Brachial plexus palsy and spinal subarachnoid pneumatosis following trauma.

The case of a 33-year-old male involved in a road traffic accident, resulting in brachial plexus palsy associated with subarachnoid pneumatosis, is described. A knowledge of the regional anatomy at the root of the neck is used to explain the patient's neurological signs and the mechanism of entry of air into the subarachnoid space.

Aggressive angioblastic meningioma with multiple sites in the neural axis.

This case report illustrates a patient who was found to have multiple sites of angioblastic meningiomas in the spinal and intracranial subarachnoid space 15 to 18 years after resection of an angioblastic meningioma from the upper cervical canal. The presence of the multiple sites of tumor could represent either multicentric origin or subarachnoid seeding.

The effect of subarachnoid erythrocyte lysate on brain injury: a preliminary study.

Abundant erythrocytes remain and lyse partially in the subarachnoid space after severe subarachnoid haemorrhage (SAH). But the effect of subarachnoid erythrocyte lysate on brain injury is still not completely clear. In this study, autologous erythrocytes (the non-lysate group) and their lysate (the lysate group) were injected separately into the cistern magna of rabbits to induce a model of experimental SAH, although the control group received isotonic sodium chloride solution instead of erythrocyte solution. Results showed that vasospasm of the basilar artery was observed at 72 h after experi…

[The use of nitrous oxide as a contrast medium during fractionated gas encephalography in halane-nitrous-oxide anesthesia].

40 children were employed as test cases in fractionated gas encephalography using Halan-nitrous oxide anesthesia, and the level of cerebrospinal pressure, the röntgenoparent quality of the extracerebral spaces and the duration of postencephalographical symptoms were examined using air and nitrous oxide as contrast media. In the course of the examination the cerebrospinal pressure showed a tendency to rise when air was introduced, to fall when nitrous oxide was introduced. After instillation of N2O however the filling of the subarachnoid space was over so quickly that it could not be included i…

Role of perivascular and meningeal macrophages in outcome following experimental subarachnoid hemorrhage.

The distribution and clearance of erythrocytes after subarachnoid hemorrhage (SAH) is poorly understood. We aimed to characterize the distribution of erythrocytes after SAH and the cells involved in their clearance. To visualize erythrocyte distribution, we injected fluorescently-labelled erythrocytes into the prechiasmatic cistern of mice. 10 minutes after injection, we found labelled erythrocytes in the subarachnoid space and ventricular system, and also in the perivascular spaces surrounding large penetrating arterioles. 2 and 5 days after SAH, fluorescence was confined within leptomeningea…

ACNU, MTX and 5-FU penetration of rat brain tissue and tumors.

The distribution of radio-labeled ACNU, MTX and 5-FU in brain and tumor tissue was studied in female Wistar rats by macroautoradiography after intrathecal administration. In normal rats, ACNU and 5-FU, administered intracisternally, distributed rapidly in the subarachnoid space, ventricular system and cerebrospinal fluid (CSF). 5-FU and MTX penetrated the brain deeply; the diffusional transport of ACNU was limited to a depth of 1 or 2 mm from the CSF surface of the brain. MTX and 5-FU clearance into the blood circulation was rather slow while ACNU cleared relatively quickly. The half time of A…

Choroid Plexus Hyperplasia : Report of Two Cases with Unique Radiologic Findings.

Choroid plexus hyperplasia (CPH), also known as diffuse villous hyperplasia of choroid plexus, is a rare condition characterized by excessiv … le ascites. In our study, we encountered two CPH patients who presented with significantly enlarged subarachnoid spaces, reduced parenchymal volume, and prominent choroid plexus. Initially, we treated these patients with a VP shunt, but eventually opted for endoscopic choroid plexus cauterization (CPC) to address the intractable ascites. Following the treatment with …

[Extraventricular obstructive hydrocephalus].

In extraventricular obstructive hydrocephalus (EVOH) there is an impaired outflow from the subarachnoid space usually at the level of the basal cisterns or arachnoid granulations. Subarachnoid haemorrhage, suppurative meningitis, and neoplastic or inflammatory exudates may fibrose or occlude the subarachnoid space. Following early reports that the response to shunt procedures was universally excellent, CSF shunting operations were performed on a large scale. Presence of the typical clinical triad, identified underlying aetiology, predominance of gait disorder, short duration of illness, absenc…

CT and MRI of ruptured intracranial dermoids.

Two patients with ruptured intracranial dermoids, examined with both CT and MRI are reported. Clinical presentation was transient cerebral ischemia in one patient and acute meningeal signs in the other. CT scan showed typical fat density of the tumor and the subarachnoid space. On MRI both the tumor and the subarachnoid fat, were strongly hyperintense on T1-weighted images.

[Spinal endoscopy in the detection of problems caused by continuous spinal anesthesia].

