Selective reconstructive options for the anterior skull base.

Carcinomas of the ethmoid, frontal, or maxillary sinuses sometimes invade the anterior skull base. It is necessary to perform en-bloc resection for this invasive carcinoma according to the concepts of surgical treatment for head and neck cancer. The anterior skull base consists of two parts, the orbital roof as the lateral portion and the roofs of the frontal sinus, ethmoid sinus, and/or sphenoid sinus as the central portion. Selective reconstructive options for the anterior skull base depend on the size of the defect of the skull base. A dural defect is repaired by a fascia lata or a pericran

Use of a galeopericranial flap for the reconstruction of anterior cranial base defects.

BACKGROUND: To evaluate the efficacy of using a galeopericranial flap for reconstruction of anterior cranial base defects. METHODS: In Linkou Chang Gung Memorial Hospital from February 1994 to November 2003, 25 patients who had tumors of the skull base underwent craniofacial resection, and a galeopericranial flap was used to reconstruct the anterior cranial base defect. The galeopericranial flap was developed and based on at least 1 side of the supraorbital or supratrochlear arteries and veins; it was only raised from the scalp after the tumor had been removed. It was then turned inwardly and

Olfactory Carcinoma, Similar yet Distinct to Olfactory Neuroblastoma: A Case Report and Literature Review.

INTRODUCTION: Olfactory carcinoma (OC) is a rare tumor that is often misdiagnosed as olfactory neuroblastoma (ONB). Due to its rarity, there is no established standard treatment for OC, and there have been no reports of successful long-term follow-up treatment yet. CASE PRESENTATION: Two male patients, aged 54 and 48 years, presented with similar symptoms of nasal obstruction, anosmia, facial swelling, and epistaxis. Imaging in both cases revealed large, locally advanced sinonasal neoplasms. Initial biopsies were concerning for ONB; however, final pathological analyses confirmed high-grade OC

Carotid Space Mass Proximal to Vagus Nerve Causing Asystole and Syncope.

Manipulation of vagal nerve rootlets, whether surgical or through mass effect of a neoplasm, can result in asystole and hypotension, accompanied by ST depression and right bundle branch block. There are few case reports of a neoplasm causing these effects, and this case describes a patient with such a mass presenting with syncopal episodes. A 43-year-old man with a past medical history of HIV, bipolar disorder, and epilepsy was admitted to the neurology service for a video electroencephalogram (vEEG) to characterize syncopal episodes that were felt to be epileptic in origin. During the study,

Outcome and analysis of the surgical management of esthesioneuroblastoma.

OBJECTIVE: The aim of this study was to evaluate the effectiveness of combined radical surgical resection and radiotherapy for the treatment of esthesioneuroblastoma, a rare malignant neoplasm accounting for 1 to 5% of malignant neoplasms of the nasal cavity. METHOD: A 10-year review of 12 patients treated at our institution suggests that primary treatment consisting of a combined otolaryngologic/neurosurgical craniofacial approach results in good local control with relatively low morbidity and complication. Patients tend to present with advanced stage disease because of the nonspecific prese

[Vagal paraganglioma].

Vagal paragangliomas are rare tumors of neural crest origin. This neoplasm is generally located at various points along the peripheral distribution of the vagus nerve, in the region of the jugular and nodose ganglia, just beneath the perineurium of the nerve, but may arise anywhere along the course of the vagus nerve. Histologically, vagal paragangliomas are similar to carotid body tumors, and occasionally can undergo malignant transformation and metastasize (10-19%) or infiltrate the base of the skull. Selective arteriography can distinguish vagal paraganglioma from carotid body tumor, becaus

Concept of robotic gamma knife microradiosurgery and results of its clinical application in benign skull base tumors.

The availability of advanced computer-aided robotized devices for the Gamma Knife (i.e., an automatic positioning system and PerfeXion) resulted in significant changes in radiosurgical treatment strategy. The possibility of applying irradiation precisely and the significantly improved software for treatment planning led to the development of the original concept of robotic Gamma Knife microradiosurgery, which is comprised of the following: (1) precise irradiation of the lesion with regard to conformity and selectivity; (2) intentional avoidance of excessive irradiation of functionally importan

Predicting epilepsy in patients diagnosed with intracranial meningiomas: A systematic review and meta-analysis of clinical and anatomical risk factors.

