The therapeutic attitude in distal radial Salter and Harris type I and II fractures in children.

INTRODUCTION: Salter Harris Fractures type, especially type I and II are treated by orthopedic reduction in the emergency room or operating room, under general anesthesia, followed by plaster immobilization. Neglected or incorrectly treated fractures, leading to malunion and radiocarpal subluxations which require surgical procedure. PURPOSE: This paper proposes to evaluate the correctly applied orthopedic treatment and the expose of an original surgical technique in case of neglected and incorrectly treated fractures, leading to mal-unions and impediments in the radiocarpal mobility and aesth

Suture Fixation of Levonorgestrel-Releasing Device Using the Hysteroscopic Surgery System.

BACKGROUND: The levonorgestrel-releasing intrauterine system (LNG-IUS) is a widely recognized method for contraception and the management of various gynecological conditions. However, the placement of LNG-IUS can be particularly challenging in patients with large or anatomically irregular uteri. The objective of this study was to evaluate the efficacy and safety of hysteroscopic cold-knife-assisted suture fixation of the LNG-IUS in the uteri of patients diagnosed with adenomyosis. METHODS: We retrospectively analyzed the data of patients who underwent LNG-IUS placement with the assistance of

Application of a self-developed femoral artery compression hemostasis device in proximal femoral nail anti-rotation surgery for intertrochanteric fractures: a case report.

BACKGROUND: Intertrochanteric fractures of the femur are a prevalent type of hip fracture among the elderly. While surgical treatment is the preferred approach for managing these fractures, conventional tourniquets cannot be applied in hip surgery due to anatomical constraints. As a result, effectively controlling intraoperative and perioperative blood loss has long posed a significant clinical challenge. CASE PRESENTATION: A 70-year-old male patient with a comminuted fracture in the left intertrochanteric region of the femur underwent closed reduction and internal fixation using a proximal f

Syndesmotic Malreduction Prevention, Assessment, and Treatment.

Syndesmotic instability or malreduction is an independent risk factor for an impaired patient-rated outcome. If a syndesmotic injury is suspected, a stepwise diagnostic approach including plane radiographs, MRI, and bilateral stress radiographs should be conducted to differentiate stable from latent unstable and frank diastasis cases. The basic requirement for any surgical treatment approach is a stable and anatomically reduced distal tibio-fibula joint. The best treatment approach for a 2-ligament injury (anterior inferior tibio-fibular ligament [AiTFL] + interosseous ligament [IOL]) seems to

Definition, quantification, and correction of translation deformities using long leg, frontal plane radiography.

The surgical realignment of mechanical axis deviation is necessary to prevent early joint degeneration. Modern types of external fixation systems allow alignment of the mechanical axis to exact degrees. Predominately, these are corrections of angulation deformities. In some cases, the analysis of the mechanical axis deviation does not show any angulation deformity, but rather a parallel staggering of the mechanical axis lines of a bone. Such parallel staggering of the mechanical axis lines is defined as a translation deformity of the bone. In combined deformities with angulation and translatio

High-resolution CT of the wrist: initial experience with scaphoid disorders and surgical fusions.

We performed high-resolution CT scans on 30 wrists in 27 patients with either previous surgical intercarpal fusion or known or suspected scaphoid abnormalities. Most examinations used the same protocol: contiguous 1.5-mm axial sections parallel to the long axis of the body of the scaphoid, small reconstruction circle, and bone reconstruction algorithm. Fifteen patients were evaluated for union of previous surgical carpal fusions; eight patients were evaluated for healing of scaphoid fractures; and the other four patients had miscellaneous disorders, including a midcarpal dislocation with a sca

Standardization of laparoscopic intraperitoneal onlay mesh repair for incisional hernia: Impact on clinical outcome and quality-of-life (LIPOM trial, NCT 02089958).

PURPOSE: Current available outcome data following laparoscopic intraperitoneal onlay mesh repair (IPOM) for incisional hernia (IH) are comparable to a limited extent only because of a huge number of variability particulary in surgical technique and use of medical devices. In this prospective observational multicenter cohort study we evaluate the impact of a consensus driven standard protocol for IPOM, that is mesh fixation with absorbable tacks in double crown technique enforced by additional non-absorbable transfascial sutures at the edges of the mesh along with the use of Physiomesh™, on cli

Three-dimensional printing designed customized plate in the treatment of coronal fracture of distal humerus in teenager: A case report.

