Perceptions of prognosis and end-of-life care outcomes in patients with advanced lung and gastrointestinal cancer.

BACKGROUND: Many patients with advanced cancer have misperceptions of their prognosis, which may impact end-of-life decision-making. Data regarding associations between prognostic perceptions over time and end-of-life care outcomes are lacking. AIM: To describe patients' perceptions of their prognosis with advanced cancer and examine associations between these perceptions and end-of-life care outcomes. DESIGN: Secondary analysis of longitudinal data from a randomized controlled trial of a palliative care intervention for patients with newly diagnosed incurable cancer. SETTING/PARTICIPANTS:

[Shared experience, multiple experiences: paramedics facing death in intensive care].

Accompanying death is part of the daily life of intensive care workers. However, far from being a trivial experience, it mobilizes the teams emotionally and requires them to constantly adjust in order to carry out their mission of care for the patient and his or her loved ones. How do nurses and orderlies deal with end-of-life care and the particularities of death in an intensive care unit?

When is it time for palliative and end-of-life care in status epilepticus?

Status epilepticus (SE) is a neurological emergency characterized by high rates of short-term and long-term morbidity and mortality. Status epilepticus seems to be a marker of the severity of other underlying conditions rather than a determinant of death on its own. Careful diagnosis and acute treatment of complications and causes of death to SE or its underlying etiology will enable the differentiation of SE patients that would benefit from different levels of treatment intensity. All SE patients should be treated actively with first- and second-line drugs as early as possible. For cases in w

Clinically assisted hydration in patients in the last days of life ('CHELsea II' trial): a cluster randomised trial.

INTRODUCTION: Provision of clinically assisted hydration (CAH) at the end of life is one of the most contentious issues in medicine. The aim of the 'CHELsea II' trial is to evaluate CAH in patients in the last days of life. The objectives are to assess the effect of CAH on delirium, audible upper airway secretions, pain and other symptoms, and overall survival, as well as the tolerability of CAH, and the health economic impact. METHODS AND ANALYSIS: The study is a cluster randomised trial, involving 80 sites/clusters (mainly hospices) and 1600 patients. Sites will be randomised to an interven

How Many Cancer Patients Need Palliative Care? A Population-Based Study.

BACKGROUND: The number of cancer patients potentially amenable to palliative care is conventionally estimated from cancer deaths, as reported in the death certificates. However, a more representative population should also include cancer patients who die from causes other than cancer, as they may develop other life-limiting chronic conditions leading to terminal prognosis. AIM: This study aimed at refining the assessment of the number of cancer patients potentially in need of palliative care, by linked hospital and death data. DESIGN: Retrospective study. SETTING/PARTICIPANTS: Residents in

No difference in effects of 'PACE steps to success' palliative care program for nursing home residents with and without dementia: a pre-planned subgroup analysis of the seven-country PACE trial.

BACKGROUND: 'PACE Steps to Success' is a multicomponent training program aiming to integrate generalist and non-disease-specific palliative care in nursing homes. This program did not improve residents' comfort in the last week of life, but it appeared to improve quality of care and dying in their last month of life. Because this program included only three dementia-specific elements, its effects might differ depending on the presence or stage of dementia. We aimed to investigate whether the program effects differ between residents with advanced, non-advanced, and no dementia. METHODS: Pre-pl

Common elements of service delivery models that optimise quality of life and health service use among older people with advanced progressive conditions: a tertiary systematic review.

INTRODUCTION: Health and social care services worldwide need to support ageing populations to live well with advanced progressive conditions while adapting to functional decline and finitude. We aimed to identify and map common elements of effective geriatric and palliative care services and consider their scalability and generalisability to high, middle and low-income countries. METHODS: Tertiary systematic review (Cochrane Database of Systematic Reviews, CINAHL, Embase, January 2000-October 2019) of studies in geriatric or palliative care that demonstrated improved quality of life and/or he

Palliative medicine and end-of-life care.

Palliative care is an approach to the care of patients, affected by serious illness, and their families that aims to reduce suffering through the management of medical symptoms, psychosocial issues, spiritual well-being, and setting goals of care. Patients and families affected by a neurodegenerative illness have significant palliative care needs beginning at the time of diagnosis and extending through end-of-life care and bereavement. We advocate an approach to addressing these needs where the patient's primary care provider or neurologist plays a central role. Key skills in providing effecti

CE: continuing education article. Integration of palliative care for patients with stage 5 chronic kidney disease.

