Tuberculous Bronchopleural Fistula: A Rare and Life-Threatening Disease.

Tuberculous bronchopleural fistula (BPF) is a rare and potentially life-threatening complication of pulmonary tuberculosis, in which abnormal connections form between the bronchial tree and the pleural space. These abnormal connections allow air and secretions to pass from the lungs into the pleural space, causing a range of symptoms from benign cough to acute tension pneumothorax. The management of tuberculous BPF requires an individualized approach based on the patient's condition and response to treatment. Anti-tuberculosis therapy is essential for controlling the active tuberculosis infect

[Tuberculous lesions of the pleura].

Tuberculous involvement of the pleura became quite incident of late. Eighty-seven patients with this condition were hospitalized at the phthisiosurgery department of the Kostroma District Antituberculosis Center in 1985-1992. Tuberculous pleurisy was diagnosed in 30 patients, in 29 pleurisy was a complication of pulmonary tuberculosis (in 13 cases of infiltrative tuberculosis, in 12 of focal, in 3 of disseminated, and in 1 of cirrhotic pulmonary tuberculosis), in 8 patients acute tuberculous pleural empyema was diagnosed which developed in the presence of infiltrative and cirrhotic pulmonary t

Spirometric changes after pleural decortication in young adults.

BACKGROUND: The collection of pleural fluid and thickened pleura restrict the movement and expansion of lung. The main treatment strategy is lung decortication for the thickened pleura. The aim of this study was to investigate lung functions before and after pleural decortication in young adults. METHODS: A total of 63 patients with thickened pleura were retrospectively evaluated. Before the operation, patients with tuberculosis (n = 36) were treated with anti-tuberculosis therapy for 3-6 months. Patients with non-tuberculosis causes (n = 27) had been treated with broad-spectrum antibiotics.

Potential Diagnostic Properties of Chest Ultrasound in Thoracic Tuberculosis-A Systematic Review.

Background: Chest ultrasound (CUS) has been shown to be a sensitive and specific imaging modality for pneumothorax, pneumonia, and pleural effusions. However, the role of chest ultrasound in the diagnosis of thoracic tuberculosis (TB) is uncertain. We performed a systematic search in the medical literature to better define the potential role and value of chest ultrasound in diagnosing thoracic tuberculosis. Aim: To describe existing literature with regard to the diagnostic value of chest ultrasound in thoracic tuberculosis. Methods: MEDLINE, EMBASE, and Scopus databases were searched for relev

Medical thoracoscopy: a useful diagnostic tool for undiagnosed pleural effusion.

OBJECTIVE: We aimed to assess the role of medical thoracoscopy in patients with undiagnosed pleural effusion. METHODS: Patiens presenting with pleural effusion underwent three pleural aspirations. Patients in whom pleural fluid analysis was inconclusive underwent closed pleural biopsy for diagnostic confirmation. Patients in whom closed pleural biopsy was incolcusive underwent medical thoracoscopy using a rigid thoracoscope with a viewing angle of zero degrees was done under local anaesthesia and sedation with the patient lying in lateral decubitus position with the affected side up. Xanthogranulomatous pleuritis - An unusual presentation of tuberculosis.

In pulmonary practice, pleural effusion is a commonly encountered entity and has various etiologies. Pleural effusions in postpartum women can be an incidental self-limiting finding. The presence of systemic or respiratory symptoms, however, calls for prompt etiological workup and targeted therapy. Tuberculous pleuritis and lupus-related pleural disease are well known to flare up in the postpartum period. We describe the case of a young healthcare worker with no previous comorbidities who presented with fever, breathlessness, and chest pain 2 weeks after an uneventful confinement. Chest radiog

Is 18F-FDG PET/CT capable of differential diagnosis from tuberculous pleurisy from malignant mesothelioma?

AIM: 18F-Fluorodeoxyglucose (FDG) PET/computerized tomography (CT) is a valuable method in the diagnosis of malignant pleural mesothelioma (MPM). But, some infections, particularly tuberculosis, are known to mimic cancer. We aimed to compare the FDG PET/CT findings of tuberculosis pleurisy (TP) and malignant mesothelioma and evaluate its role of differential diagnosis. MATERIAL AND METHOD: We retrospectively reviewed the data from 85 patients (45 patients with MPM and 40 patients with TP) who underwent FDG PET/CT. All images were reevaluated and pleural thickening, maximum standardized uptake

Thoracoscopic crush biopsy unmasking drug resistant extrapulmonary tuberculosis.

Tuberculosis remains a major global health concern with extrapulmonary manifestations contributing to approximately 15-20% of cases in immune-competent hosts. In 2022, National Tuberculosis Elimination Program (NTEP) reported multidrug resistance in 2.5% of new tuberculosis cases and 13% in previously treated cases. Among extrapulmonary tuberculosis pleural tuberculosis is one of the most common forms. It becomes a diagnostic challenge when microbiological tests are inconclusive. In most developing countries, antitubercular drugs are initiated empirically in patients on the basis of clinical d

Pleural Toxocariasis Presenting as Eosinophilic Pleural Effusion: A Case Report.