INTRODUCTION: Meningiomas comprise 40% of all central nervous system neoplasms, with peak prevalence occurring in the fifth and sixth decades of life. Presenting symptoms vary between patients and are location-dependent, but new-onset seizures bring a subset of patients to clinical attention. OBJECTIVE: The present meta-analysis aims to identify the clinical features and tumor characteristics most associated with seizures at diagnosis of intracranial meningioma. METHODS: The PubMed, Scopus, and Embase databases were queried for all primary retrospectives (case-control studies, cohort studies

P14.05 How far can they grow... - Two clinical examples

Abstract Introduction: Cerebral metastases are the most common form of central nervous system (CNS) tumours in adults. However, malignant neoplasm may also involve structures external to the brain, such as tissue surrounding the base of the skull, and then metastasize to the brain either by direct invasion or by spreading by the cranial nerves. CASE1: A 74 year-old man, with a past history of chronic kidney disease due to renal artery thrombosis and hypertension, was admitted in the emergency room (ER) complaining of persisting pain in the superior half of the right hemiface and frontal region

Remitting sixth nerve palsy in skull base tumors.

OBJECTIVE: Spontaneous recovery of a sixth nerve palsy is thought to rule out a neoplastic origin. We reviewed cases of sixth nerve palsy that improved without treatment but that ultimately proved to be caused by a tumor at the base of the skull. DESIGN: Case series. SETTING: Hospital-based, neuro-ophthalmology referral practice. PATIENTS: Seven patients with an age range from 7 to 61 years had sixth nerve palsy secondary to a slow-growing neoplasm at the skull base. MAIN OUTCOME MEASURES: Return of lateral rectus function and resolution of diplopia without intervention. RESULTS: Seven pa

[Endoscopic Endonasal Surgery for Skull Base Chordomas and Chondrosarcomas : Basic to Advanced Cases].

Skull base chordomas and chondrosarcomas are rare bone tumors predominantly found in the petroclival areas and involve the cranial nerves, internal carotid arteries, and brain stem. Their clinical aggressiveness and high risk of recurrence necessitate multimodal treatments, including extensive surgical resection and high-dose radiotherapy. Surgery is commonly the first step in their treatment, and ABCD rules should be respected; anatomical preservation of the innocent nasal anatomy for potential repeated surgeries in case of recurrence, block-by-block resection, coverage of the intradural resi

Giant intracerebral schwannoma of the skull base misinterpreted as a macroprolactinoma.

Mucopolysaccharidosis (MPS) is a group of inherited metabolic diseases, characterized by defects in the degradation of glycosaminoglycans and their accumulation in lysosomes. However, various secondary cellular changes also contribute to the pathomechanism of MPS. Previous studies have reached contradictory conclusions about the changes in mitochondria in MPS, from increased numbers of mitochondria to impaired activities of some mitochondrial respiratory chain enzymes to no changes in mitochondrial respiration. In this preliminary, hypothesis-generating study, mitochondrial network morphology

MRI and CT of tumors of the pharynx: comparison of the two imaging procedures including fast and ultrafast MR sequences.

The aim of this prospective study was to compare the diagnostic value of computed tomography (CT) and magnetic resonance imaging (MRI) in the evaluation of tumors of the nasopharynx, oropharynx and hypopharynx. For this reason, we performed CT and MRI in a total of 473 patients with neoplasms of the nasopharynx [116], oropharynx [282] and hypopharynx [39]. Furthermore, the MR imaging protocol had to be optimized including 2D and 3D gradient echo sequences. We compared the use of conventional and fast spin echo and 3D gradient echo sequences for the evaluation of tumors of the nasopharynx and t

Landscape of immune cell gene expression is unique in predominantly WHO grade 1 skull base meningiomas when compared to convexity.

Modulation of tumor microenvironment is an emerging frontier for new therapeutics. However in meningiomas, the most frequent adult brain tumor, the correlation of microenvironment with tumor phenotype is scarcely studied. We applied a variety of systems biology approaches to bulk tumor transcriptomics to explore the immune environments of both skull base and convexity (hemispheric) meningiomas. We hypothesized that the more benign biology of skull base meningiomas parallels the relative composition and activity of immune cells that oppose tumor growth and/or survival. We firstly applied gene c

A narrative review of targeted therapy in meningioma, pituitary adenoma, and craniopharyngioma of the skull base.

Management of solid tumors involving the skull base are primarily managed with surgery and radiation, though proximity to important vascular and neuroanatomic structures often limit the extent of resection and permissible radiation dose. Meningiomas are the most common primary brain tumor in adults, and although the majority of skull base meningiomas are low-grade, their location in proximity to critical anatomical structures precludes aggressive surgical resection, and larger tumors are often resistant to radiation treatment. In patients with clinically aggressive, unresectable meningiomas, s

Esthesioneuroblastoma.

Esthesioneuroblastoma is a rare malignant neoplasm in the olfactory region of the nasal cavity and anterior skull base. Diagnosis and staging require anatomic imaging and careful pathologic assessment. Standard treatment is anterior craniofacial resection with postoperative irradiation. The role for chemotherapy is not defined, but is generally for the most advanced cases and used in the neoadjuvant setting and/or postoperatively with irradiation. Prognosis is favorable; however, metastasis rates remain relatively high. Regional and distant metastasis portends a poor outcome. Intensity-modulat
