Minimally invasive thoracic surgery in pediatric patients: the Taiwan experience.

Minimally invasive technology or laparoscopic surgery underwent a major breakthrough over the past two decades. The first experience of thoracoscopy in children was reported around 1980 for diagnosis of intrathoracic pathology and neoplasia. Up until the middle of the 1990s, the surgical community in Taiwan was still not well prepared to accept the coming era of minimally invasive surgery. In the beginning, laparoscopy was performed in only a few specialties and only relatively short or simple surgeries were considered. But now, the Taiwan's experiences over the several different clinical scen

Cerebrospinal fluid drainage for thoracic and thoracoabdominal aortic aneurysm surgery.

BACKGROUND: During aortic aneurysm surgery, cross-clamping can lead to inadequate blood supply to the spinal cord resulting in neurological deficit. Cerebrospinal fluid drainage (CSFD) may increase the perfusion pressure to the spinal cord and hence reduce the risk of ischaemic spinal cord injury. OBJECTIVES: To determine the effect of CSFD during thoracic and thoracoabdominal aortic aneurysm (TAAA) surgery on the risk of developing spinal cord injury. SEARCH METHODS: For this update the Cochrane Peripheral Vascular Diseases Group Trials Search Co-ordinator searched the Specialised Register

Abdominal wall bulging after thoracic surgery, an underdiagnosed wound complication.

BACKGROUND: Complications after thoracic surgery have well been established, pain being the most prominent. Intercostal nerves are mixed type nerves combining motor and sensory functions. This notion is not consistent with the incidence of PTPS compared to the incidence of muscle paresis or paralysis. We would hypothesize that abdominal wall paresis or paralysis is underdiagnosed. METHODS: In our hospital, three patients developed abdominal wall paralysis after thoracic surgery and consequent nerve damage. Their cases are discussed, and a review of the literature was conducted concerning (int…

Postoperative complications after thoracic surgery in the morbidly obese patient.

Little has been recently published about specific postoperative complications following thoracic surgery in the morbidly obese patient. Greater numbers of patients who are obese, morbidly obese, or supermorbidly obese are undergoing surgical procedures. Postoperative complications after thoracic surgery in these patients that can lead to increased morbidity and mortality, prolonged hospital stay, and increased cost of care are considered. Complications include difficulties with mask ventilation and securing the airway, obstructive sleep apnea with risk of oversedation, pulmonary complications …

Pre-emptive local analgesia in video-assisted thoracic surgery sympathectomy.

OBJECTIVE: Our goal is to determine whether infiltration with a short-acting local anaesthetic such as lidocaine before the surgical incision has a pre-emptive effect on postoperative pain intensity and on incidence of paraesthesia in patients undergoing standard thoracoscopic sympathectomy for palmar hyperhidrosis. MATERIAL AND METHODS: This prospective study includes a consecutive series of 18 patients undergoing bilateral standard thoracoscopic sympathectomy for palmar hyperhidrosis during January 2005-December 2007. Each patient enrolled in the study was randomised to receive pre-incision

Endoscopic thoracic sympathectomy: at what level should you perform surgery?

Several interventions are possible on the sympathetic chain and the nomenclature has been confusing. The authors propose a uniform nomenclature for each procedure, mainly, sympathectomy for resection or ablation of the ganglion, sympathicotomy for the transaction of the chain, ramicotomy for the procedure preserving the chain and ganglia and severing the rami, and finally, sympathetic block for clipping above and below the ganglia. They recommend intervention on the T2 ganglia for facial hyperhidrosis and rubor, on the T3 ganglia for palmar hyperhidrosis, and on the T3 and T4 ganglia for axill

[Surgery for upper or middle thoracic esophageal carcinoma after gastrectomy].

OBJECTIVE: To evaluate the surgical treatment and technical key-points of upper or middle thoracic esophageal carcinoma in patients with history of gastrectomy. METHODS: Eighty-six patients with upper or middle thoracic esophageal carcinoma after previous gastrectomy received surgical treatment between 1980 and 2004. Among them, tumor location was in middle thoracic esophagus in 50 patients, in upper thoracic esophagus in 31 and cervical esophagus in 5. Postoperative pathological staging was stage I in 16 patients, stage IIa in 62, stage IIb in 5 and stage III in 8. The interval between gastr

Surgery for thoracic disc disease. Complication avoidance: overview and management.

