Chronic patellar tendon rupture is a rare injury whose treatment poses technical difficulties. Surgical repair is mandatory to restore the extensor mechanism of the knee. Many different surgical methods of patellar tendon reconstruction have been described. We present the case of a 48-year-old male patient who sustained patellar tendon rupture 8 months before surgery. A semitendinosus tendon graft and the modified Ecker technique were used for the reconstruction of the patellar tendon. Full extension was achieved alongside high patient satisfaction. However, the patient required prolonged inte

Injured rat Achilles tendons were treated with botulism toxin to create a mechanically unloaded condition (unloaded) or left untreated (loaded), and then treated with phosphate-buffered saline (PBS), platelet-rich plasma (PRP), tendon stem cells (TSCs), or a combination (TSCs + PRP). mRNA and protein expression of collagen I, collagen III, tenascin C, and Smad 8 were determined by real time PCR and immunostaining, respectively. Loaded tendons treated with PBS, PRP, or TSCs for 3 or 14 days had higher collagen I mRNA expression than unloaded tendons. Loaded tendons treated with PBS for 3 or 14 …

Pathologic abnormality of the peroneal tendons is an uncommon but underappreciated source of lateral hindfoot pain and dysfunction that can be difficult to distinguish from lateral ankle ligament injuries. When left untreated, peroneal tendon disorders can lead to persistent lateral ankle pain and substantial functional problems. Unfortunately, the treatment recommendations for these disorders are primarily based on case series and expert opinion. The goals of this review are to develop a current understanding of the anatomy and diagnostic evaluation of the peroneal tendons, and to present cur…

Calcific tendonitis is a common disease of the shoulder which usually responds to conservative treatment. In cases unresponsive to conservative management, arthroscopic treatment is sometimes required. While there are several reports on calcifications within the supraspinatus tendon, documented cases involving the subscapularis tendon are rare. We present a case of a 47 year old farmer with recurrent anterior shoulder pain. An MRI revealed calcium deposits as well as a large subcoracoid cyst. Arthroscopic excision of the multiple calcific deposits left a large defect in the subscapularis tendo

Most rotator cuff surgery is performed on chronic tears, but changes in the composition of chronically torn tendons remain poorly understood. In this study we surgically created supraspinatus tears in the rat and analyzed the composition of the tendon over time using immunohistochemistry. We found that collagen types I and XII were greatly increased initially after injury and then decreased with time. Collagen type III was detected and persisted in the scar for months. Decorin and biglycan were increased initially and then decreased, although decorin remained elevated from normal for months af…

We assessed the outcome of tendon grafting of multiple finger extensor tendon ruptures in 14 patients with rheumatoid arthritis. Extensor lags improved from a preoperative mean of 33 degrees (range, 20 degrees-65 degrees) to a postoperative mean of 18 degrees (range, 0-60 degrees). However, loss of finger flexion was observed, with a mean postoperative fingertip to palm distance of 1.6 (range: 0-7.5) cm. Patient satisfaction correlated with the fingertip to palm distance, though not with the postoperative extensor lag. Because of the loss of finger flexion which was probably due to muscle cont

Twenty-one patients with ruptures of the extensor pollicis longus tendon were treated with a subcutaneously placed free tendon graft. The repair was performed a mean of 6 weeks after rupture, range from 1 to 21 weeks. Data was collected on 19 of these patients, with a mean follow-up of 30 months, range from 6 to 124 months. Near uniform satisfaction was reported. The mean loss of interphalangeal joint extension and flexion and combined metacarpophalangeal-interphalangeal joint motion was 6 degrees, 3 degrees, and 10 degrees, respectively. The mean loss of pinch strength was 7%. All patients we

INTRODUCTION: Extensor indicis proprius (EIP) tendon transfer has traditionally been the preferred surgical modality for the treatment of extensor pollicis longus (EPL) tendon rupture. In this study, we report a case series of EPL to extensor pollicis brevis (EPB) tendon transfers for the treatment of EPL tendon rupture. MATERIAL AND METHOD: This retrospective observational study included patients with acute EPL tendon rupture following distal radius fracture. Patients were evaluated at 3, 6, and 12 months postoperatively. Outcome measures included grip strength, thumb range of motion (ROM), a

