Spine fractures in ankylosing spondylitis.

Patients with ankylosing spondylitis are susceptible to spine fracture, usually in the cervical spine. Less frequently, the thoracic and lumbar spine is affected. The fracture line may involve anterior and posterior elements. Frequently, it extends through the entire width of the spine. As a result the fracture tends to be unstable and may cause neurologic damage. Prompt immobilization and reduction of the dislocated spine followed by stabilization may prevent neurologic damage. We report a 45-year-old man who fell and sustained a fracture dislocation of L2 vertebra. The patient was operated a…

[Cervical spine injuries in children].

Cervical spine injuries in children are usually benign, severe traumatism being exceptional. The authors report 4 observations of severe traumatisms, 2 of which have required surgical treatment. Moreover they insist on the interest of acupuncture in the treatment of torticollis and of pain in the cervical spine after injury. The authors also point out the frequent aspect of anterior pseudo-subluxation C2-C3 which corresponds to a genuine medio-cervical physiological hinge.

Aneurysmal bone cyst of spine.

Four patients (aged 5, 7, 18, and 42) suffering from spinal cord involvement due to an aneurysmal bone cyst are reported. In two cases only a preoperative radiological diagnosis was made, one of them by a CT scan. Combined surgical and radiological treatments were done in three cases with a follow-up of 16 years in one patient and 9 years in the other two. In the fourth case, presenting with a C1 lesion, after radical excision and curettage X-ray therapy was not instituted. Pathological and radiological manifestations are discussed. Therapeutic measures are reviewed.

Birth defects involving the spine.

There is a multiplicity of birth defects of the spine occurring as isolated anomalies or as aspects of multifaceted syndromes. The clinical significance of these anomalies varies from the asymptomatic to the life threatening. Symptoms and signs may present at birth or not until years later. The optimal clinical result for the patient requires a careful history and physical examination, appropriate routine and specialized roentgenograms, laboratory data, and a high index of suspicion for signs or symptoms of neurological dysfunction. When the diagnosis is made, conservative or surgical treatmen…

Gender Disparities in the Spine Surgeon Workforce Performing Cervical Spine Surgery in the Medicare Population.

INTRODUCTION: Gender disparities persist across surgical specialties including orthopaedic surgery and neurosurgery, which may limit workforce diversity and potentially influence care delivery. Understanding representation among spine surgeons performing common procedures such as anterior cervical diskectomy and fusion (ACDF) may identify opportunities to enhance equity. METHODS: This was a retrospective cross-sectional study of spine surgeons performing ACDF using Medicare Provider Utilization and Payment Data from 2013 to 2021. The total representation of female spine surgeons performing AC…

Practice and principles of stereotactic body radiation therapy for spine and non-spine bone metastases.

Radiotherapy is the dominant treatment modality for painful spine and non-spine bone metastases (NSBM). Historically, this was achieved with conventional low dose external beam radiotherapy, however, stereotactic body radiotherapy (SBRT) is increasingly applied for these indications. Meta-analyses and randomized clinical trials have demonstrated improved pain response and more durable tumor control with SBRT for spine metastases. However, in the setting of NSBM, there is limited evidence supporting global adoption and large scale randomized clinical trials are in need. SBRT is technically dema…

From in vitro evaluation of a finite element model of the spine to in silico comparison of spine instrumentations.

Growth-preserving spinal surgery suffer from high complications rate. A recent bipolar instrumentation using two anchoring points (thoracic and pelvic) showed lower rates, but its biomechanical behaviour has not been characterised yet. The aim of this work was to combine in vitro and in vivo data to improve and validate a finite element model (FEM) of the spine, and to apply it to compare bipolar and classical all-screws implants. Spinal segments were tested in vitro to measure range of motion (ROM). Thoracic segments were also tested with bipolar instrumentation to measure ROM and rod strain …

The Utility of Cervical Spine Bracing As a Postoperative Adjunct to Multilevel Anterior Cervical Spine Surgery.

BACKGROUND: Use of cervical bracing/collar subsequent to anterior cervical spine discectomy and fusion (ACDF) is variable. Outcomes data regarding bracing after ACDF are limited. Here, we study the impact of bracing on short-term outcomes related to safety, quality of care, and direct costs in multilevel ACDF. METHODS: Retrospective cohort analyses of all consecutive patients undergoing multilevel ACDF with or without bracing from 2013 to 2017 was undertaken (n = 616). Patient demographics and comorbidities were analyzed. Tests of independence and logistic regressions were used to assess diff…

[Current Status of Spine Surgery in Germany: Results of a Survey of Members of the German Spine Society].

Background Mono- (MSS) and bisegmental spinal fusion (BSS) are frequently performed surgical interventions in German hospitals. Despite the high number of cases, only few data on treatment procedures and concepts for pre-, peri-, and postoperative care are available. The aim of the survey was to gain detailed insights into treatment procedures and medical approaches in elective MSS and BSS care in Germany in order to extract recommendations for improving patient care. Materials and Methods Members of the German Spine Society (DWG) were sent a newsletter, inviting them to participate in a surve…

Hip-Spine Syndrome: Is There an Association Between Markers for Cam Deformity and Osteoarthritis of the Lumbar Spine?

PURPOSE: To examine a large osteological collection to assess the relations between the well-described means of quantifying cam deformities of the proximal femur-alpha angle (AA) and anterior femoral neck offset (AFNO)-and osteoarthritis of the lumbar spine. METHODS: AA and AFNO were measured on paired femurs of 550 well-preserved cadaveric skeletons by use of standardized cephalocaudal digital photographs. Degenerative disease of these specimens' lumbar spines was graded from 0 to 4 with a validated grading system. Proximal femurs showing obvious arthritic changes such as lipping or osteophy…

[Influencing factors on the length of stay in lumbar spine surgery : analysis of the German spine registry].

INTRODUCTION: Spinal disc herniation, lumbar spinal stenosis and spondylolisthesis are known to be leading causes of lumbar back pain. The cost of low back pain management and related operations are continuously increasing in the healthcare sector. There are many studies regarding complications after spine surgery but little is known about the factors predicting the length of stay in hospital. The purpose of this study was to identify these factors in lumbar spine surgery in order to adapt the postoperative treatment. MATERIAL AND METHODS: The current study was carried out as a post hoc analy…

Harrington instrumentation and spine fusion for unstable fractures and fracture-dislocations of the thoracic and lumbar spine.

Of forty patients with unstable fractures and fracture-dislocations of the thoracic or lumbar spine treated with Harrington instrumentation and spine fusion, thirty-five had a neural deficit (twenty-three with incomplete or cauda equina lesions and twelve with complete lesions). Laminectomy or posterolateral decompression was performed prior to instrumentation and fusion in twenty-three patients and at the time of stabilization, in thirteen. Solid fusion was obtained in all but one patient. Back pain persisted in four. No patient had residual spinal deformity. Twenty-one patients with incomple…

Multirod Constructs in Spine Surgery.

➢ The use of multirod constructs (≥3 rods) in complex spine surgery has increased as its utility has been recognized over the past decade.➢ There are multiple different rod configurations that may be utilized on the basis of the desired supplemental rod function, with each type having its own advantages and clinical indications.➢ Literature has continued to demonstrate a reduced incidence of pseudarthrosis, rod fracture, and reoperation when comparing multirod constructs with traditional dual-rod constructs.➢ The use of consistent nomenclature when describing multirod constructs will allow for…
