A 20-year-old male with craniomandibular deformity and almost total visual loss of the right eye due to bilateral optic atrophy underwent cerebral nonenhanced computed tomography (NECT) examinations. He had multiple sphenoidal-temporoparietal meningiomas, with adjacent hyperostoses and distant calvarial hyperdense lesions, as well as sclerotic expansion of right mandibular condyle. History, clinical and imaging findings were suggestive of associated fibrous dysplasia (FD), which explained the visual loss due to optic nerve compression through sphenoidal optic foramens. Cranial and mandibular F

Venous malformations have previously been reported to be associated with skeletal changes in humans, typically demineralization and localized deformation of the long bones. We report a presumed developmental association between venous malformations of the head and neck and meningoencephaloceles involving the sphenoid and temporal bones. Recognition of this association is important to avoid misinterpretation of these changes as a more aggressive process. We present the imaging findings and suggest an embryologic basis for this previously unreported association.

We present the case of an 11-year-old female patient with tuberous sclerosis who had a right nasal mass. CT examination revealed fibrous dysplasia involving the frontal, ethmoid, sphenoid, and vomer bones. Biopsy findings of the mass confirmed this diagnosis, and follow-up revealed marked expansion of these lesions. The authors emphasize the association of bone abnormalities and tuberous sclerosis and discuss the consideration of fibrous dysplasia as a component of this syndrome.

The postnatal growth of the nasal bones, the piriform aperture, the choanae, the anterior and posterior nasal spine, and the floor of the nasal cavity were investigated. The development of weight and width of the nasal cavity as much as some results of the postnatal development of the inferior and medical conchae are connected with out investigations. Moreover the increase of the distance between Nasospinale and the sphenoidal sinus aperture and also the development of height and width of the nasal crista of the palatine bone were determined.

The increase in the level of neutrophils following subclinical Cushing's disease is an uncommon clinical phenomenon that is characterized by insignificant biochemical or clinical evidence of hypercortisolism. In this study, we reported a 37-year-old female patient who presented with palpitations and fatigue, and showed increased neutrophils that were unaffected by anti-infection therapy. The patient was suspected of having a urinary tract infection because of occasionally with urinary frequency, urgency, increased procalcitonin, leukocytosis, and an increased proportion of neutrophils. INTRODUCTION: Fibrous dysplasia (FD) is a benign, idiopathic skeletal disorder, in which the normal bone and marrow are replaced by fibrous tissue and immature woven bone. The authors aim to analyze the long-term results of patients diagnosed with FD of the paranasal sinuses via imaging or surgical biopsy.
METHODS: The authors retrospectively enrolled 21 patients with FD of the paranasal sinuses from 2006 to 2021.
RESULTS: The mean age at the time of diagnosis of FD of the paranasal sinuses was very high at 45.9 years. The most common site ofoccurrence was the sphenoid sinus (71.4%). During

Reactive hyperostosis of spheno-orbital meningiomas (SOMs) often occurred in the sphenoid wing, while osteolytic SOMs (O-SOMs) were rarely discussed. This study preliminarily evaluated the clinical characteristics of O-SOMs and analyzed prognostic factors affecting the recurrence of SOMs. We retrospectively analyzed the medical records of consecutive patients who underwent surgery for a SOM between 2015 and 2020. According to the bone changes of sphenoid wing, SOMs were divided into O-SOMs and hyperostosis SOMs (H-SOMs). A total of 31 procedures were performed in 28 patients. All cases were tr

BACKGROUND: Only 2% to 6% aneurysmal bone cyst (ABC) involve the cranial region, and even fewer show sphenoid and orbital involvement. The spheno-orbital ABC is prone to misdiagnosis and can result in intraoperative bleeding and residual lesions. The study was to summarize the clinical and therapeutic characteristics of patients with spheno-orbital ABC in children.
METHODS: We retrospectively analyzed three childhood-onset spheno-orbital ABC cases at our hospital. A literature review was conducted and the spheno-orbital ABC cases were retrieved. The characteristics of clinical manifestations,

