Helmet use patterns in urban trauma centers in New York City: A retrospective review of patients from 2013 to 2022.

BACKGROUND: There has been a recent rise in micromobility device use, which can be attributed to the increase in delivery services, faster 2-wheeled vehicles, and COVID. However, the rate of helmet use is largely unknown. We aim to document the trends and identify risk factors for helmet use over the last decade in urban trauma centers in New York City to find opportunities for intervention. METHODS: A bi-institutional retrospective review of patients from January 1, 2013, to January 1, 2023, in a level I and II urban trauma center who were in motorcycle or motorized vehicle collisions was pe…

Association of Whole Blood With Survival Among Patients Presenting With Severe Hemorrhage in US and Canadian Adult Civilian Trauma Centers.

IMPORTANCE: Whole-blood (WB) resuscitation has gained renewed interest among civilian trauma centers. However, there remains insufficient evidence that WB as an adjunct to component therapy-based massive transfusion protocol (WB-MTP) is associated with a survival advantage over MTP alone in adult civilian trauma patients presenting with severe hemorrhage. OBJECTIVE: To assess whether WB-MTP compared with MTP alone is associated with improved survival at 24 hours and 30 days among adult trauma patients presenting with severe hemorrhage. DESIGN, SETTING, AND PARTICIPANTS: This retrospective co…

Rates of nonoperative management and secondary overtriage in patients with spine fracture transferred to level I or II trauma centers.

OBJECTIVE: Traumatic spine fractures are common in trauma patients, and with an aging population, the incidence of spine fractures will continue to increase. However, wide variability in injury severity can lead to unnecessary transfers to higher-level trauma centers (TCs), potentially resulting in the overuse of healthcare resources. The primary objective of this study was to assess the rate of nonoperative management and the extent of secondary overtriage (SOT) among patients with traumatic spine fractures. The secondary objective was to identify patient and hospital factors associated with …

An evaluation of the variability in the number of high-level trauma centers per million people across the United States.

INTRODUCTION: The number of High-Level Trauma Centers (HLTCs) (Level I or II) per million people across the United States (US) is variable. The purpose of this study was to describe that variability and to determine if it was correlated with state characteristics. METHODS: A cross-sectional study of the US was performed utilizing census data. The number of HLTCs per million people in each state was calculated. The correlation between the number of HLTCs per million and the following variables was evaluated for: population density, black and white race, land area, median household income, empl…

Disparities in provision of rehabilitative services in adolescents with traumatic brain injury at level I adult versus pediatric trauma centers.

BACKGROUND: Rehabilitative care after traumatic brain injury (TBI) in children is a high-priority research area. Children often receive different care (e.g., imaging, operative management) when treated in adult trauma centers (ATCs) versus pediatric trauma centers (PTCs). Adolescent patients may be cared for in PTCs or ATCs depending upon local trauma triage criteria. Possibly appearing physically mature, adolescents are still developing. Neurologically focused rehabilitation, vital for optimal recovery, requires a different rehabilitative focus in adolescents as compared with adults. The purp…

Multicenter study of US trauma centers examining the effect of the COVID-19 pandemic on injury causes, diagnoses and procedures.

BACKGROUND: The COVID-19 pandemic resulted in nationwide social distancing and shelter-in-place orders meant to curb transmission of the SARS-CoV-2 virus. The effect of the pandemic on injury patterns has not been well described in the USA. The study objective is to determine the effect of the COVID-19 pandemic on the distribution and determinants of traumatic injuries. METHODS: This retrospective multi-institutional cohort study included all hospital admissions for acute traumatic injury at six community level I trauma centers. Descriptive statistics were used to compare injury causes, diagn…

Admission Hypothermia in Trauma Patients Undergoing Prehospital Tracheal Intubation: 15-Year Review of a Level-1 Trauma Center.

OBJECTIVES: The adverse role of accidental hypothermia in trauma patients has been studied for decades while patients undergoing prehospital tracheal intubation are at particular risk due to impaired temperature autoregulation. The primary objective of the study was to determine the prevalence and risk factors associated with admission hypothermia (body temperature <35 °C) at the emergency department. Secondary objectives included the assessment of the association of hypothermia with all-cause mortality, transfusion requirement, intensive care unit length of stay (ICU LOS), and duration of mec

Quality of Integration of Air Force Trauma Surgeons Within the Center for Sustainment of Trauma and Readiness Skills, Cincinnati: A Pilot Study.

INTRODUCTION: While previous studies have analyzed military surgeon experience within military-civilian partnerships (MCPs), there has never been an assessment of how well military providers are integrated within an MCP. The Center for Sustainment of Trauma and Readiness Skills, Cincinnati supports the Critical Care Air Transport Advanced Course and maintains the clinical skills of its staff by embedding them within the University of Cincinnati Medical Center. We hypothesized that military trauma surgeons are well integrated within University of Cincinnati Medical Center and that they are expo

Emergency thoracotomies in traumatic cardiac arrests following blunt trauma - experiences from a German level I trauma center.

PURPOSE: Resuscitative thoracotomies (RT) are the last resort to reduce mortality in patients suffering severe trauma. In recent years, indications for RT have been extended from penetrating to blunt trauma. However, discussions on efficacy are still ongoing, as data on this rarely performed procedure are often scarce. Therefore, this study analyzed RT approaches, intraoperative findings, and clinical outcome measures following RT in patients with cardiac arrest following blunt trauma. METHODS: All patients admitted to our level I trauma center's emergency room (ER) who underwent RT between 2

UK-wide major trauma center tertiary trauma survey pro forma review and aggregation and consolidation into a redesigned document.

OBJECTIVES: The trauma tertiary survey (TTS) is an essential part of the continued care for major trauma patients which is performed to ensure that all injuries have been identified and none have been overlooked during the patient's stay. Although the Advanced Trauma Life Support Course states a need for a tertiary survey, there is currently no standard for what this survey comprises. METHODS: Using local consultant expert opinion and a literature search we identified a set of 32 TTS potential features that may be included within a TTS pro forma. Major trauma center (MTC) documents were reque