Continuous spinal anaesthesia has a number of advantages, but there are a number of drawbacks as well: difficulties in threading the cathete … to inserting the needle too far. It is impossible for the catheter to bend and be inserted into the subarachnoid space. In many cases the catheter encountered the anterior wall of the spinal canal and would slide along various structures. Distribution of the drug: the injection is better dispersed with a 22-gauge needle and it completely fills the subarachnoid space …

Primary central nervous system lymphoma in acquired immune deficiency syndrome mimicking toxoplasmosis.

A 37-year-old man, a hepatitis B virus carrier due to mother-to-child transmission, had a medical examination in September 2008 in nearby hospitals due to anorexia and weight loss. He was transported to our hospital because computed tomography (CT) detected intracranial lesions, and he had a positive human immunodeficiency virus (HIV) antibody test. Head computed tomography (CT) revealed multiple hemorrhagic lesions and enhancement effect, suggesting a thin wall. Also, an enhancement effect was present in the ventricle walls and the subarachnoid space. No accumulation was found in the thallium…

Noncommunicating spinal extradural arachnoid cyst causing spinal cord compression in a child.

Extradural arachnoid cysts in the spine are relatively uncommon causes of spinal cord compression in the pediatric population that are thought to arise from congenital defects in the dura mater. Most reports describe such cysts communicating with the intrathecal subarachnoid space through a small defect in the dura. The authors describe the case of a child who presented with spinal cord compression caused by a large spinal extradural arachnoid cyst that did not communicate with the intradural subarachnoid space. An 11-year-old girl presented with urinary urgency, progressive lower-extremity we…

Diagnosis of normal-pressure hydrocephalus using CT with CSF enhancement.

Cisternography with the water-soluble contrast medium metrizamide provides substantial advantages for CT demonstration: the discrimination and sharpness of definition of the basal cisterns can be considerably improved by the positive demonstration of the subarachnoid spaces. Studies of CSF dynamics make it possible to differentiate between a communicating hydrocephalus aresorptivus and a hydrocephalus within the scope of a brain atrophy. Brain substance defects and subarachnoid cysts can be clarified with respect to communication with the CSF space. Apart from providing static insights, the ci…

A severe vicious cycle in uncontrolled subarachnoid hemorrhage: the effects on cerebral blood flow and hemodynamic responses upon intracranial hypertension.

In subarachnoid hemorrhage (SAH), Cushing postulated that the increase in systemic arterial pressure (SAP) in response to elevation of intracranial pressure (ICP) was beneficial to cerebral perfusion. However, in uncontrolled SAH, the increased SAP may cause more bleeding into the subarachnoid space and further increase the ICP. We created an animal model to simulate SAH by connecting a femoral arterial catheter to the subarachnoid space. The global cerebral blood flow (CBF) was measured with a venous outflow method. The purposes were to observe the CBF change under the simulated SAH, and to e…

Subcortical cystic lesions within the anterior superior temporal gyrus: a newly recognized characteristic location for dilated perivascular spaces.

Cystic parenchymal lesions may pose an important diagnostic challenge, particularly when encountered in unexpected locations. Dilated perivascular spaces, which may mimic cystic neoplasms, are known to occur in the inferior basal ganglia and mesencephalothalamic regions; a focal preference within the subcortical white matter has not been reported. This series describes 15 cases of patients with cystic lesions within the subcortical white matter of the anterior superior temporal lobe, which followed a CSF signal; were located adjacent to a subarachnoid space; demonstrated variable surrounding s…

Pantopaque contrast mimicking intracanalicular vestibular schwannoma.

Pantopaque (iophendylate) is an oily contrast medium historically used during spine imaging. Due to its persistence in the subarachnoid space and the potential to lead to severe arachnoiditis, it is no longer used today. We present a 40-year-old male with new-onset headaches, imbalance, and vertigo. Brain magnetic resonance imaging revealed a 2-mm T1 -hyperintense intracanalicular lesion. Numerous hyperdense foci were scattered throughout the subarachnoid space on computed tomography. Further history revealed the patient received Pantopaque 30 years prior, after sustaining spinal trauma. Remna…

[CSF-evaluation in unilateral, bilateral and alternant facial paralysis (author's transl)].

Etiology of idiopathic facial paralysis remains mostly unknown because further investigations seem unnecessary being the only symptom. Diffe … ninges; protein of CSF perhaps will increase, if local inflammation of the nerve is more distant to subarachnoid space. "Idiopathic facial paralysis" however will not exclude focal inflammation far from subarachnoid space. Surgical decompression should not be performed without previous examination of CSF in regard of it's uncertain success.

When Fat Hits the Brain: Intraventricular and Subarachnoid Fat Migration Secondary to a Complex Sacropelvic Fracture-Diagnosis and Treatment.

Massive migration of fat droplets in the ventricles and the subarachnoid space is a very rare sequel of spinal trauma. Owing to its rarity, knowledge about treatment and outcome remains limited. We report on the uncommon occurrence of massive subarachnoid and intraventricular fat dissemination in a 41-year-old man who suffered a complex sacropelvic fracture with spondylopelvic dissociation but who had no head injury. We show that early placement of an external ventricular drain with prolonged drainage for washout of the fat depots can prevent chronic hydrocephalus and subsequent shunt dependen…
