Investigating the impact of white matter hyperintensities on longitudinal progression in progressive supranuclear palsy.

BACKGROUND: White matter hyperintensities (WMH), traditionally linked to cerebral small vessel disease, are frequent in Progressive Supranuclear Palsy (PSP). This study assessed their prognostic value in disease progression. METHODS: Sixty PSP patients underwent 3 Tesla Magnetic Resonance Imaging (MRI) at baseline and then were followed for of 48.62 ± 25.11 (mean ± standard deviation) months with extensive neurological evaluations. WMH were rated with the age-related white matter changes (ARWMC) scale (total and lobar), classifying patients into two groups: ARWMC = 0 (without WMH) and ARWMC >

Language impairment is associated with faster progression in progressive supranuclear palsy-Richardson syndrome.

INTRODUCTION: Cognitive impairment is common but often overlooked due to motor symptoms in progressive supranuclear palsy-Richardson syndrome (PSP-RS). This study investigates whether cognitive deficits predict disease progression in PSP-RS. METHODS: A total of 146 PSP-RS from the Tilavonemab trial were evaluated at baseline and over 52 weeks using the PSP-Rating Scale (PSPRS), the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS), and the Unified Parkinson's Disease Rating Scale Part-II (UPDRS-II). Multiple linear regression analyses were performed between the RBANS,

Identification of metabolic progression and subtypes in progressive supranuclear palsy by PET molecular imaging.

INTRODUCTION: Progressive supranuclear palsy (PSP) is a neurodegenerative disorder with diverse clinical presentations that are linked to tau pathology. Recently, Subtype and Stage Inference (SuStaIn) algorithm, an innovative data-driven method, has been developed to model both the spatial-temporal progression and subtypes of disease. This study explores PSP progression using 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) imaging and the SuStaIn algorithm to identify PSP metabolic progression subtypes and understand disease mechanisms. METHODS: The study included 72 PSP patie

The Genetic Background of the Immunological and Inflammatory Aspects of Progressive Supranuclear Palsy.

Progressive supranuclear palsy (PSP) is a neurodegenerative disease, classified as an atypical Parkinsonian syndrome, that has been pathologically and clinically defined. The histopathological aspects of the disease include tufted astrocytes, while the clinical features involve oculomotor dysfunction, postural instability, akinesia, cognitive impairment, and language difficulties. Although PSP is generally considered a sporadic disease, interest is growing in its genetics, with contemporary research focusing on familial backgrounds and neuroinflammation. Indeed, microglial activation and other

The potential of proteomics for in-depth molecular investigations of progressive supranuclear palsy.

INTRODUCTION: Progressive supranuclear palsy (PSP) is a rare neurodegenerative disorder. The lack of comprehension about the pathogenesis of the disease, its heterogeneity, and the complex clinical evaluation in early stages, limit the development of effective treatments for PSP patients and highlight the need of further research on the field. AREAS COVERED: In this work, we review the current knowledge of the physio- and neuropathology of PSP, its clinical features, diagnosis markers, and treatment options. We also compare the proteomic-based studies done to date in brain tissues as well as

Possible Impact of Peripheral Inflammatory Factors and Interleukin-1β (IL-1β) on Cognitive Functioning in Progressive Supranuclear Palsy-Richardson Syndrome (PSP-RS) and Progressive Supranuclear Palsy-Predominant Parkinsonism (PSP-P).

Progressive supranuclear palsy (PSP) is a tauopathic atypical parkinsonian syndrome. Recent studies suggest that inflammation may play a role in PSP pathogenesis, highlighting markers like the neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and cytokines such as IL-1β and IL-6. This study aimed to assess the relationship between peripheral inflammatory markers and psychological abnormalities in PSP-RS and PSP-P patients. The study included 24 participants: 12 with PSP-RS, 12 with PSP-P, and 12 controls. Cognitive function was assessed using the Montreal Cognitive Asse

Brainstem and Cerebellar Volume Loss and Associated Clinical Features in Progressive Supranuclear Palsy.

INTRODUCTION: Progressive Supranuclear Palsy (PSP) is a neurodegenerative 'tauopathy' with predominating pathology in the basal ganglia and midbrain. Caudal tau spread frequently implicates the cerebellum; however, the pattern of atrophy remains equivocal. We hypothesise that volume loss is appreciable in the cerebellum and brainstem regions-beyond the midbrain-in individuals with PSP and linked to motor and non-motor clinical features. METHODS: In this cross-sectional observational study, volumetric brainstem and cerebellar subsegmentation of T1-weighted magnetic resonance imaging (MRI) was

Prevalence of Intestinal Methanogen Overgrowth in Progressive Supranuclear Palsy: A Case-Control Study.

Objective Progressive supranuclear palsy (PSP) is a rare neurodegenerative disorder frequently accompanied by severe gastrointestinal dysfunction, particularly constipation. Although small intestinal bacterial overgrowth (SIBO) has been reported in several neurological diseases, intestinal microbial phenotypes in patients with PSP have not been systematically evaluated. This study aimed to assess the prevalence of intestinal methanogen overgrowth (IMO) and hydrogen-type SIBO in patients with PSP.Methods We conducted a cross-sectional case-control study to evaluate hydrogen- and methane-based b

Diagnostic utility of tectal plate measures in clinical variants of progressive supranuclear palsy.

