Assessment of illicit drugs in wastewater and estimation of drugs of abuse in Adana Province, Turkey.

Wastewater-based epidemiology (WBE) is a rapidly developing discipline that has the potential to monitor real-time data on regional and temporal differences regarding the use of illicit drugs. With WBE, metabolic products discharged for drug consumption are rapidly collected by sewage systems, which provide valuable evidence on the types and quantities of drugs consumed by a given community. In this study, the samples were collected from the wastewaters entering the facilities in Seyhan and Yuregir Waste Water Treatment Plants (WWTPs), in Adana Province. Composite wastewater analysis was perfo

Roadside drug testing: comparison of two legal approaches in Belgium.

BACKGROUND: Internationally, urine on-site testing has been used for detecting drivers under the influence of drugs (DUID) but more and more countries, such as Belgium, are switching to oral fluid screening. OBJECTIVE: To compare the previous (published in 1999) and current (published 2009) enforcement procedures of DUID in Belgium. The two evaluated procedures differ in the way the drivers are screened by the police (signs of impairment versus signs of recent drug use), the matrix for screening (urine versus oral fluid) and the analytical cut-off concentrations in plasma. METHODS: Data on p

The trazodone metabolite meta-chlorophenylpiperazine can cause false-positive urine amphetamine immunoassay results.

Amphetamines and methamphetamines are part of an important class of drugs included in most urine drugs of abuse screening panels, and a common assay to detect these drugs is the Amphetamines II immunoassay (Roche Diagnostics). To demonstrate that meta-chlorophenylpiperazine (m-CPP), a trazodone metabolite, cross-reacts in the Amphetamines II assay, we tested reference standards of m-CPP at various concentrations (200 to 20,000 g/L). We also tested real patient urine samples containing m-CPP (detected and quantified by HPLC) with no detectable amphetamine, methamphetamine, or MDMA (demonstrated

Drugs of abuse: management of intoxication and antidotes.

Illicit drug intoxications are an increasing public health problem for which, in most cases, no antidotes are clinically available. The diagnosis and treatment of these intoxications requires a trained clinician with experience in recognizing the specific signs and symptoms of intoxications to individual drugs as well as polydrug intoxications, which are more the rule than the exception. To make the diagnosis, the clinical observation and a urine toxicology test are often enough. Evaluating the blood levels of drugs is frequently not practical because the tests can be expensive and results may

Driving under the influence of drugs in Sweden with zero concentration limits in blood for controlled substances.

OBJECTIVE: This article describes the background and implementation in Sweden of zero-concentration limits for controlled drugs in the blood of drivers. Eliminating the need to prove that a person's ability to drive safely was impaired by drugs has greatly simplified the prosecution case, which now rests primarily on the forensic toxicology report. Driving under the influence of a prescription drug listed as a controlled substance is exempt from the zero-limit law provided the medication was being used in accordance with a physician's direction and the person was not considered unfit to drive.

Clinical issues associated with urine testing of substances of abuse.

Several factors may affect the validity and outcome of urine testing for abused drugs such as amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, ethanol, opiates, and phencyclidine. Urine is used for large-scale testing because acquisition of the sample is noninvasive and because most abused drugs can be detected in urine for a reasonable duration after ingestion. Urine testing for drugs of abuse is a two-step process. In the first step, screening assays are used to identify presumably positive specimens. Common screening tests are radioimmunoassays, enzyme immunoassays, fluor

Substance abuse.

The potential for human developmental toxicity caused by chemical exposure was not generally recognized until the thalidomide tragedy of the early 1960s, at which time it was acknowledged that the placenta is not a barrier protecting the fetus. The focus of this chapter is to present the role of substance abuse screening in prenatal care, the gamut of drugs, and the clinical implications of the substances abused by pregnant women, as well as the legal and ethical considerations. Physiologic and care issues will be discussed. Legal substances discussed include the commonly used artificial sweet

Drug abuse during pregnancy in an inner-city hospital: prevalence and patterns.

Because of the increase of illicit drug use within the inner city--and especially because of the increasing popularity of cocaine--Michigan Health Center (Detroit) implemented a policy of routine drug screening of all patients admitted to the hospital through the labor unit. This policy was first implemented in February 1989. Routine drug screening made it possible to perform a retrospective study of the prevalence and patterns of drug abuse in a population consisting predominantly of inner-city patients of minority origin and indigent circumstance. The data derived from drug screening on 290

Cocaine use in pregnancy.

