Carpal Tunnel Syndrome Secondary to Tuberculous Tenosynovitis With Rice Bodies: A Case Report.

CASE: We report the case of a 34-year-old African man who presented with severe symptoms of recurrent left carpal tunnel syndrome (CTS) and left hand swelling after previous open decompression. Considering the recurrent unilateral affection of the left hand in a patient working in a slaughterhouse in an area with a moderate burden for tuberculosis, tuberculous infection was suspected. Open surgery and biopsy revealed tuberculous tenosynovitis of flexor tendon sheath and shiny white rice bodies. CONCLUSION: Tuberculous tenosynovitis should be considered as a differential diagnosis of the CTS w…

Prevalence of Septated First Dorsal Compartment Among Iranian Patients with De Quervain Tenosynovitis.

Background: A septum in the first extensor compartment and variation of abductor pollicis longus (APL) and extensor pollicis brevis (EPB) tendons affect the development and treatment of the de Quervain disease. This study aimed to determine the prevalence of septum and the number of APL and EPB tendons in Iranian surgical de Quervain patients. Methods: In this case series, we evaluated 37 consecutive wrists from September 2019 to April 2020 that were evaluated and surgically explored by the same senior surgeon; and the number of tendons and the existence of septum were recorded. Results: Patie

Functional outcome of De Quervain's tenosynovitis with longitudinal incision in surgically treated patients.

INTRODUCTION: De Quervain described tenosynovitis of first dorsal compartment more than 120 years ago. Women, particularly of 4th-5th decades, are at more risk of developing disease. Steroid injection has been described as first line of management over many decades, but it is associated with some significant complications like depigmentation of skin, atrophy of subcutaneous tissue, suppurative tenosynovitis and even tendon rupture. Animal studies have also reported increased risk of peritendinous adhesions with steroid injection. MATERIALS AND METHODS: We prospectively managed 46 cases of De …

Acute Carpal Tunnel Syndrome Due to Pyogenic Flexor Tenosynovitis without Any Antecedent Injury.

We herein report a case of acute carpal tunnel syndrome due to pyogenic flexor tenosynovitis in the absence of any antecedent injury whose rapid progression and course was similar to that seen with necrotizing fasciitis. This potentially disastrous clinical condition must be promptly recognized, since it needs early surgical management to prevent morbidity.

Polyarticular extension of pigmented villonodular synovitis to contiguous joints via pigmented villonodular tenosynovitis.

Pigmented villonodular synovitis is an uncommon benign neoplastic process that affects synovial-lined joints, bursae and tendon sheaths. We describe polyarticular extension of pigmented villonodular synovitis across joints secondary to pigmented villonodular tenosynovitis. Given that treatment is required to prevent progressive destruction of the involved joint, tendon or bursa, radiologists must be vigilant for diffuse polyarticular or extrasynovial involvement to optimize patient care and initiate appropriate therapy.

Endoscopic versus open release in patients with de Quervain's tenosynovitis: a randomised trial.

The purpose of this study was to compare the outcome and complications of endoscopic versus open release for the treatment of de Quervain's tenosynovitis. Patients with this condition were randomised to undergo either endoscopic (n = 27) or open release (n = 25). Visual Analogue Scale (VAS) pain and Disabilities of Arm, Shoulder, and Hand (DASH) scores were measured at 12 and 24 weeks after surgery. Scar satisfaction was measured using a VAS scale. The mean pain and DASH scores improved significantly at 12 weeks and 24 weeks (p < 0.001) in both groups. The scores were marginally lower in the e…

De qeurvian tenosynovitis: clinical outcomes of surgical treatment with longitudinal and transverse incision.

OBJECTIVES: De Quervain disease is a mechanical tenosynovitis due to inadequacy volume between abductor pollicis longus, extensor pollicis brevis and their tunnel. Treatment methods include immobilization, steroid injections, and operation. For the first time Fritz De Quervain described surgical treatment of this disease. Since then, various ways of treatment have been reported. The purpose of this study is to compare the clinical outcomes of a longitudinal incision with a transverse incision in De Quervain disease. METHODS: This was a randomized controlled clinical trial conducted in three h…

Corticosteroid injection for treatment of de Quervain's tenosynovitis: a pooled quantitative literature evaluation.

BACKGROUND: There have been many approaches described to treat de Quervain's tenosynovitis, but no definite consensus emerges in the literature. We conducted a pooled quantitative literature evaluation to review the therapeutic studies in the English language to determine the various reported cure rates. METHODS: All citations in the MEDLINE and Ovid databases that addressed de Quervain's tenosynovitis were independently reviewed. Appropriate studies from the bibliographies of these articles were then obtained. Of the 35 articles on de Quervain's temosynovitis that were found in the modern li…

A Standardized Acupotomy Protocol For The Treatment of Tenosynovitis of Hand Flexor Tendons In Human Patients.

Tenosynovitis of hand flexor tendons (THFT) is a high-incidence overuse injury of the hand. Its pathological essence is a dynamic imbalance between the volume of the tendon sheath and the tendon caused by fibrous connective tissue hyperplasia in the A1 pulley region, leading to mechanical entrapment and typical clinical symptoms, including palmar pain, triggering, and limited movement. Traditional conservative treatments yield suboptimal long-term efficacy, while open surgery is associated with high trauma and complication risks, creating demand for a minimally invasive therapeutic approach. A…

The Efficacy of Thumb Spica Casting With or Without Corticosteroid Injection for De Quervain's Tenosynovitis.

Background and objective De Quervain's tenosynovitis is a highly prevalent wrist pathology primarily caused by chronic thumb overuse. Its management typically begins with conservative methods, progressing to corticosteroid injections or surgery if necessary. This study compares the efficacy of thumb spica casting plus corticosteroid injection versus casting alone for treating De Quervain's tenosynovitis. Materials and methods This quasi-experimental study was conducted at the Department of Orthopaedics, Khyber Teaching Hospital, Peshawar, and enrolled adults aged 18-50 who presented with De Qu…
