Patients with subacute, granulomatous thyroiditis usually present with a typical clinical picture of fever, anterior neck pain and an exquisitely tender thyroid. Abnormal thyroid function tests and suppressed radio-iodine uptake strongly support the diagnosis. Occasionally the disease may simulate systemic or malignant disease when it presents atypically, without signs or symptoms directly related to the thyroid, We report a 43-year-old man who presented with fever of unknown origin. A diagnosis of silent thyroiditis was made from evidence of biochemical thyrotoxicosis, intense gallium-67 citr

The term "thyroiditis" refers to several syndromes of differing etiology. Chronic or Hashimoto's thyroiditis (HT), an autoimmune disorder that is manifested by goiter and hypothyroidism, is by far the most common of these syndromes. Subacute thyroiditis (SAT) encompasses two distinct syndromes: subacute granulomatous thyroiditis (SAGT) and subacute lymphocytic thyroiditis (SLT). SAGT is viral in origin and usually presents with neck tenderness and hyperthyroid symptoms, while SLT, which is likely to be an autoimmune entity, results in goiter and transient hyperthyroidism. SLT is often seen in

A patient with subacute thyroiditis developed a thyroid abscess after drainage of a pilonidal abscess. An infective focus in a patient with subacute thyroiditis on steroids should be treated aggressively with adequate antibiotic cover.

Eighty consecutive patients with typical subacute thyroiditis were evaluated. Sex distribution showed a higher incidence in females (F/M 3.2/1), with a mean age of 44 yr. In the majority of patients (51/80 = 66%) the onset of the disease was between June and September (46% in July and August). The remaining cases were distributed in the other months without a clear monthly prevalence. These results indicate that the seasonal distribution of subacute thyroiditis is almost superimposable to that of established infections due to some enteroviruses (Echovirus, Coxsackie A and B viruses), suggestin

This description of 8 cases of thyroid nodules secondary to subacute thyroiditis serves to emphasise the relative prevalence of the circumscribed forms. These nodules, variable in size, firm and painful, developed after a period of cervical discomfort and pain. A contrast between a raised or normal blood hormone level and a very low uptake of I131 was found in five cases. It was all the more valuable when there was no iodine overload. Scintigraphy showed the nodule to be cold in 6 cases, isofixing in one and hot in one case. An essential diagnostic criterion is the rapidly favourable course: 4

Six patients with subacute thyroditis were followed with serial measurements of T4, FT4, TSH and RAI uptake. Five of the six underwent TRH stimulation early in the course of their illness. All six patients had elevated or high normal values for T4 and FT4 at the time of their clinical presentation (mean = 13.8 microgram per 100 ml and 3.9 ng per 100 ml, respectively). RAI uptakes were 1% or less in all six. TRH testing revealed a suppressed TSH response (mean deltaTSH less than0.1 muU/ml) in all five patients tested, suggesting hyperthyroidism. After initial studies were performed, five patien

The author made an electron microscopic observation on multinucleated giant cells which appear in the thyroid gland in subacute thyroiditis (de Quervain). Numerous typical desmosomes, which intermingled with myriad of mitochondria and small electron dense granules, were found in the cytoplasm of giant cell. Desmosome is one peculiar to the intercellular junction of follicular epithelium of the thyroid gland, and the fact that many desmosomes were found in the cytoplasm is one of the proofs showing this giant cell to be possibly derived from the follicular epithelium.

OBJECTIVE: Subacute thyroiditis (SAT) is a differential diagnosis in the clinical care of hypo- and hyperthyroid patients, but large-scale population data on the incidence, patient characteristics, and outcome are limited. Thus, the aim was to determine the incidence of SAT in the Danish population and to characterise patient demographics and the risk of permanent hypothyroidism.
DESIGN: Retrospective cohort study based on Danish nationwide health registers.
PATIENTS: Individuals diagnosed with SAT at Danish hospitals from 1995 to 2016, covering the years before and after the implementation