BACKGROUND: The length of the tectal plate, which sits above the midbrain, is reduced in both the Richardson's syndrome (RS) and parkinsonism (P) variants of progressive supranuclear palsy (PSP). However, it is unclear whether tectal measurements could be useful biomarkers in other PSP variants or predict PSP pathology. METHODS: The Neurodegenerative Research Group, Mayo Clinic, enrolled 115 PSP (including seven variants), 19 corticobasal syndrome (CBS), 21 Parkinson's disease (PD), and 50 controls. Forty-seven PSP patients had PSP at autopsy, and 15 had a non-PSP pathology. Tectal plate leng

Evidence for trans-synaptic propagation of oligomeric tau in human progressive supranuclear palsy.

In the neurodegenerative disease progressive supranuclear palsy (PSP), tau pathology progresses through the brain in a stereotypical spatiotemporal pattern, and where tau pathology appears, synapses are lost. We tested the hypothesis that pathological tau contributes to synapse loss and may spread through the brain by moving from presynapses to postsynapses. Using postmortem PSP brain samples and a living human brain slice culture model, we observe pathological tau in synaptic pairs and evidence that oligomeric tau can enter live human postsynapses. Proteomics revealed increased clusterin in s

Midbrain cytotoxic T cells as a distinct neuropathological feature of progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) is a neurodegenerative disorder characterized by four-repeat (4R) tau protein deposition. The substantia nigra (SN) and midbrain tegmentum nuclei (MBT) are consistently affected. Lymphocyte infiltrates are scarce in the brains of patients with neurodegenerative diseases, although a few reports have described their presence in the α-synucleinopathy Parkinson's disease (PD). To evaluate the cytotoxic T-cell response, serial sections spanning 120 μm of the SN were immunostained consecutively for phosphorylated tau (p-tau, AT8) or α-synuclein, cytotoxic T-cell

Neuronal-specific antibody in patients with multiple system atrophy and progressive supranuclear palsy.

Anti-neuronal antibodies can manifest with diverse movement disorders, including parkinsonism and ataxia, and may mimic neurodegeneration. The prevalence of neuronal-specific antibodies in atypical parkinsonism remains unclear. We examined 166 patients, including 136 patients with clinically diagnosed progressive supranuclear palsy (PSP) and 30 patients with multiple system atrophy (MSA) from the Taiwan PSP/CBS Consortium using an integrated set of tissue-based immunofluorescence assays and a subsequent fixed cell-based assay as well as confirmatory tests to detect neuronal-specific antibodies

Transcriptional expression patterns of the cortical morphometric similarity network in progressive supranuclear palsy.

BACKGROUND: It has been demonstrated that progressive supranuclear palsy (PSP) correlates with structural abnormalities in several distinct regions of the brain. However, whether there are changes in the morphological similarity network (MSN) and the relationship between changes in brain structure and gene expression remain largely unknown. METHODS: We used two independent cohorts (discovery dataset: PSP: 51, healthy controls (HC): 82; replication dataset: PSP: 53, HC: 55) for MSN analysis and comparing the longitudinal changes in the MSN of PSP. Then, we applied partial least squares regress

Utility of Cerebrospinal Fluid Transferrin Receptor per Ferritin Ratio in Progressive Supranuclear Palsy.

BACKGROUND: Progressive supranuclear palsy (PSP) is a major atypical parkinsonism. Because diagnosis based on the cardinal clinical features is often difficult, misdiagnosis with Parkinson's disease (PD) and multiple system atrophy (MSA) is common in PSP patients. Iron metabolism genes are reportedly involved in tau-accumulating neuronal cell death and ferroptosis in PSP, which is more severe than PD and MSA. The validity of transferrin receptor (TfR) expression as a biomarker of ferroptosis was also demonstrated. OBJECTIVE: We investigated whether TfR and the TfR to ferritin ratio in the cer

Uncovering spatiotemporal patterns of atrophy in progressive supranuclear palsy using unsupervised machine learning.

To better understand the pathological and phenotypic heterogeneity of progressive supranuclear palsy and the links between the two, we applied a novel unsupervised machine learning algorithm (Subtype and Stage Inference) to the largest MRI data set to date of people with clinically diagnosed progressive supranuclear palsy (including progressive supranuclear palsy-Richardson and variant progressive supranuclear palsy syndromes). Our cohort is comprised of 426 progressive supranuclear palsy cases, of which 367 had at least one follow-up scan, and 290 controls. Of the progressive supranuclear pal

Low clinical sensitivity and unexpectedly high incidence for neuropathologically diagnosed progressive supranuclear palsy.

The objective of this study was to determine the prevalence, incidence, and clinical diagnostic accuracy for neuropathologically diagnosed progressive supranuclear palsy (PSP) with data from a longitudinal clinicopathological study using Rainwater criteria to define neuropathological PSP. Of 954 autopsy cases, 101 met Rainwater criteria for the neuropathologic diagnosis of PSP. Of these, 87 were termed clinicopathological PSP as they also had either dementia or parkinsonism or both. The prevalence of clinicopathologically defined PSP subjects in the entire autopsy dataset was 9.1%, while the i
