Esophageal dysmotility and the sudden infant death syndrome: experimental observations of neonatal puppies.

The authors review the relationship between esophageal dysfunction (dysmotility) and the unusual cardiopulmonary and neurological manifestations equated with the symptoms of the "near-miss" sudden infant death syndrome (SIDS). Six beagle puppies were studied to see whether esophageal irritation and dilatation could provoke any cardiac dysrhythmia. Some of the cardiographic irregularities observed in 10 of 11 trials occasionally precede fatal ventricular dysfunction. The authors conclude that esophageal irritation can be responsible for cardiac arrest and that one important cause of SIDS is eso…

The epidemiology of sudden infant death in upstate New York: II: birth characteristics.

This study describes the epidemiology of sudden infant death syndrome (SIDS) among infants born during 1974 to upstate New York residents. Birth certificate characteristics for 184 SIDS cases are compared with those of 417 infants dying from other causes in the same age range, 7-365 days. The results confirm the following as infant risk factors: fall or winter birth, low birthweight for gestational age, twin birth, and live birth order three or more. Maternal risk factors include: age under 20, abnormal uterine bleeding during pregnancy, late initiation of prenatal care, less than 12 years of …

Previous fetal loss and risk of sudden infant death syndrome in subsequent offspring.

The relationships between previous fetal loss (obtained by mother's statement) and certain categories of infant death including probable instances of the sudden infant death syndrome (SIDSp) were examined. The deaths were those occurring among the cohort of live singleton births born in the State of North Carolina, 1960-1967. SIDSp were defined by age (28-364 days), place of death (outside the hospital), and certain ICD code numbers of the 7th Revision (273, 331, 422.2, 491-493, 500-501, 522, 525, 527.2, 762, 795 and 924). Other deaths in the postneonatal period were dichotomized between conge…

Inconsistent classification of unexplained sudden deaths in infants and children hinders surveillance, prevention and research: recommendations from The 3rd International Congress on Sudden Infant and Child Death.

This report details the proceedings and conclusions from the 3rd International Congress on Unexplained Deaths in Infants and Children, held November 26-27, 2018 at the Radcliffe Institute at Harvard University. The Congress was motivated by the increasing rejection of the diagnosis Sudden Infant Death Syndrome (SIDS) in the medical examiner community, leading to falsely depressed reported SIDS rates and undermining the validity and reliability of the diagnosis, which remains a leading cause of infant and child mortality. We describe the diagnostic shift away from SIDS and the practical issues …

Continuous microbiological and pathological study of 70 sudden and unexpected infant deaths: toxigenic intestinal clostridium botulinum infection in 9 cases of sudden infant death syndrome.

As part of a programme to exclude infection as the cause of death in infants who died suddenly and unexpectedly necropsies were carried out on 70 such infants. In 11 cases (15%), a pathological diagnosis could be made at necropsy; in 9 of these, causative bacteria or viruses were found. The 59 cases in which the cause of death could not be found had histological features characteristic of sudden infant death syndrome (SIDS). Botulinum toxin was found in 9 SIDS cases (15%). 8 of these infants had botulinum toxin and organisms of different types (A, B, C, F, G) in the contents of the ileojejunum…

A narrative review of sudden infant death syndrome: pathophysiology, risk factors, and prevention.

UNLABELLED: Despite increasing public awareness and clinical advances, sudden infant death syndrome (SIDS) continues to account for a substantial proportion of these deaths, largely due to limited mechanistic understanding. This knowledge gap underscores the need for in-depth investigation and population-specific prevention strategies. In this review, we synthesize current knowledge and recent advances in SIDS epidemiology, key demographic and clinical characteristics, and underlying pathophysiology. We discuss intrinsic and extrinsic risk factors within the triple-risk model, including brains…

A Case of Sudden Unexpected Infant Death with Presumptive SARS-CoV-2 Infection.

COVID-19 remains a challenge to the global healthcare despite the end of the pandemic, including due to the significant involvement of children in the epidemic process. During the pandemic period, an increase in the incidence of Sudden Unexpected Infant Death (SUID) and Sudden Infant Death Syndrome (SIDS) was observed. Currently, their rates remain elevated compared to the prepandemic period. The pathogenetic mechanisms underlying the fulminant course of infection in infants leading to fatal outcomes remain insufficiently understood. In this study, we report for the first time the results of h…

Sudden Unexpected Infant and Perinatal Death: Pathological Findings of the Cardiac Conduction System.

Objective: Sudden infant death syndrome (SIDS), sudden neonatal unexpected death (SNUD), and sudden intrauterine unexpected death (SIUD) are major unsolved, shocking forms of death that occur frequently and without warning. The body of literature on the anatomo-pathological substrates in the cardiac conduction system of SIDS-SIUD and their possible relationship with risk factors and triggers is fragmentary and scarce. The work aims is to analyze the cardiac conduction system findings collected at the national referral center for SIDS-SIUD. Methods: A total of 123 autopsied cases of SIDS (59.35…

Rates of Sudden Unexpected Infant Death Before and During the COVID-19 Pandemic.

