinical presentation is typically less obv

BACKGROUND: Revision arthroplasty for failed post-traumatic humeral head replacement associated with rotator cuff and glenoid deficiency is challenging. Current surgical solutions are fraught with complications, and no best-practice strategy has been established. We hypothesized that the computer-assisted design/computer-assisted manufacturing (CAD/CAM) shoulder (Stanmore Implants, Elstree, UK), a total shoulder design resembling a total hip prosthesis, can offer a reliable alternative in this surgically challenging subset of patients with rotator cuff deficiency and advanced glenoid bone loss

Nineteen patients with 20 severe proximal humeral fractures were treated early with a Neer humeral prosthesis from 1979 to 1986. There were 18 four-part and two three-part fractures. The average age of the patients at the time of injury was 64 years. Follow-up evaluation was performed at an average of 40 months. Pain relief was satisfactory in 18 of 20 fractures. Patients were satisfied with the result in 16 of the 20 shoulders. Functional evaluation was good or fair in 15 of 20 patients. Active forward elevation averaged 72 degrees and active external rotation 16 degrees. Complications consis

PURPOSE: Vitamin D is increasingly being recognized as an important mediator of immune function and may have a preventive role in the pathogenesis of periprosthetic joint infection. To the best of our knowledge, no other study has examined possible associations between periprosthetic joint infection and vitamin D deficiency. We investigated the rate of vitamin D deficiency in patients treated for periprosthetic joint infection and whether vitamin D deficiency is independent of other risk factors for vitamin D deficiency in patients with periprosthetic joint infection.
METHODS: Serum 25-hydrox

Non traumatic pathology of the shoulder was broken down in four entities: (1) articular stiffness are best treated with rehabilitation and an arthroscopic arthrolysis is rarely recommended; (2) rotator cuff pathology is represented by calcifying and non calcifying tendinitis, partial and full thickness rotator cuff tears; surgical treatment is performed nowadays under arthroscopy; after 50 years old, surgery is indicated after failure of conservative treatment; (3) glenohumeral osteoarthritis and arthritis are best treated with prosthesis; the results are now comparable to hip arthroplasty; (4

OBJECTIVE: Total elbow arthroplasty (TEA) is a commonly performed surgical technique for the management of elbow disorders. The Coonrad-Morrey (CM) prosthesis is the most commonly used prosthesis in TEA. The study from Chinese cohorts remains limited, particularly regarding differences between patients with and without RA. Therefore, the purpose of this study was to evaluate the medium- to long-term clinical outcomes using the CM prosthesis in a Chinese cohort, and to compare clinical outcomes between patients with and without RA.
METHODS: A retrospective cohort study was conducted involving

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has increasingly become a reliable intervention to improve function and pain for a variety of indications. Glenoid fixation is critical to the success of rTSA, and traditionally the standard anatomic centerline is used, allowing for central baseplate fixation. However, in cases of severe bone loss, baseplate fixation along the alternative centerline has been described. The baseplate axis is anteverted and inferiorly tilted, allowing for fixation into a column of bone where the base of the scapular spine and coracoid meet, thereby prioritiz

Prosthesis users often experience muscle fatigue and reduced control due to the weight of the device, contributing to high abandonment rates. This study investigates the effects of integrating a soft exoskeleton with a myoelectric prosthesis on upper-limb muscle fatigue and user experience. Nine non-disabled participants performed four functional tasks: drinking from a cup, using a Fork, lifting a box, and reaching overhead, using the prosthesis alone and in combination with the exoskeleton. Muscle activity was recorded via surface electromyography, and perceived exertion was measured using th