RATIONALE: Because of the complicated anatomy and considerable change in size and morphology with age in teenagers, the appropriate internal fixator of coronal shear fracture of distal humerus is difficult to choose, and therefore, the fixation of this kind of fracture is difficult and controversial. Furthermore, distal humeral fractures in teenagers often involve the epiphysis, the rigid fixation of fracture and the simultaneous minimally invasive and protection of the epiphysis are contradictory. Coronal shear fractures of the distal humerus in teenagers are great challenge for orthopedic su

Two-Year Follow-up Comparison of Three Surgical Techniques for Implant Placement in Posterior Maxilla with Limited Alveolar Bone Height.

PURPOSE: The aim of this pilot study was to compare three surgical techniques (bicortical fixation, unicortical fixation, and indirect sinus elevation) for implant placement in the posterior maxilla with limited alveolar bone height. This 2-year follow-up assessment on the implants of a previously published prospective clinical trial compared (1) restoration/implant stability among the three surgical groups, (2) intrasinus bone formation between the bicortical fixation and indirect sinus elevation groups, and (3) implant and prosthetic complications reported among the three groups. Mechanical Complications After Intramedullary Fixation of Extracapsular Hip Fractures.

Extracapsular hip fractures occur frequently in the elderly as a result of low-energy trauma. Achieving stable fixation in osteoporotic bone to allow early weight bearing is a key objective in the treatment of these injuries. Many of the intraoperative decisions facing surgeons are directed by first determining fracture stability. Unstable fracture patterns should be fixated with intramedullary nails, aiming to control motion around the implant and at the fracture site. Torsion control devices provide additional stability and control, although their exact indications are not precisely defined.

TomoFix: a new LCP-concept for open wedge osteotomy of the medial proximal tibia--early results in 92 cases.

High Tibial Osteotomy (HTO) is an established treatment for unicompartmental osteoarthritis of the knee with malalignment. The classic procedure for correcting varus deformity is the lateral closed wedge osteotomy of the tibia with osteotomy of the fibula. The disadvantages of this technique are well known. Open wedge osteotomy from the medial side eliminates the risk of compartment syndrome and peroneal nerve injuries. A new fixation device (TomoFix) with an adapted surgical technique allows stable fixation of the osteotomy without the need to fill the osteotomy gap with bone grafts. In a pro

Treatment of femoral shaft fractures with the Hoffmann external fixator in prepuberty.

Twenty-two children with femoral shaft fractures, between the age of 10 and 14 years, were treated with the Hoffmann external fixation apparatus from 1980 to 1985. This method of treatment is very simple. It requires less surgical exposure and allows earlier ambulation and easy nursing care. In addition, it makes patients comfortable in multiple trauma conditions. The average time to union and removal of fixators was 72 days (range, 44 to 80 days). There were no deep wound infections, nonunions, or rotationary deformities. On followup, all patients regained the range of motion of knee joints i

The treatment of pathologic and impending pathologic fractures of the proximal femur in the elderly.

Forty-eight pathologic and impending pathologic fractures of the proximal femur were surgically treated in 38 patients averaging 70 years of age. The most common tumors were breast (45%), multiple myeloma (24%), and lung (11%). Tumor size, aggressiveness, and location influenced the type of fixation. Sixty-four percent of the lesions were treated by flexible intramedullary nails. This "stress sharing" device afforded sufficient stability to provide pain relief, restore function, and permit healing. Pain was relieved in 92% of the patients, and 84% became ambulatory. The rate of fracture union

A short history of posterior dynamic stabilization.

Interspinous spacers were developed to treat local deformities such as degenerative spondylolisthesis. To treat patients with chronic instability, posterior pedicle fixation and rod-based dynamic stabilization systems were developed as alternatives to fusion surgeries. Dynamic stabilization is the future of spinal surgery, and in the near future, we will be able to see the development of new devices and surgical techniques to stabilize the spine. It is important to follow the development of these technologies and to gain experience using them. In this paper, we review the literature and discus

The complicated Jones fracture, including revision and malalignment.

Using radiographs culled over a 33-month period, the treatment, complications, revision, and rehabilitation of complicated Jones fractures and stress fractures involving the proximal diaphysis are examined. Although the non-operative approach remains viable, the exigencies and desires of the athletic and leg-based working population require sooner-rather-than-later return to play or work. Fortunately, these needs can be matched by the available and functioning orthopedic practice of intramedullary screw fixation. This practice is coupled with prevention, reliable orthopedic techniques, the ort

Treatment of distal radius fractures with a nonbridging cross-pin fixator (the CPX system).

Many treatment methods exist for patients presenting with a fracture of the distal radius. With the evolution of innovative procedures and devices, treatment of these particular fractures is shifting to more contemporary approaches. The minimally invasive technique with the cross-pin fixator (CPX) system offers a new biomechanical concept for relatively rigid fixation of distal radius fractures (DRF). The CPX system uses percutaneous cross Kirschner wire fixation in combination with a nonbridging external fixator. The stability of the model is derived from the external unilateral frame and the
