We evaluated two patients who developed hypotonic maculopathy following trabeculectomy and adjunctive 5-fluorouracil injections. Both patients had ocular hypotony and decreased visual acuity following these injections. The first case showed marked choroidal foldings, retinal striae, increased retinal vascular tortuosity, engorgement and optic disc oedema. A 71-year-old white woman with primary open angle glaucoma received trabeculectomy with antimetabolite. After laser suture lysis, intraocular pressure has remained at 1-2 mmHg for 23 months and 12 months in the right and left eye, respectively. However, visual acuity has remained at the preoperative levels of 6/6 and 6/7.5 in the right and left eye, respectively, without hypotony maculopathy in either eye.

In a prospective study 35 eyes of 25 patients with coexisting cataract and glaucoma underwent trabeculectomy, phacoemulsification and implantation of a folded polyHema intraocular lens through the trabeculectomy opening. Follow-up ranged from 6 to 27 months (mean 13.3). Trabeculectomy was performed in 24 eyes affected by primary open angle glaucoma. Visual fields were measured using automated perimetry preoperatively and in the first, third, sixth and twelfth months after surgery. Significant improvement in visual field occurred in 96% of cases.

A 66-year-old pseudophakic man with primary open-angle glaucoma and ocular cicatricial pemphigoid underwent trabeculectomy and was given postoperative subconjunctival 5-fluorouracil. He developed an inferior corneal epithelial defect and, later, a well-demarcated area of inferior corneal keratinization. Since the corneal keratinization clinically improved with conservative management, surgical intervention was not required.

This retrospective study evaluated the association between preoperative topical glaucoma medication containing preservatives and long-term trabeculectomy outcomes in 309 eyes from 201 patients with primary open-angle glaucoma, followed for over 10 years.