Solitary plasmacytomas are localized single tumors of monoclonal plasma cells that occur in two variants: solitary plasmacytoma of bone and extraosseous plasmacytoma. Solitary plasmacytoma of bone accounts for only 1%-2% of plasma cell lesions, and extraosseous plasmacytoma is also approximately 1%. These are both very uncommon at the skull base. We report two cases of plasmacytic neoplasms at the skull base where the differential diagnosis included pituitary adenoma when the tumor was sellar/suprasellar, or other bone-related tumors such as chordoma/chondrosarcoma when clivus/sphenoid bones w…

An anatomic approach is developed to sphenooccipital synchondrosis in newborn infants and nurslings. After removal of the squamous portion of the occipital bone by the method of Medvedev, opening of the cerebrospinal canal, removal of the spinal cord and hypophysis, two parallel cuts converging at an angle of approximately 90 degrees were made with a dissecting blade saw along both sides of sella turcica towards the anterior edge of the great occipital foramen to transect the sphenoid bone and main part of the occipital bone. The resulting osteochondrous fragment showed up defects in synchondr…

OBJECTIVE: To study the anatomico-radiological aspects of ossified pterygospinous ligament in human skulls.
BACKGROUND: The pterygospinous ligament is usually attached to spine of the sphenoid and the posterior border of the lateral pterygoid plate. It may sometimes be ossified.
MATERIALS AND METHODS: The lateral pterygoid plate of the sphenoid bone and the presence of ossified pterygospinous ligament was studied in 50 dried bone specimens and photographed. A skiagram of the ossified pterygosphenoid ligament was also obtained.
RESULTS: Out of 50 specimens studied, we observed the presence o…

BACKGROUND: Septoplasty is one of the most common otolaryngological operations. It is often dismissed as a simple procedure, despite the wide range of potential complications. We describe the first reported case of unilateral hemiplegia as a complication of septoplasty.
METHODS AND RESULTS: A 51-year-old man presented with right hemiplegia following a septoplasty and turbinoplasty procedure carried out elsewhere. Cranial imaging showed a breakthrough fracture of the left sphenoid sinus anterior wall and clivus, with a haemorrhagic area in the left paramedian pons, which was responsible for th

INDICATIONS CORRIDOR AND LIMITS OF EXPOSURE: The endoscopic endonasal transpterygoid approach (EETPA) provides direct access to the petrous apex, lateral clivus, inferior cavernous sinus compartment, jugular foramen, and infratemporal fossa. 1,2 In the coronal plane, it provides exposure far beyond a traditional sphenoidotomy.
ANATOMIC ESSENTIALS NEED FOR PREOPERATIVE PLANNING AND ASSESSMENT: The pterygoid process of the sphenoid bone forms the junction between the body and greater sphenoid wing before bifurcating because it descends into medial and lateral plates. The key to this exposure li…

The roof of the parapharyngeal space (PPS) is poorly defined. Although it is generally described as having prestyloid and poststyloid compartments, we believe that these terms are imprecise. Therefore, we define its boundaries, partition, and compartments. We completed macroanatomical and microanatomical dissections in 10 specimens from 5 human cadaver heads; bone measurements in 50 dry skulls; and axial and coronal cross-sectional studies in 2 cadaveric specimens. The PPS roof is bordered laterally by the medial pterygoid fascia and medially by the pharyngobasilar fascia. The tensor veli pala

BACKGROUND: Surgical management of extensive skull base tumors, such as chordoma and chondrosarcoma, remains very challenging. The need for gross total removal to improve survival must be weighed against the risk of injury to neurovascular structures and the loss of stability at the craniovertebral junction. In cases of tumors that are already compromising craniovertebral junction stability, the occipital condyle can be exploited as a deep keyhole to reach the clivus, petrous apex, and sphenoid sinus.
METHODS: We performed an anatomic study on 7 cadaveric specimens to describe the main landma

Background/Objectives: Identification of unknown individuals is an essential part of Forensic Medicine. Recent anthropological research has demonstrated subtle but consistent sexual dimorphism in the sella turcica, while indicating comparatively limited population-specific differences in its size, shape, and contour. The aim of this paper is to analyze the variability and sexual dimorphism of the sphenoid bone and to determine if there are statistically significant correlations between the sella turcica dimensions and the degree of pneumatization of the sphenoid sinus.
