The effects of acutely induced metabolic and respiratory alkalosis with reduction of serum potassium concentration as well as chronic total body potassium depletion induced by furosemide treatment were evaluated and correlated with alteration of neuromuscular blockade induced by d-tubocurarine, pancuronium, and succinylcholine in 12 mongrel dogs. The clinical pharmacology of neuromuscular blocking agents is described. During neuromuscular blockade, succinylcholine attaches to receptors in the motor end plate and depolarizes the neuromuscular junction, making the end plate refractory to acetylcholine. Succinylcholine has been indicted on account of the diverse risks associated with its administration, which include hyperkalemia, vagal arrest, and malignant hyperthermia. However, it provides excellent intubating conditions reliably and quickly, characteristics that sustain its appeal for use in the intensive care unit, where airway management is challenging. There is an increasing body of evidence, outlined here, that rocuronium is an acceptable alternative to succinylcholine.

We report the case of a patient suffering from dysthymia. He underwent successful ECT treatment and was medicated with nefazodone and midazolam. Comparing the EEG and motor seizure duration in the periods with and without midazolam, no significant differences were recorded, but the dosage of thiopental and of succinylcholine had to be increased markedly after midazolam was discontinued (P < 0.05; ANOVA). This case indicates a substantial pharmacodynamic interaction between nefazodone, midazolam, thiopental, and succinylcholine.

A 28-year-old woman died of irreversible ventricular fibrillation after administration of succinylcholine because of post partum curettage. Afterwards it became known she had undiagnosed myotonic dystrophy. She also had metabolic acidosis because of haemorrhagic hypovolaemia resulting in a probably elevated serum K+ concentration preoperatively. Succinylcholine given to patients with myotonic dystrophy may lead to life threatening hyperkalaemia and cardiac arrest. Therapy should be aimed at immediate lowering of serum K+ concentration with calcium, sodium bicarbonate and hyperventilation.

Operative charts were reviewed in 86 patients with Charcot-Marie-Tooth disease, a condition characterized by chronic muscular denervation. A total of 161 surgical procedures was performed. Major complications were few, and only one operative death occurred, unrelated to anaesthesia. Succinylcholine and malignant hyperthermia triggering agents were used in 41 (48%) and 77 (90%) patients, respectively, without untoward effects. Contrary to previous reports, this survey supports the safe use of succinylcholine and MH triggering agents in this disease.

In a 11 1/2 year-old boy, acute myolysis with myoglobinuria followed anesthesia and had a favourable outcome. Biological signs of myolysis, without clinical symptoms seem to be frequent after Halothane-succinylcholine anesthesia. They are even more frequent in children, especially when succinylcholine is repeatedly given during Halothane anesthesia. Patient clinical symptoms (visible myoglobinuria, pain and/or paralysis) are rare and occur chiefly in children with chronic muscular disease, in whom they may be the presenting symptoms.

During a three year period, pediatric patients 4 to 16 years old were post-operatively screened for anaesthesia-induced myoglobinuria. All investigated cases had general anaesthesia and received succinylcholine prior to intubation. Anaesthetics were predominantly performed for tonsillectomies and/or adenoidectomies.
