Transition of care to an adult spina bifida clinic: patient perspectives and medical outcomes.

PURPOSE: Transitioning care of patients with spina bifida to adult centers poses a challenge. We sought patient and parent perspectives on the transition process at our center and correlated these perspectives with medical outcomes. MATERIALS AND METHODS: Patients who attended the adult spina bifida clinic at our institution were invited to complete SF-36®, the Ambulatory Care Experience Survey and a Transition of Care Survey. Urological and neurosurgical medical outcomes were correlated with health status, clinic experience and perspectives on the transition process. Statistical analysis was

[Care continuity for patients with myasthenia gravis during transition from childhood to adulthood].

The onset of myasthenia gravis in Japan shows two peaks, the highest in childhood, which has specific HLA association. The clinical types are ocular, latent general and general types, latent general is most frequently observed. Clinical characteristics and effects of the treatments are associated with the clinical types and ages at onset. Anti-acetylcholine receptor antibody is negative to low positive. Age-dependent changes, i. e., higher as age advances are observed, and general type shows higher titer than other types. Treatments consist of anticholinesterase, steroid and thymectomy. Most o

[Care continuity for patients with eating disorder during transition from childhood to adulthood].

Eating disorder is mainly seen at the younger woman. In recent years, there has been a growing population of younger or elder women. In children, most of the eating disorder is anorexia nervosa(AN), but if eating disorders are prolonged, these switched over to ANBP and bulimia nervosa(BN). When a psychological problem becomes bigger that abnormal eating behavior and psychiatric disorder stand out at ANBP and BN, we can't answer as the pediatrics, and it's taken over psychiatry and the department of psychosomatic medicine. Because ego and the women's establishment and the lifestyle are related

[Care continuity for patients with cerebral palsy during transition from childhood to adulthood].

Around adolescence and thereafter, many cases with severe cerebral palsy have worsening of respiration and swallowing due to worsening of deformity and other factors. Appropriate management including prone positioning, naso-pharyngeal air-way, and positive pressure breathing with mask and bag or in-ex sufflator is effective for chronic respiratory disorder. Modification of posture or food texture according to the result of video-fluorographic examination may be effective for dysphagia along with intermittent oral catheterization feeding. Gastro-esophageal reflux and stasis in duodenum get wors

Transition of new residents to long-term care: basing practice on residents' perspective.

Admission to a long-term care facility is a time of disorganization and adaptation. New residents experience transition breakdown beginning with a sense of homelessness as they learn the rules of institutional living. With the passage of time, residents may be able to create a place of their own. Story sharing is an important way to help new residents integrate into the unfamiliar long-term care facility.

Recipients of bone marrow transplants: making a smooth transition to an ambulatory care setting.

Health care is changing rapidly due in part to Diagnostic Related Groups (DRGs), patients becoming consumer-oriented, and the Acquired Immune Deficiency Syndrome (AIDS) crisis. Patients now spend less time in acute care settings and more time recovering in an ambulatory care environment. Patients undergoing bone marrow transplants have a long, stressful, and expensive course of recovery. Patients who have had transplants are similar to other groups of patients with cancer due to the severe immunosuppression they experience. Discharge teaching is begun once granulocyte counts start to rise, and

Improving Access to Surgical Care Through a High-Volume, Operating Room-Based Hysteroscopy Program: A Transitional Model Towards Outpatient Delivery.

OBJECTIVES: Outpatient hysteroscopy offers a potential solution to improve access to gynaecologic surgical care; however, its widespread adoption remains limited in Canada. This quality improvement study aims to assess safety and efficiency outcomes of a dedicated high-volume operating room (OR)-based transitional hysteroscopy program. METHODS: A retrospective observational study of patients who underwent elective hysteroscopy at a single tertiary care centre in Toronto, Canada, between April 3, 2023 and May 9, 2024 was performed. The preimplementation group (n = 251) comprised patients who u

Evaluation of a nurse practitioner-led post-discharge transitional care program for patients with liver disease: A retrospective cohort study.

BACKGROUND: We evaluated an outpatient, nurse practitioner-led transitional care program with respect to its efficacy in reducing unplanned readmission rates for patients with liver disease. METHODS: We conducted a retrospective cohort study using data from an academic health system in Toronto, Ontario. The study included all admissions associated with an ICD10 code of R18 (ascites), I85.0 or I98.3 (variceal bleeding), or K70-K77 (diseases of the liver). Patients were selected to receive the transitional care (intervention group) or not (no-intervention group) by discretion of the hepatologis…

Preferences for transitional HIV care among people living with HIV recently released from prison in Zambia: a discrete choice experiment.

INTRODUCTION: No studies from sub-Saharan Africa have attempted to assess HIV service delivery preferences among incarcerated people living … ing preferences for 12 systematically designed choice scenarios, each presenting three hypothetical transitional care options. Options combined six attributes: (1) clinic type for post-release HIV care; (2) client focus of healthcare workers; (3) transitional care model type; (4) characteristics of transitional care provider; (5) type of transitional care support; …

Treatment guidelines for persistent cloaca, cloacal exstrophy, and Mayer-Rokitansky-Küster-Häuser syndrome for the appropriate transitional care of patients.

We developed treatment guidelines (TGs) for appropriate transitional care of the genitourinary system in patients with persistent cloaca (PC), cloacal exstrophy (CE), or Mayer-Rokitansky-Küster-Häuser syndrome (MRKH). These TGs are in accordance with the Medical Information Network Distribution Service (Minds), published in 2014 in Japan. Clinical questions (CQs) concerning treatment outcomes of the genitourinary system, pregnancy and delivery, and quality of life in adulthood were prepared as six themes for PC and CE and five themes for MRKH. We were able to publish statements on chronic rena…

[Transition: between recommendations and real-world care : Results from the InfoTrans project].

BACKGROUND: The transition from pediatric to adult rheumatology care represents a particular challenge for young people with juvenile idiopa … ressive symptoms. CONCLUSION: Relevant discrepancies remain between recommendations for structured transitional care and actual care delivery. Disease-related knowledge appears to be a central factor for continuous follow-up in rheumatology. The findings underline the importance of early patient education and better integration of support services.
