Joint-brain communication in rheumatoid arthritis: glial mechanisms, pain phenotypes, and translational evidence.

Rheumatoid arthritis (RA) is a systemic autoimmune disease in which persistent pain, fatigue, cognitive impairment, depression, and anxiety may involve altered central nervous system (CNS) processing. This narrative review synthesizes preclinical and clinical evidence concerning proposed glial cell-mediated joint-brain communication in RA. Joint-derived neural inputs and systemic inflammatory signals may communicate with the CNS through humoral, cellular, neural, neuroendocrine, and neurovascular pathways. Preclinical studies implicate microglia and astrocytes in neuroinflammatory responses, s

Advancing hepatocyte-based therapies: A translational perspective.

Hepatocyte-based therapies represent a promising alternative to liver transplantation, yet their clinical translation is constrained by the limited availability of functional cells and inefficient engraftment. Here, we review progress in the field from a translational perspective, focusing on strategies to overcome these core challenges. We analyze emerging cell sources derived from stem cell technologies and assess their therapeutic potential. These translational efforts are organized around two clinical paradigms: hepatocyte replacement for long-term functional correction and temporary hepat

Targeting metabolic reprogramming to enhance adoptive immunotherapy: emerging mechanisms and translational perspectives.

BACKGROUND: Adoptive cell therapy (ACT) has achieved breakthrough advances in the treatment of malignancies, demonstrating particularly remarkable efficacy in hematologic cancers. However, the therapeutic efficacy of ACT remains limited by multiple factors, including low tumor antigen expression, the limited in vivo persistence of infused immune cells, and the immunosuppressive characteristics of the tumor microenvironment (TME). Meanwhile, metabolic reprogramming, a hallmark of cancer, plays a pivotal role in immune cells by directly regulating their differentiation, functional states, and lo

The Translational Medicine Regarding Ozone in Saline Solutions. Comment on Armeli et al. Ozone Saline Solution Polarizes Microglial Cells Towards an Anti-Inflammatory Phenotype. Molecules 2025, 30, 3932.

This commentary critically evaluates the translational relevance of a recent study investigating the effects of ozonated saline solution (O3SS) on microglial and endothelial cell models. While the original research proposes potential antioxidant and anti-inflammatory benefits of low-dose ozone exposure, we identify significant methodological and conceptual flaws that undermine its conclusions. Key concerns include the unjustified assumption that ozone behaves similarly in microwell cultures and clinical infusion settings, despite known physicochemical differences affecting ozone stability and

Live tissue microbiota and bacterial translocation: mechanisms and translational perspectives in cardiometabolic diseases.

The long-standing view of sterile internal tissues has been challenged by accumulating evidence that microbial material, and under specific conditions, viable bacteria may translocate from the gut and oral cavity into metabolic and cardiovascular tissues. This review synthesizes current knowledge on the mechanisms underlying bacterial translocation and its implications for cardiometabolic disease, complemented by original experimental data. Dysbiosis and epithelial barrier disruption facilitate the passage of microbial components and, in some settings, culturable bacteria across mucosal surfac

Circulating C1QC in cancer: analytical platforms, evidence gaps, and prospects for clinical chemistry translation.

Traditional serum tumor markers, such as carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA 19-9), cancer antigen 125 (CA 125), α-fetoprotein (AFP), and prostate-specific antigen (PSA), exhibit limited diagnostic sensitivity and specificity during the early stages of disease and in malignancies with overlapping clinical features. This limitation has led to the investigation of complement pathway proteins as potential clinical chemistry analytes. Measurements of circulating complement C1q C chain (C1QC) are frequently derived from total C1q or C1q-binding activity assays, rather than

Matrix metalloproteinases in intestinal fibrosis and fibrostenosing Crohn's disease: Current evidence, mechanistic hypotheses, and translational challenges.

Fibrostenosing complications in Crohn's disease arise from mixed transmural remodeling that includes inflammation, fibrosis, and muscular hypertrophy/hyperplasia. Matrix metalloproteinases (MMPs) are implicated in extracellular matrix turnover and inflammatory remodeling, but their precise role in stricture evolution remains incompletely defined. We review current evidence (2020-2026) on MMP biology in intestinal fibrosis and fibrostenosing disease, emphasizing assay limitations, tissue-layer heterogeneity, and the predominance of cross-sectional rather than longitudinal human data. This revie

Ferroptosis in cancer: integrating multi-omics, AI-driven biomarkers, and translational challenges.

Ferroptosis, an iron-dependent regulated cell death driven by lipid peroxidation, has emerged as a potential target in cancers resistant to apoptosis and other programmed cell death pathways. This review integrates classical mechanistic insights with multi-omics and artificial intelligence (AI) approaches to examine ferroptosis regulation, biomarker identification, and therapeutic potential across malignancies. Core regulators (GPX4, SLC7A11, ACSL4, FSP1, labile iron pool) interact with oncogenic pathways (RAS, p53, NRF2, YAP/TAZ) to influence sensitivity. Multi-omics integration and AI models

Translational changes: evidence around efficacy of red-light therapy in myopia management.

The global prevalence of myopia has continued to increase significantly over time. By 2050, it has been estimated that approximately half of the world's population will be myopic. Additionally, 10% of myopes will be highly myopic, which greatly increases the risk of sight-threatening ocular complications including retinal detachments and myopic maculopathies. Significant global efforts have been invested in the development of interventions and strategies to prevent myopia onset and progression. Several different myopia control treatments are now available to clinicians for this purpose. Additi

Physical activity and lncRNA-mediated regulation in Parkinson's disease: Mechanistic insights and translational perspectives.