Patients with advanced chronic kidney disease (Stage 5 CKD) have palliative care needs similar to patients with cancer. The decision not to commence dialysis or to withdraw from active treatment can have a profound impact upon all those closely involved in the patient's care. It is essential that every effort is made to minimise the physical and psycho-social symptoms experienced by patients who require palliative care. Effective teamwork across professional boundaries and specialities will ensure that patients and their families are provided with maximum comfort during their final days. All m

Moral Worlds and Institutional Scripts at the End of Life.

This commentary responds to Jennifer Blumenthal-Barby and colleagues' article, "Can I Get a Witness? The Ethical Dimensions of Family Presence in Patient Suffering." The authors examine clinicians' moral distress when family members are absent from the bedside of dying patients and assess moral arguments for requiring family presence. We concur with their skepticism toward any strong obligation for family members to be physically present when continued treatment is considered burdensome or futile. We extend the authors' analysis by arguing that institutional culture shapes expectations of the

End-of-life prognostic models in advanced cancer: a scoping review of model development, validation, and impact.

BACKGROUND: Accurate survival estimation is important for clinical decision-making in advanced cancer. Prognostic models can support prediction of end-of-life and should ideally undergo staged evaluation across a development framework encompassing model development, validation, and impact evaluation before clinical implementation. No synthesis currently maps existing models across this framework, making it difficult to determine which tools are available and the extent to which they are ready for clinical use. This review aimed to identify end-of-life prediction models for advanced cancer and

The sociality of dying: Contextual philosophical reflections on end-of-life care.

Sociality is an ineluctable aspect of the human condition. Both the meaning of our existence in the world and the meaning of the end of our existence come to us from the other and their death. Sharing these meanings in joy, grief and angst fills the moments of our existence with purpose and takes the sting out of our distress, most of all the distress of dying. The essay critiques social practices that increasingly make possible the denial of care (social death) to the dying or providing them with uncompassionate hyper-care in contexts like India, where a palpable sense of community and family

Post-biographical dignity in the age of artificial intelligence: Narrative, ePROMs and ethical challenges in end-of-life care.

OBJECTIVES: The growing integration of artificial intelligence (AI) and patient-reported digital tools (ePROMs and ePREMs) in palliative care offers new opportunities for personalised care yet also raises profound ethical and philosophical concerns. This paper examines how emerging technologies intersect with the concept of human dignity at the end of life, proposing an expanded notion of post-biographical dignity. METHODS: Ethical-philosophical analysis based on critical readings of AI ethics, narrative medicine, and the philosophy of technology. RESULTS: While digital tools such as ePROMs

[Which interventions are recommended in the last phase of Huntington disease?].

Huntington Disease is a chronically progressive and neurodegenerative disease, which leads to death after 10 to 30 years of suffering and worsening. When suffering Huntington Disease the patient will experience physical, cognitive and psychological issues, so that the patient won't be able to communicate or express his will in the end-of-life-phase of his disease. The problem is, after suffering a while the patient often won't be able to eat by himself, so a feeding tube must be implanted. [Logical Explanation of Medical Condition for Patients with Terminal Cancer at Home-Retrospective Analysis of 145 Cases].

Frequently, patients with advanced cancer are suffered by various symptoms at the end of life. Furthermore, sometime experience unexpected loss of consciousness(LOC)and/or respiratory arrest. By assuming meningeal carcinomatosis, multifocal neurological findings are logically expected. As a result, before the LOC or respiratory arrest, I was able to logically explain to a

Response to Suffering of the Seriously Ill Child: A History of Palliative Care for Children.

Most pediatric clinicians aspire to promote the physical, emotional, and developmental well-being of children, hoping to bestow a long and healthy life. Yet, some infants, children, and adolescents confront life-threatening illnesses and life-shortening conditions. Over the past 70 years, the clinician's response to the suffering of these children has evolved from veritable neglect to the development of pediatric palliative care as a subspecialty devoted to their care. In this article, we review the history of how clinicians have understood and responded to the suffering of children with serio

[An experimental weekly visit of the ward at Higashi Yamato Hospital--a holistic cooperation].

The city of Higashi Yamato is located in the northern part of Tama, Suburbs of Tokyo, and the population of Higashi Yamato is approximately 80,000. The Higashi Yamato visiting nursing station was opened in April 1998. As of April 2006, we have over 100 patients, and the aggregate visiting nursing services have provided more than 600 cases. Our station's uniqueness is that forty percent of the patients have malignant neurological disorders and are terminal stage patients, and that they are all covered by medical care insurance. We also provide nursing services to patients who are expected to be
