Comparison of Therapeutic Response and Clinical Outcome between HCV Patients with Normal and Abnormal Alanine Transaminase Levels.

BACKGROUND AND AIMS: Patients with chronic hepatitic C (HCV) infection and normal serum alanine transaminase (ALT) levels were considered to have mild disease. In Taiwan, these patients were not suggested for interferon (IFN) based therapies. The aim of study is to compare therapeutic outcomes between HCV patients with normal and elevated ALT levels. METHODS: We conducted a retrospective study on 3241 HCV patients treated by IFN based therapies. Patients with normal ALT levels were classified as group A (n = 186) while those with elevated ALT levels were group B (n = 3055). RESULTS: At basel

Serum alanine transaminase total bilirubin concentrations predict CYP3A activity as measured by midazolam and 1'-hydroxylation.

BACKGROUND: Microsomal enzyme P450 (CYP450) plays an important role in metabolism of most xenobiotics. The activity of CYP3A decreases in patients with liver dysfunction. However, whether serum concentrations of liver enzymes reflect the activity of CYP3A is unclear. We aimed to search for a new clue to predict the activity of CYP3A and guide clinical medication. MATERIAL AND METHODS: Forty-five patients undergoing surgery under general anesthesia were enrolled in the study, including 15 cases with normal liver function (Group N), 15 cases with moderate fatty liver according to both the resul

Alanine transaminase has opposite associations with death from diabetes and ischemic heart disease in NHANES III.

PURPOSE: Diabetes increases the risk of ischemic heart disease (IHD). Stringent control of diabetes does not reliably reduce cardiovascular events. Some global regions, such as East Asia, have low mortality rates from IHD and high rates of diabetes. We hypothesized that some aspects of liver function might underlie this paradox. METHODS: We used multivariable proportional hazards regression in 16,865 adults from the National Health and Nutrition Examination Survey (NHANES) III (1988-1994) followed until December 31, 2006, to assess the adjusted associations of gender-specific tertiles of alan

[Histological changes in the livers of chronic hepatitis B patients with persistently normal serum alanine transaminase levels].

OBJECTIVE: To study the histological changes in livers of chronic hepatitis B (CHB) patients with persistently normal serum ALT levels (PNAL). METHODS: 274 CHB patients who had percutaneous liver biopsies and had a detectable viral load (lower limit of detection is 10(3) copies/ml) in our department between October 2003 and February 2007 were included in this study. Among these patients, 139 had PNAL, group A, (with at least 3 normal serum ALT levels, with intervals of more than two months over a period of 12 or more months before the biopsy). The other 135 patients, group B, had abnormal ser

Differential effects of diclofenac and aspirin on serum glutamic oxaloacetic transaminase elevations in patients with rheumatoid arthritis and osteoarthritis.

OBJECTIVE: To examine elevations in levels of serum glutamic oxaloacetic transaminase (SGOT) in patients with rheumatoid arthritis (RA) and osteoarthritis (OA) taking placebo, aspirin, or diclofenac, and to seek possible explanations for the occurrence of these elevations. METHODS: We conducted a meta-analysis of individual case reports from 3 RA protocols and 5 OA protocols, encompassing 814 diclofenac-treated patients, 443 aspirin-treated patients, and 359 placebo-treated patients. All of the RA protocols had nearly identical inclusion and exclusion criteria, as well as safety studies and f

Lactate dehydrogenase and aspartete transaminase of the cerebrospinal fluid in patients with brain tumours, congenital hydrocephalus, and brain abscess.

The diagnostic value of CSF lactate dehydrogenase and aspartate transaminase in cases of brain tumours (except for CSF AST in the benign tumours), congenital hydrocephalus, and brain abscess is established. Tumour cyst fluids show a higher enzymatic activity than does the CSF. The two enzyme estimations do not help in differentiating the supratentorial from the infratentorial tumours. CSF AST is superior to CSF LD in discriminating the malignant and benign tumours, in so far as the AST is increases selectively in malignancy. Estimates of CSF LD are slightly superior to those of CSF AST, both i

Prevalence of elevated liver transaminases and their relationship with alcohol use in people living with HIV on anti-retroviral therapy in Uganda.

BACKGROUND: Isoniazid preventive therapy (IPT) reduces tuberculosis reactivation and mortality among persons living with HIV (PLWH), yet hepatotoxicity concerns exclude "regular and heavy alcohol drinkers" from IPT. We aimed to determine the prevalence of elevated liver transaminases among PLWH on antiretroviral therapy (ART) who engage in alcohol use. SETTING: The Immune Suppression Syndrome Clinic of Mbarara, Uganda. METHODS: We defined elevated liver transaminases as ≥1.25 times (X) the upper limit of normal (ULN) for alanine aminotransferase (ALT) and/or aspartate aminotransferase (AST).…

Acute exacerbation of hepatitis in a boy with chronic HCV genotype-2 infection after a 9-year period of normal transaminases levels.

