Pulmonary carcinoid tumors are uncommon neuroendocrine tumors that rarely metastasize to the skin. We report the case of a 71-year-old woman with a longstanding history of primary atypical pulmonary carcinoid tumor who presented with a new tender cutaneous nodule. Immunostaining of the nodule was consistent with metastatic atypical carcinoid tumor of the skin including positive staining for neuroendocrine markers chromogranin and synaptophysin. A 4-cm paravertebral mediastinal tumor was resected in a 70-year-old male patient treated for hypertension. A 69-year-old male was admitted to an outside institution for severe right abdominal pain radiating to the flank. An extensive work-up, including lower GI series, IVP, and ultrasound of the kidneys were normal. Ultrasound of the liver demonstrated a thickened gallbladder as well as cholelithiasis. We report the case of a 72-year-old non-smoking woman with a 2.4cm parenchymal nodule in the left lower lobe discovered incidentally. Fine-needle aspiration cytology demonstrated an atypical papillary lesion with a CK7+/TTF1+ immunohistochemical profile and negativity for CK20, PS100 and synaptophysin, initially suggesting a primary pulmonary adenocarcinoma.