IMPORTANCE: Infection has been postulated as a driver in the sudden infant death syndrome (SIDS) cascade. Epidemiologic patterns of infection, including respiratory syncytial virus and influenza, were altered during the COVID-19 pandemic. Comparing month-to-month variation in both sudden unexpected infant death (SUID) and SIDS rates before and during the pandemic offers an opportunity to generate and expand existing hypotheses regarding seasonal infections and SUID and SIDS. OBJECTIVE: To compare prepandemic and intrapandemic rates of SUID and SIDS, assessing for monthly variation. DESIGN, S…

Association between short-term exposure to air pollution and sudden infant death syndrome.

The association between air pollution and infant mortality has been inconsistently reported. A few studies have estimated short-term effects of air pollution on infants' health. This population-based case-control study aimed to examine the potential effects of air pollution on sudden infant death syndrome (SIDS) in the post-neonatal period in Taiwan during 1997-2002. Each case of infant death was matched with 20 randomly selected sex-matched controls who were born on the same day and were still alive. We obtained 24-h measurements of air pollutants and meteorological factors in each case and c…

Postmortem diagnosis of infectious heart diseases: A mystifying cause of Sudden Infant Death.

Sudden infant death (SID) is an unresolved problem of high relevance and previous studies have indicated a role of viral heart infections. The diagnosis remains difficult in clinical practice using routine diagnostic tests and must be substantially improved. A prospective study based on post-mortem samples from SID victims whose heart disease was not clinically recognized was conducted for 4 years in a Tunisian University Hospital. Pediatric cases of unnatural death served as controls. Both SID victims and controls were investigated for possible coxsackievirus-B (CV-B) infection in heart tissu…

Prenatal alcohol exposure and miscarriage, stillbirth, preterm delivery, and sudden infant death syndrome.

In addition to fetal alcohol syndrome and fetal alcohol spectrum disorders, prenatal alcohol exposure is associated with many other adverse pregnancy and birth outcomes. Research suggests that alcohol use during pregnancy may increase the risk of miscarriage, stillbirth, preterm delivery, and sudden infant death syndrome. This research has some inherent difficulties, such as the collection of accurate information about alcohol consumption during pregnancy and controlling for comorbid exposures and conditions. Consequently, attributing poor birth outcomes to prenatal alcohol exposure is a compl…

A possible murine model for investigation of pathogenesis of sudden infant death syndrome.

Several studies have indicated a possible causative role of toxigenic bacteria in sudden infant death syndrome (SIDS). This study examined the effect of toxigenic E. coli on pregnant and infant mice to determine if these animals could be used as a model for SIDS pathogenesis. Strains of E. coli from the intestinal contents of infants who have died of SIDS or other causes and from the faeces of healthy infants were collected over a broad time scale. The isolates were tested for their ability to produce then known toxins of E. coli and were serotyped (O and H antigens). Certain serotypes (e.g. O…

Fibromuscular hyperplasia of the pulmonary artery in sudden infant and perinatal unexpected death.

INTRODUCTION: The purpose of this study was to describe cases presenting with fibromuscular hyperplasia of the pulmonary arteries that could belong to the group of sudden infant death syndrome (SIDS) and sudden unexpected perinatal death "gray zone" or borderline cases. METHODS: In a total of 12 cases, eight females and four males, ranging in age from 39 gestational weeks to 93 postnatal days, dying suddenly and unexpectedly, a fibromuscular hyperplasia of the pulmonary artery was detected. Postmortem examinations were requested with a clinical SIDS or sudden unexpected perinatal death. A com…

Risk factors for sudden infant death syndrome: changes associated with sleep position recommendations.

OBJECTIVE: To quantify the decreased incidence of sudden infant death syndrome (SIDS) and investigate risk profile changes associated with changes in infant sleep position. DESIGN: A population-based case-control study that used birth and death certificates from Washington State. Cases (n = 1515) were all singletons born in Washington between 1985 and 1995 (excluding 1991) who died of SIDS. Control cases (n = 6060) were randomly selected singletons born in the same period who did not die of SIDS. Multivariate logistic regression compared changes in the magnitude of associations between risk f…

An interesting case of infant sudden death: severe hypertrophic cardiomyopathy in Pompe's disease.

Glycogen storage disease type II (Pompe's disease) is a rare inherited metabolic disorder, which often leads to infantile death from severe cardiomyopathy. This case of sudden death illustrates the features of the cardiac findings in the disorder, resulting from massive lysosomal accumulation of glycogen in the heart and other tissues. Pompe's disease should be considered in cases of unexplained infantile cardiomyopathy.