BACKGROUND: Cocaine use during pregnancy has many deleterious effects on both the mother and the fetus, including the following: preterm labor (< 37 weeks' gestation), congenital anomalies, intrauterine growth retardation (IUGR), abruptio placenta, low-birth-weight infants (< 2500 g), neonatal death, and sudden infant death syndrome (SIDS). Potentially catastrophic maternal outcomes include a pre-eclampsia-like syndrome, acute pulmonary edema, seizures, cardiac arrhythmia, and sudden death. Family physicians who practice obstetrics might encounter cocaine-abusing women as part of their primary

Stigmatizing Language in Substance Use-related International Classification of Diseases Codes.

OBJECTIVES: Healthcare-associated stigma is a critical barrier for treatment engagement for patients with substance use disorders. Although there are efforts to combat stigmatizing language in clinical documentation, little is known about the presence of substance use-related stigmatizing language in structured diagnosis codes ubiquitous in clinical medicine. METHODS: We examined the presence of substance use-related stigmatizing terms contained within the International Classification of Diseases, 10th revision, clinical modification (ICD-10-CM) diagnosis code descriptions. Stigmatizing terms

Use of fentanyl test strips by people who inject drugs: Baseline findings from the South Atlantic Fentanyl Test Strip Study (SAFTSS).

BACKGROUND: Research published during the early fentanyl period exposed a growing concern of unwitting fentanyl exposure and a general willingness to use fentanyl test strips (FTS). A paucity of FTS studies over the last several years has restricted our ability to understand FTS use in the late fentanyl era. The South Atlantic FTS Study (SAFTSS) was established to investigate contemporaneous changes in FTS use and drug use behavior among a rural cohort of PWID. METHODS: Between June 2021 and March 2022, a total of 541 PWID completed an in-person survey. Baseline survey questions included demo

Efficient on-site detection of nitazenes in hair: Integrating MCX pipette-tip solid-phase extraction with miniaturized mass spectrometry.

As a emerging class of synthetic opioids, nitazenes have witnessed an alarming global proliferation in the illicit drug market, posing severe threats to public health and social stability. Consequently, there is an urgent and growing demand for robust on-site rapid detection methods targeting these substances, especially within complex biological matrices. Herein, we report the first development of a miniaturized mass spectrometry (miniMS)-based analytical platform enabling rapid, on-site detection of 16 nitazenes in hair samples for the first time. Micropulverized extraction solvents and key

The influence of cosmetic treatments in the uptake of in vitro cocaine contamination in hair.

Hair analysis is a powerful tool to assess drug use, yet the challenge of external contamination complicates its interpretation. Understanding the influence of cosmetic hair treatments is pivotal as their presence may affect this phenomenon. This study investigated the effects of four cosmetic treatments (bleach, henna, gel, and dry shampoo) on the external in vitro contamination of cocaine and its primary metabolite, benzoylecgonine (BE). Hair samples were divided into four groups: A-hair treated with cosmetics then contaminated; B-hair contaminated then subjected to cosmetic treatment; and C

Determination of chlorphenesin in cosmetics, a preservative that can affect anti-doping test result interpretations.

Chlorphenesin is a legitimate preservative commonly used in cosmetics. It shares one urinary metabolite of 4-chlorophenoxyacetic acid with meclofenoxate, a prohibited stimulant in sports. Recently, there have been cases where athletes using chlorphenesin-containing products were falsely identified as users of meclofenoxate. This study developed and validated a liquid chromatography method with diode-array detection to determine the chlorphenesin content in 61 selected personal care products with various functions (e.g., facial care, body cleansing, sun protection, make-up, hairstyling, perfume

Supramolecular solvent-based sample treatment workflow for determination of multi-class drugs of abuse in hair by liquid chromatography-tandem mass spectrometry.

Hair is becoming a main matrix for forensic drug analyses due to its large detection window compared to traditional matrices (i.e. urine & blood) and the possibility of establishing the temporal pattern of drug consumption. However, the extremely time- and solvent-consuming nature of conventional sample treatments render it difficult for routine use of hair analysis in forensics. In this paper, this drawback was intended to be addressed by the use of hexanol-based supramolecular solvents (SUPRAS) with restricted-access properties. The aim was to develop a fast and interference-free sample trea