BACKGROUND: Tendon adhesions after hand tendon repair are one of the most difficult complications of hand surgery and can cause severe disability. This study aimed to assess the risk factors associated with tendon adhesions after hand tendon repair to provide a theoretical foundation for the early prevention of tendon adhesions in patients with tendon injuries. Moreover, this study intends to increase doctors' awareness of the issue and serves as a reference for developing new prevention and treatment strategies.
METHODS: We retrospectively analyzed 1,031 hand trauma cases that underwent repa

BACKGROUND: An infected Achilles tendon after tendon repair is particularly difficult to treat because of the poor vascularity of the tendon as well as the thin surrounding soft tissue. For treatment of an infected Achilles tendon following tendon repair, we first focused on complete debridement and then promoted fibrous scar healing of the Achilles tendon using functional treatment.
METHODS: We retrospectively reviewed all of the medical records of 15 tertiary referral patients with postoperative infection of the Achilles tendon occurring between 2007 and 2012. The mean follow-up time was 33

BACKGROUND: With the advent of arthroscopy, more partial subscapularis tears are being recognized. The biomechanical effects of partial subscapularis tears are unknown, and there is no consensus as to their treatment. Therefore, the objective of this study was to evaluate and to quantify the changes in range of motion and glenohumeral kinematics for isolated subscapularis partial tears, combined subscapularis and supraspinatus tears, supraspinatus repair, and combined supraspinatus and subscapularis repair.
METHODS: Six cadaveric shoulders were tested in the scapular plane with 0°, 30°, and 6

Few cases of spontaneous rupture of the tibialis anterior tendon have been reported in the literature. We report a case of a 66-year-old male with a spontaneous rupture of his left tibialis anterior tendon, with a delayed presentation of approximately 3 months. At the time of reconstruction there was marked retraction of the proximal portion of the tibialis anterior tendon, which precluded an end-to-end repair; therefore, we used an Achilles tendon allograft.

Percutaneous repair of the Achilles tendon is known to minimize tendon adhesions and wound complication through a minimally invasive technique. The usually reported complications are sural nerve injury and inadequate contact of the tendon stumps, which can lead to rerupture or tendon lengthening. We experienced an interesting case of Achilles tendon adhesion after percutaneous repair, which had been initially misdiagnosed as a rerupture. To our knowledge, this is the only report of Achilles tendon adhesion as a complication after percutaneous repair, which stresses the importance of functional

This article provides a review of posterior tibial tendon pathology and the authors' preferred management. The tibialis posterior musculotendinous unit is the most powerful inverter of the foot and an important dynamic stabilizer of the arch. In the stance phase of the gait cycle, it serves as an initiator of both ankle plantar flexion and subtalar inversion. This creates a rigid midfoot by stabilizing the transverse tarsal joint, and allows for increased power generation by the gastrocsoleus complex through toe-off. Injuries to the posterior tibialis tendon include traumatic laceration and di

Disorders of the Achilles tendon include both acute and chronic ruptures as well as a spectrum of chronic overuse injuries involving inflammatory and degenerative changes within the tendon and surrounding tissues. These injuries are relatively common in athletes as well as among the general population. There is no consensus on the optimal treatment of Achilles tendon disorders. The goals of this review are to develop a current understanding of the anatomy and diagnostic evaluation of the Achilles tendon, and to present current treatment options and the authors' preferred surgical techniques fo

BACKGROUND: Tendon sheath lipoma is an uncommon tumor characterized morphologically by mature fatty tissue lobes adherent to the tendon or intersecting its fibers. The association with quadriceps tendon rupture is unusual. Etiologic factors that may condition those ruptures have not yet been fully described. However, several authors suggest the existence of a subjacent pathology such as metabolic or inflammatory diseases. Minimal trauma may lead to such ruptures as well as associated ligamentous or bony injuries. The prognosis regarding quadriceps tendon rupture mainly depends on the lap betwe

BACKGROUND Kluth demonstrated that esophageal atresia/tracheoesophageal fistula (EA/TEF) has several anatomical variations and thus requires a preoperative imaging study to determine the surgical strategy. We routinely perform a contrast examination with iodixanol to assess the location of the TEF and the upper end of the esophageal pouch to determine the most appropriate approach. We herein present two cases of type C EA/TEF who successfully underwent radical surgery by a cervical approach based on the information from the contrast examination. CASE REPORT Case 1 was a Japanese boy suspected