Eosinophilic pleural effusion (EPE) is a rare pleural disorder characterized by a significant proportion of eosinophils in the pleural fluid. Its etiologies are diverse and include malignancy, infections, pulmonary embolism, drug-induced reactions, and parasitic diseases. Pleural involvement in toxocariasis is rare and may pose diagnostic challenges, particularly in tuberculosis-endemic regions. We report the case of a 67-year-old male farmer from a rural area who presented with progressive dyspnea, right-sided chest pain, fever, and general deterioration. Physical examination revealed signs c

[Results of combined blind pleural biopsy and cytology in the differential diagnosis of pleural effusions].

In 80 patients with pleural effusion of uncleared aetiology cytological, bacteriological and chemical examinations of punctate were combined with blind pleural biopsy. Malignant pleural diseases were verified via pleural biopsy in 48% and in combination with pleural biopsy and cytology in 93% of the cases. About one third of the tuberculosis pleural effusions could be diagnosed histologically. Blind pleural biopsy can be recommended as a method of low risk in differential diagnosis of pleural effusions.

[Extrapulmonary tuberculosis].

Although the respiratory tract is the major site for tuberculous lesions, all organs can be affected. The authors review the clinical files of 25 patients admitted to our Department with lung and/or pleural tuberculous disease, confirmed by bacteriology or histopathology, who simultaneously suffered from tuberculosis in other organs. We included 20 men and 5 women, aged from 10 to 74 years (mean: 37.12 years); 6 patients had pleural tuberculosis, 19 had pulmonary tuberculosis, 4 of them having miliary tuberculosis. Lymph node tuberculosis was diagnosed in 14 patients, tuberculosis of the bones

Histologic aspects in pleural diseases revealed by needle biopsy.

In 59 patients, 18 to 82 years old, with pleural exudates, a number of 78 needle biopsies were performed in view of establishing the etiologic diagnosis. The diagnosis was: pleural tuberculosis in 9 cases, acute or chronic non-tuberculous inflammation in 8 cases, postinfectious sequelae in 6 cases, secondary malignant neoplasia in 10 cases; 2 mesotheliomas, and in 24 cases the aspect was normal or the minimal alterations found were not conclusive. The proportion of confirmations was of 81% for tuberculosis and 37% for cancer. The method is recommended as being of particular value in establishi

Tuberculosis and renal transplantation--observations from an endemic area of tuberculosis.

Ninety-five renal transplant recipients from an endemic area of tuberculosis were investigated to find out the prevalence and course of tuberculosis in pre- and post-transplant periods. Eleven patients had tuberculosis in the pre-transplant period - pulmonary (2), pleural (2), miliary (1), abdominal (2), lymph node (5) and pericardial (1). They were transplanted after antituberculous therapy of 3 to 6 months with satisfactory results. The anti-tuberculous treatment was usually continued for 2 years. Only one of the above 11 patients had evidence of tuberculosis in the post-transplant period. N

Competence in pleural procedures.

Diseases of the pleura and pleural space are common and present a significant contribution to the workload of respiratory physicians, with most cases resulting from congestive heart failure, pneumonia, and cancer. Although the radiographic and ultrasonographic detection of pleural abnormalities may be obvious, the determination of a specific diagnosis can often represent a challenge. Invasive procedures such as pleural drainage, ultrasound/CT-guided pleural biopsy or medical thoracoscopy can be useful in determining specific diagnosis of pleural diseases. Management of primary and secondary sp

T-lymphocyte subsets in pleural fluids: discrimination according to traditional and monoclonal antibody-defined markers.

T-lymphocyte subpopulations in pleural fluid and in peripheral blood from 17 patients admitted for pleural effusion were identified by E-rosette formation and delineated by monoclonal antibodies OKT3 (peripheral T-cells), OKT4 (helper/inducer cells), and OKT8 (suppressor/cytotoxic cells). We studied 13 patients with specified pleural diseases (tuberculosis, malignancies, connective tissue diseases, congestive heart failure) and 4 patients with non-specified pleural diseases. Our findings showed that the percentage of T-cells increases in pleural fluid versus peripheral blood whatever the diagn

Altered inflammatory responses following transforming growth factor-beta neutralization in experimental guinea pig tuberculous pleurisy.

The predominant extrapulmonary form of tuberculosis, which develops in 10% of diseased individuals, is pleurisy. The immune response mounted against Mycobacterium tuberculosis in the pleural cavity is one that is sufficient for clearing the organism without therapeutic intervention. Thus, examining the role of immune constituents in this context will provide understanding of the vital role they play in controlling tuberculosis. In this study, experimental tuberculous pleurisy was induced in guinea pigs, and anti-TGF-beta was administered intrapleurally to the guinea pigs daily throughout the s

AN EPIDEMIC OF PLEURISY AMONGST MILITARY RECRUITS.

An epidemic amongst recruits who presented with acute viral exudative pleural effusion with lymphocytic pleocytosis is analysed. Histologic and bacteriologic proof of tuberculosis was lacking in majority. Most of them recovered without pleural thickening. Overcrowding, inadequate clothing protection, stress and strain of vigorous recruit training could be important precipitating factors. None reported with parenchymal tuberculosis in two year follow up.