Parkinson's disease (PD) is characterized by dopaminergic neurodegeneration, α-synuclein aggregation, mitochondrial dysfunction, and neuroinflammation contributing to motor and non-motor impairment. Beyond pharmacological management, structured physical activity has been associated with biological adaptations relevant to these processes, including modulation of neurotrophic signaling, mitochondrial function, and inflammatory pathways. Emerging evidence suggests that long noncoding RNAs (lncRNAs) participate in these responses through transcriptional and epigenetic regulation, although current

Bispecific antibody-drug conjugates in cancer: therapeutic-window determinants, tumour-immune interactions, and clinical translation.

Antibody-drug conjugates (ADCs) have established targeted payload delivery as an important anticancer strategy, but their efficacy remains constrained by antigen heterogeneity, incomplete internalisation, resistance and dose-limiting toxicity. Bispecific antibody-drug conjugates (BsADCs) add a second antigen- or epitope-binding specificity with the aim of broadening tumour coverage or altering receptor trafficking. The field crossed a major translational threshold in 2026, when izalontamab brengitecan (iza-bren; BL-B01D1), an EGFR×HER3 BsADC, received its first regulatory approvals in China fo

Advances in Photoacoustic Imaging for Brain Diseases: Principles, Applications, and Clinical Translation Challenges.

The growing prevalence of brain diseases and the limitations of existing imaging techniques highlight the urgent demand for advanced, biocompatible, and high-resolution brain imaging approaches. Photoacoustic imaging (PAI) uniquely integrates scalable spatial resolution, deep penetration, and non-ionizing excitation, which enables multi-scale visualization from subcellular structures to the whole brain. This review systematically summarizes the fundamental principles of PAI, commonly used contrast agents, and key system components, and provides a comparative analysis of the major imaging modal

PANoptosis as a drug discovery framework: integrating cell death architecture with clinical translation.

Programmed cell death pathways-apoptosis, pyroptosis, and necroptosis-were long regarded as discrete entities, yet mounting evidence reveals their convergence in PANoptosis, a unified inflammatory death program orchestrated by supramolecular PANoptosome complexes. By integrating upstream sensors (ZBP1, AIM2, NLRP3, Pyrin), scaffolding adaptors (ASC, RIPK1, FADD), and executioners (caspase-1/8, RIPK3-MLKL, gasdermins), PANoptosis operates as a fail-safe against pathogens and oncogenic stress, but also drives immunopathology in sterile injury, sepsis, cancer, and neurodegeneration. This review s

Knowledge Mapping of Human Genetic Variation in Multiomics Biomarker Translation: A Bibliometric Analysis.

Human genetic variation and multiomics biomarker translation has emerged as a rapidly developing research field at the intersection of molecular medicine, bioinformatics, and clinical decision-making. This study used bibliometric methods to map its global structure, intellectual base, and thematic evolution. The eligibility framework was deliberately restricted to studies in which inherited or somatic human genetic variation was substantively linked to omics-integrated biomarker discovery, functional interpretation, clinical stratification, or therapeutic response; generic omics studies withou

Evolution of Hsp90 targeting: From stress biology to clinical translation.

Hsp90 inhibitors represent a decades-long experimental framework that has progressively uncovered how molecular chaperone systems are organized, regulated, and rewired in disease. Early natural products established that pharmacologic engagement of Hsp90 simultaneously destabilizes broad client networks and exposes a central layer of proteostasis control. Subsequent structural, biochemical, and translational studies revealed the mechanistic importance of nucleotide-driven conformational cycling, cochaperone exchange, paralog specialization, and subcellular compartmentalization, as well as the e

Pleiotropic mechanisms and translational trajectory of SGLT2 inhibitors in sepsis-induced cardiomyopathy: Navigating the evidence hierarchy.

Sepsis-induced cardiomyopathy (SICM) is a common but heterogeneous syndrome of acute myocardial dysfunction characterized by inflammatory dysregulation, endothelial and microcirculatory injury, mitochondrial bioenergetic failure, ionic instability, and regulated cell injury. Although recognition of SICM is increasing, management remains largely supportive, and no therapy has yet been established to directly modify myocardial vulnerability. Sodium-glucose cotransporter 2 inhibitors (SGLT2i), initially developed as glucose-lowering agents, exert pleiotropic cardiovascular effects that intersect

Engineering immune-competent niches: strategies, applications, and translational hurdles in ovarian cancer organoid models.

Ovarian cancer remains the most lethal gynecologic malignancy due to strong interpatient heterogeneity and immune evasion. Traditional two-dimensional cultures and animal models lack the ability to maintain interactions among tumors, immune cells, and stromal cells and have limitations in clinical translation. This review discusses the organoid construction methods using adult stem cells (normal epithelium, tumor tissues and ascites), and induced pluripotent stem cells, comparing various culture platforms from air-liquid interface to microfluidic devices. We highlight organoids containing immu

Aberrant Fear: Biological Underpinnings Relevant to Psychosis, Antipsychotic Drugs, and Psychotherapeutic Treatments, a Translational Approach.

Fear is a transdiagnostic construct implicated in multiple psychiatric disorders, reflecting a partial dissociation between clinical phenotypes and underlying neurobiological mechanisms. Converging evidence suggests that aberrant fear processing plays a central role in cognitive and psychopathological models of psychosis. In this narrative review, we synthesize evidence on the neurobiological mechanisms of aberrant fear modulation in schizophrenia from a translational perspective, integrating findings from neuroimaging, preclinical models, pharmacological interventions, and psychotherapy.
