Arthroscopic treatment of localized pigmented villonodular synovitis of the knee.

PURPOSE: The purpose of this study was to review the outcome of the arthroscopic treatment of localized pigmented villonodular synovitis of the knee and to determine the recurrence rate with clinical and magnetic resonance imaging evaluation at midterm follow-up. METHODS: Thirty consecutive patients diagnosed with localized pigmented villonodular synovitis of the knee were treated arthroscopically between 1990 and 2008. Clinical assessment was made with the use of Lysholm Knee Scale, and radiologic assessment was done by plain radiographs and magnetic resonance imaging of the knee. Recurrence…

Effects of hyaluronan on carrageenan-induced synovitis in rat TMJ.

Nitric oxide is one of many proinflammatory mediators that are involved in temporomandibular joint (TMJ) inflammatory disorder and is synthesized by inducible nitric oxide synthase (iNOS). iNOS is transcriptionally regulated by nuclear factor-κB (NF-κB) in cases of inflammation, proliferation, and apoptosis. It has also been reported that nitric oxide is positively regulated by carrageenan and negatively regulated by hyaluronan in the knee joint. The aim of this study was to histologically evaluate how inflammation and cell proliferation of the synovial membrane are affected by the exogenous a

Pigmented villonodular synovitis of the spine: report of a lumbar case.

Pigmented villonodular synovitis is a benign proliferative disease of the synovial structures. This disease is most commonly seen in the extremities, and spinal involvement is extremely rare. In the relevant literature, 53 cases have been reported. We present a 66-year-old female patient with a long history of back pain, which progressed and radiated to the left leg over a period of 2 months. The patient also had a mass in the lumbar region on the grounds of spinal degenerative changes. Following gross total excision, the diagnosis was established by histopathology. In this report, we present …

Tuberculous synovitis of the knee in a 65-year-old man.

INTRODUCTION: Tuberculous (TB) synovitis is a rare, treatable, potentially lethal form of extrapulmonary TB resulting from massive lymphohematogenous dissemination of Mycobacterium tuberculosis (M. tuberculosis). We presented a case of TB synovitis of the knee in a Caucasian HIV-negative man from Romania, a high TB incidence country. CASE REPORT: A 65-year old man presented with cough, high fever, mild wheezing, and swelling of the left knee. Chest radiography was normal. Sputum smears were Acid Fast Bacilli negative and Lowenstein-Jensen (L-J) culture negative for M. tuberculosis. Tuberculin…

Specific synovitis of a knee as the first manifestation of miliary tuberculosis.

Tuberculosis (TB) is declared global emergency. Miliary TB is a treatable, potentially lethal form of TB resulting from massive lympho-hematogenous dissemination of Mycobacterium tuberculosis. Impaired cell-mediated immunity underlies the disease's development. We present a case of specific synovitis in a 21-year-old Caucasian HIV-seronegative woman. She presented with high fever and swelling of the right knee. Chest radiograph revealed bilateral nodular opacities in upper pulmonary lobes and signs of pleural effusions. Sputum samples were negative for Acid Fast Bacilli (AFB) and Löwenstein-Je…

Synovitis in a murine model of human factor VIII deficiency.

Recurrent joint bleeding is the most common musculoskeletal manifestation of haemophilia and leads to a target joint and synovitis. The pathobiology of haemophilic synovitis (HS) is not well understood. Here the histopathological changes that occur following haemarthrosis were examined in an animal model for human HS. After two haemarthrosis, there was soft tissue and joint swelling and histological changes of acute synovitis included infiltration of the sub-synovial layer by mononuclear cells and neutrophils, thickening of the synovial membrane with villus formation, and hyperplasia of blood …

Localized pigmented villonodular synovitis in an unusual location in the knee.

PURPOSE: Localized pigmented villonodular synovitis (LPVNS) is a rare lesion that can affect any joint, although it is most frequently found in the knee. Clinically, it is difficult to diagnose. Radiographs are usually within normal limits. TYPE OF STUDY: Case series. METHODS: We present a series of 9 cases of LPVNS of the knee in 9 patients with an average age of 36 years. In 7 of the 9 cases, the lesions were in an an unusual location, 4 in the fat pad and 3 in the posterior compartment of the knee. The other 2 cases were located in the meniscocapsular junction and the intercondylar notch,…

Transient synovitis: is there a need to aspirate hip joint effusions?

There is debate as to the optimal management of children with hip joint effusions especially regarding the decision to aspirate. The objective of this study was to determine whether there is a need to aspirate hip joint effusions detected on ultrasound in patients with clinical transient synovitis (TS) and to identify the natural history of these effusions. Twenty-five children with proven hip joint effusions were followed up sequentially by clinical and radiological examination until symptom free. The mean age was 6 years with equal right/left distribution. No child underwent hip aspiration a…

Cell populations involved in pigmented villonodular synovitis of the knee.

OBJECTIVE: Pigmented villonodular synovitis (PVNS) of the knee is a tumor-like process of uncertain nature. We analyzed the involved cell populations, iron deposition, and cell proliferation in PVNS to propose a pathogenetic concept of this still elusive disease entity. METHODS: The study was performed on a series of 14 cases of localized PVNS of the knee. Histology and histochemistry were used to evaluate basic morphology and iron deposit distribution. Immunohistochemistry was performed to characterize the inflammatory cell infiltrate and to identify the proliferating cell compartments. In s…

Multiple neuromas coexisting with rheumatoid synovitis and a rheumatoid nodule.

The authors present a rare case of multiple intermetatarsal neuromas coexisting with rheumatoid synovitis and a rheumatoid nodule. A brief review of rheumatoid nodules as a source of forefoot pain and a review of the relevant literature are provided. A rheumatoid nodule is just one of the many diagnoses that must be considered when one encounters pedal symptoms similar to those associated with Morton's neuroma.

Treatment of acute and chronic synovitis by non-surgical means.

This paper reviews personal experience in the treatment of recurrent haemarthrosis and chronic synovitis by non-surgical means. Experience with synoviorthesis with rifampicine and radioactive colloids is analyzed, and a multiple chromosomal study to demonstrate safety of radioactive injections is described. The results obtained are so very satisfactory as to recommend non-aggressive synoviorthesis as the treatment of choice to prevent recurrence of bleeding. Long experience in the treatment of chronic arthropathy with intrarticular corticosteroids and hyaluronic acid has shown very promising r…

[Late flexor synovitis induced by the use of a fabric lace].

A thirty year old woman underwent trigger finger release of her left middle finger after two injections failed to give relief. Several months later, she developed an area of nodular tenosynovitis localized to the operative site. Her finger had recovered full function, her general health was good, and routine lab work was normal. Because of the persistent mass, flexor tenosynovectomy was performed. Histologic examination revealed synovial hyperplasia associated with a filamentous birefringent foreign material. The bluish color of the foreign material indicated that it came from a fabric retract…

Ultrasound in the diagnosis and follow-up of transient synovitis of the hip.

Of 59 patients (2-15 years of age) with acute synovitis of the hip, one patient developed Perthes disease (1.7%) and the others transient synovitis. The diagnostic criterion for intracapsular effusion was a side difference greater than or equal to 2.0 mm in the distance between the anterior joint capsule and the femoral neck (anterior capsule distance, ACD). At the time of admission, the mean ACD of the affected and unaffected hips was 8.2 and 4.2 mm, respectively. The effusion persisted for greater than 1 week in 58% of the patients. We recommend ultrasound as the main imaging technique in th…