The operative approach for discectomy in the treatment of thoracic disc disease has changed from standard laminectomy to a variety of dorsolateral and ventral approaches. The procedure-related complications have been reported in numerous clinical studies over the last seven decades: death, neurological deterioration, postoperative vertebral column instability, incomplete disc resection, cerebrospinal fluid leak and fistula, infection, misdiagnosis, pulmonary embolism, pneumonia, and intercostal neuralgia. The authors conducted a Medline search to identify series reporting clinical data related

[Combined surgery for coronary artery disease and thoracic aortic disease].

Combined surgery in 6 cases who had coronary artery disease and thoracic aortic disease simultaneously was analyzed. Case # 1 had ascending aortic replacement under deep hypothermic circulatory arrest because of iatrogenic aortic dissection caused by aortic clamp during the routine coronary artery bypass grafting (CABG). Case # 2 had DeBakey type II chronic dissection. Case # 3 had type I aortic dissection 4 years after the initial CABG. Both case # 2 and # 3 had ascending aortic replacement under retrograde cerebral perfusion along with CABG. Transverse aortic replacement was performed in cas

[Video-assisted thoracoscopy and thoracic surgery: the first 50 patients].

We report our first full year of clinical experience with video assisted thoracic surgery. From September 1991 to October 1992, 50 patients were treated using endoscopic surgical techniques. There were 29 men and 21 women with a mean age of 48 years, ranging from 17 to 76 years. Drainage and talc poudrage of pleural effusions, with or without pleurodesis, was done in 18 patients (36%), blebectomies with pleurectomy or pleural abrasion was done in 11 patients (22%), and 7 patients (14%) underwent wedge resections of pulmonary lesions. Four other patients (8%) underwent resections or biopsies of…

[Neglected traumatic diaphragmatic rupture in a thoracic surgery patient sample].

Ten outdated traumatic ruptures of the diaphragm are reported in this paper. The accidents had been up to 34 year back. Symptoms were rarely characteristic. Long intervals with no complaint were found to be possible. X-ray techniques were helpful in diagnosis in seven cases. All ten patients underwent thoracotomy.

[Ketamine hydrochloride by continuous drip intravenous infusion in thoracic surgery].

The Authors report an open trial on 50 patients anaesthetized with Ketamine hydrochloride, by continuous drip infusion progressively slowed, with the aid of Fentanil, in thoracic surgery. The proposed anaesthetic technique proved to be excellent as for as analgesia during and after operation as well cardiovascular stability and lackness of side effects is concerned. Recovery from anaesthesia was quick and extremely quite and free from side effects which are common in Ketamine anaesthesia.

Medical Malpractice Claims in Thoracic Surgery From 1986 to 2024.

Thoracic surgery-related malpractice claims recorded in Catalonia (Spain) were collected from the Council of Medical Associations of Catalonia (CCMC, 1986-2024) and the Catalan Health Service (CatSalut, 2017-2024) databases. Of a total of 15,696 claims, 56 (0.4%) corresponded to the field of thoracic surgery. Lung cancer (51.8%) and pneumothorax (8.9%) were the most common diagnoses. Death occurred in 18 patients (31.2%). In relation to the reasons of allegations, in one patient with lung cancer undergoing lobectomy who died in the postoperative period, this information was missing. In the rem…

Thoracic and retroperitoneal lymphatic injuries following spine surgery: illustrative cases.

BACKGROUND: Postoperative lymphatic leakage is a rare complication of spine surgery caused by injury to the thoracic duct, cisterna chyli, or retroperitoneal lymphatic channels. Because of its rarity, diagnosis is often delayed and optimal management remains poorly defined. The authors present 3 illustrative cases and review current diagnostic and therapeutic strategies. OBSERVATIONS: A 66-year-old man developed a right chylothorax after posterior T2-3 corpectomy with costotransversectomy and cervicothoracic fusion for metastatic spinal cord compression. Following failed thoracic duct emboliz
