Cryptococcal myositis and vasculitis: an unusual necrotizing soft tissue infection.

BACKGROUND: Cryptococcus neoformans var. neoformans is an opportunistic yeast that typically infects immunocompromised patients. METHODS: A case report and review of the pertinent English-language literature are presented. RESULTS: Necrotizing vasculitis associated with cryptococcal invasion was identified in 1986. Until now, only 24 cases of cryptococcal cellulitis have been reported, including one case of cryptococcal necrotizing fasciitis and one case of necrotizing vasculitis. We report an unusual case of occult disseminated cryptococcosis presenting as necrotizing cellulitis, fasciitis,

[Contribution of linezolid to the treatment of complicated infections of the skin and soft tissues].

THREE GROUPS OF INCREASING SEVERITY: According to the 2000 consensus conference, bacterial infections of the skin and soft tissues can be classified in three categories of increasing severity: bacterial dermo-hypodermal infections, bacterial dermo-hypodermal necrotizing infections, necrotizing fasciitis. The last two categories are medicosurgical emergency conditions requiring repeated surgical debridement and probabilistic antibiotic therapy. LINEZOLID: The clinical efficacy of linezolid in bacterial infections of the skin and soft tissues is comparable to that of oxacillin/dicloxacillin. Sev

Soft tissue and cartilage infection by Salmonella oranienburg in a healthy girl.

Focal extraintestinal infections from nontyphoid salmonellae have increased in incidence during the past decade. Typically, they are manifested as either osteomyelitis or meningitis as a complication of either bacteremia or enteric fever. Isolated salmonellal soft tissue infections, however, are rare and occur mostly in adults with chronic underlying conditions such as human immunodeficiency virus (HIV) infection, diabetes mellitus, and cell-mediated immunity defects. We report a case of an otherwise healthy adolescent who was exposed to a guinea pig with a skin mass. She subsequently had an i

[Infection of deep soft tissues of the neck in intravenous drug abuse].

BACKGROUND: Despite the availability of antibiotics deep neck space infections with anaerobic germs (Ludwig's angina) still carry the potential for significant morbidity and mortality with delayed treatment. Usually, odontogenic infections with involvement of the submandibular space are the source of deep neck space infections in adults, whereas in the pediatric population the most common cause is acute tonsillitis with involvement of the peritonsillar space. During the second half of this century intravenous drug abusers appeared as a new group of patients at risk for deep neck space infectio

Surgical treatment of fracture-related infections of the calcaneus with relevant soft-tissue defects.

BACKGROUND: Fracture-related infection (FRI) of the calcaneus with associated soft tissue defects after open reduction and internal fixation (ORIF) presents significant therapeutic challenges. This study evaluates the efficacy of two filling techniques-bone cement with local flap transfer (BC+LFT) and free myocutaneous flap (FMF)-in managing these complex cases, with consideration of the FRI classification system. METHODS: A retrospective analysis on patients treated between June 2017 and June 2024 was done. Based on post-debridement defect size, patients were allocated to BC + LFT (defects ≤

Emergency department disposition and hospital-based resource utilization for skin and soft tissue infections.

BACKGROUND: Skin/soft tissue infections (SSTIs) are common emergency department (ED) presentations with substantial healthcare costs. When determining ED disposition, clinicians must balance risks of treatment failure against overusing hospitalization. Existing risk-stratification strategies target inpatient or outpatient subgroups and morbidity/mortality outcomes, leaving evidence gaps addressing the complete case mix and hospital-based resource utilization. METHODS: In this retrospective regional cohort of adult SSTI cases, the primary composite outcome captured hospital-based care requirem

Acute phase reactants in the diagnosis of soft tissue infections of the upper extremity.

BACKGROUND: Upper extremity soft tissue infections are commonly encountered in emergency department settings and often require timely interventions. Acute phase reactants, such as C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and white blood cell count (WBC), are commonly used in diagnostic workups. However, the utility of these biomarkers in the management of upper extremity infections remains uncertain. This study evaluates the diagnostic value of these acute phase reactants in a broad cohort of patients requiring intervention for upper extremity infections. RESULTS: A ret

Risk Identification and Mitigation of Skin and Soft Tissue Infections in Military Training Environments.

OBJECTIVE: Staphylococcus aureus (SA), including methicillin-resistant strains (MRSAs), is a major cause of skin and soft tissue infections (SSTIs) in military populations. This study investigated SSTI incidence and SA carriage in a military training site over 16 weeks using a prospective observational cohort design. METHODS: Two training cohorts provided pre- and post-training self-collected swabs for bacterial carriage, and environmental swabs from accommodations, personal items, and training facilities. Hygiene awareness and practices were assessed through questionnaires. Bacteria were ide

Penicillin susceptibility among Staphylococcus aureus skin and soft tissue infections at a children's hospital.