We report on a boy with HCV genotype-2a infection who presented with a severe degree of ALTflare after 9 years of asymptomatic period and was treated with pegylated interferon and ribavirin. We suggest that children with HCV genotype-2 infection who show a significant ALT flare should be treated with anti-viral therapies in order to avoid progression of hepatic fibrosis.

Cerebroprotective effects of glutamate-oxaloacetate transaminase enzyme in ischemic stroke: Systematic review and meta-analysis of preclinical studies.

While independent preclinical studies have reported beneficial effects of glutamate-oxaloacetate transaminase (GOT) in models of ischemic stroke, these findings remain dispersed across the literature. This PRISMA-compliant systematic review and meta-analysis aims to combine data from these studies to determine GOT's therapeutic potential in ischemic stroke, focusing on the effects on lesion size and neurofunctional deficit. After screening results from searches in PubMed and Web of Science, and citation chasing, 12 eligible studies were included. Range of evidence met nine out of 13 STAIR crit

Protein-engineered shift of the reaction equilibrium in transaminase reaction direction enables the asymmetric synthesis of (R)-1-phenylethylamine.

(R)-1-Phenylethylamine [(R)-1-PEA] is an important chiral compound widely used in pharmaceuticals, chemical synthesis, and materials science. ω-Transaminases are valuable biocatalysts for the asymmetric amination of ketone substrates under mild conditions and represent an attractive tool for the green synthesis of chiral amines. However, in natural (R)-selective transaminases, acetophenone (AP) is more commonly produced as the deamination product of (R)-1-PEA, whereas its amination as an amino acceptor is generally inefficient. In this study, an (R)-selective ω-transaminase from Thermomyces st…

Engineering an (R)-Selective Transaminase for High-Substrate-Loading Asymmetric Amination of a Sterically Demanding N-Heterocyclic Ketone.

The biocatalytic asymmetric amination of bulky N-heterocyclic ketones is often limited by low enzyme activity and poor performance at high substrate loading. Herein, we developed an efficient (R)-selective transaminase for the synthesis of (R)-1-Boc-3-aminopiperidine through an integrated strategy combining phylogeny-guided enzyme mining, structure-guided engineering, and mechanistic analysis. Screening of transaminases identified MbATA as the "best" parent enzyme, affording the target amine from N-Boc-3-piperidone (1B3OP) in specific activities of 0.269 U·g-1 and > 99% ee. Subsequent engineer…

Development of prediction model for alanine transaminase elevations during the first 6 months of conventional synthetic DMARD treatment.

Frequent laboratory monitoring is recommended for early identification of toxicity when initiating conventional synthetic disease-modifying antirheumatic drugs (csDMARDs). We aimed at developing a risk prediction model to individualize laboratory testing at csDMARD initiation. We identified inflammatory joint disease patients (N = 1196) initiating a csDMARD in Turku University Hospital 2013-2019. Baseline and follow-up safety monitoring results were drawn from electronic health records. For rheumatoid arthritis patients, diagnoses and csDMARD initiation/cessation dates were manually confirmed.

Lipids and transaminase elevations in ARV-experienced PLWH switching to a doravirine-based regimen from rilpivirine or other regimens.

BACKGROUND: Doravirine (DOR) is a newly approved antiretroviral belonging to the class of non-nucleoside reverse transcriptase inhibitors (NNRTI), well tolerated and leading to an improved lipid profile in antiretroviral experienced people living with HIV (PLWH). We aimed at evaluating if the lipid-lowering effect is linked to the drug class, using real-life data from the SCOLTA cohort. METHODS: We compared the lipid profile modifications in experienced PLWH switching to a DOR-based regimen from rilpivirine or another NNRTI-based regimen or from an integrase strand transferase (INSTI)-based r

Effect of illness uncertainty on alanine transaminase levels and aspartate aminotransferase levels in patients with nonalcoholic fatty liver disease.

Patients with nonalcoholic fatty liver disease (NAFLD) have illness uncertainty. The purpose of this longitudinal study was to investigate the effect of the degree of illness uncertainty in patients with NAFLD on liver function values. We conducted a questionnaire survey and collected blood samples from outpatients with NAFLD. The items in the questionnaire were measured for illness uncertainty using the Japanese version of the Mishel Uncertainty in Illness Scale-Community (MUIS-C). Blood samples were collected at baseline and after 1 year. We divided the patients into two groups: one with hig