Shortly after its introduction into clinical practice, Staphylococcus aureus isolates gained resistance to penicillin via the acquisition of β-lactamases. A number of centers have recently described an increase in the proportion of invasive methicillin-susceptible S. aureus (MSSA), which are also susceptible to penicillin (PSSA). Little data are available regarding the prevalence or impact of PSSA in skin and soft tissue infections (SSTI). Community-acquired MSSA SSTI isolates were obtained through a surveillance study at Texas Children's Hospital from January 2017 to December 2021. A total of

Periorbital necrotizing sweet syndrome: A report of two cases mimicking necrotizing soft tissue infections.

PURPOSE: Two cases are described of necrotizing Sweet syndrome (nSS), a rare variant of acute febrile neutrophilic dermatosis that mimics necrotizing soft tissue infections. OBSERVATION: A 74-year-old female with myelodysplastic syndrome (MDS) presented with isolated periorbital nSS that closely mimicked necrotizing fasciitis (NF); she displayed pathergy to debridement, was exquisitely responsive to corticosteroids, and underwent successful first-stage reconstruction of the eyelid with full-thickness skin grafting. Repeated Hemolytic Streptococcal Infections in Soft Tissue Eosinophilic Granulomatosis (Kimura's Disease): A Case Report.

Soft tissue eosinophilic granulomatosis (Kimura's disease) is an eosinophilic granuloma with a proliferation of lymphoid follicles in the subcutaneous soft tissue. Although no established treatment exists, it is considered a disease with a good prognosis. On the other hand, bacteremia caused by group G hemolytic streptococcus (GGS) is said to be caused by chronic local lymph abnormalities and is likely to recur. We present the case of a 41-year-old Japanese man. He had a history of treatment for Kimura's disease and sepsis due to hemolytic streptococcus and came to our hospital with a chief co

Systemic immune activation profiles in streptococcal necrotizing soft tissue infections: A prospective multicenter study.

OBJECTIVE: Early stages with streptococcal necrotizing soft tissue infections (NSTIs) are often difficult to discern from cellulitis. Increased insight into inflammatory responses in streptococcal disease may guide correct interventions and discovery of novel diagnostic targets. METHODS: Plasma levels of 37 mediators, leucocytes and CRP from 102 patients with β-hemolytic streptococcal NSTI derived from a prospective Scandinavian multicentre study were compared to those of 23 cases of streptococcal cellulitis. Hierarchical cluster analyses were also performed. RESULTS: Differences in mediator

Skin and soft tissue infections, active component, U.S. Armed Forces, January 2016-September 2020.

During 1 January 2016-30 September 2020, there were 210,914 incident cases of skin and soft tissue infections (SSTIs) among active component U.S. military members, corresponding to a crude overall incidence rate of 352.8 per 10,000 person-years (p-yrs). An additional 4,250 cases occurred in theaters of operations (251.0 per 10,000 p-yrs). Of the total incident SSTI diagnoses, 64.5% were classified as cellulitis/abscess, 30.0% were "other SSTIs" (e.g., folliculitis, impetigo), 5.3% were carbuncles/furuncles, and 0.2% were erysipelas. Crude annual incidence rates declined by 21.9% over the surve

Substance Abuse-Related Soft Tissue Infections: A Uniquely Complicated Subset of a Common Problem.

We hypothesize that soft tissue infections (SSIs) related to intravenous drug usage (IVDU) are associated with a more complicated and costly course than those not associated with IVDU. For the period 2005-2018, ICD 9/ICD 10 billing codes were used to identify patients admitted with SSIs and their causes/complications and associated costs. IVDU-related infections were then compared with non-IVDU-related infections. t test was used to compare treatment costs and length of stay. Logistic regression analysis was used to assess the likelihood/risk of specific events in the IVDU versus non-IVDU popu

Gram-negative bacteria as emerging pathogens affecting mortality in skin and soft tissue infections.

INTRODUCTION: Skin and soft tissue infections (SSTIs) are commonly encountered in clinical practice and mainly caused by gram-positive cocci such as S.aureus and β-hemolytic streptococci. Complicated SSTIs involving deeper tissues often necessitate surgical intervention and occur in patients with significant comorbidities such as diabetes or immunocompromising conditions. METHODS: In this study, we retrospectively reviewed the epidemiology, clinical characteristics, microbiology, and treatment of patients admitted with SSTI during a five-year period in the Internal Medicine Department of a te

Guidelines vs Actual Management of Skin and Soft Tissue Infections in the Emergency Department.

BACKGROUND: Infections of skin and soft tissue (SSTI) commonly cause visits to hospital emergency departments (EDs). The Infectious Diseases Society of America (IDSA) has published guidelines for the management of SSTI, but it is unclear how closely these guidelines are followed in practice. METHODS: We reviewed records of patients seen in the ED at a large tertiary care hospital to determine guidelines adherence in 4 important areas: the decision to hospitalize, choice of antibiotics, incision and drainage (I&D) of abscesses, and submission of specimens for culture. RESULTS: The decision to
