Comparison of serum neopterin levels in pulmonary tuberculosis patients with positive acid fast bacilli and inactive pulmonary tuberculosis.

BACKGROUND: Tuberculosis (TB) is caused by infection with Mycobacterium tuberculosis. Sometimes it is difficult to distinguish between active and inactive pulmonary TB based on chest X-ray images. TB biomarkers are needed to diagnose active TB when TB bacilli are not detected either through microscopic examination, culture or geneXpert MTB/Rif. Neopterin can be used as an additional diagnosis and therapy monitoring as well as assessing activation of macrophage cells. This study aims to examine and comparing serum neopterin levels in new cases of pulmonary TB with positive AFB and inactive pulm

Surgical intervention in bronchiectasis and pulmonary tuberculosis.

Bronchiectasis and pulmonary tuberculosis (TB) remain common causes of chronic respiratory morbidity worldwide, despite major advances in antimicrobial therapy and structured airway clearance strategies. In a subset of patients, medical treatment alone is insufficient, and surgical intervention may be required when disease is anatomically localised or when serious complications develop. From a surgical perspective, resection of non-functional, chronically infected lung segments in bronchiectasis can lead to sustained symptom improvement, fewer exacerbations, and effective control of haemoptysi

Pulmonary tuberculosis presenting as a hilar mass.

Tuberculosis (TB) is a great mimicker due to its various unusual and atypical presentations. Mass-like lesions in thoracic radiology may raise the suspicion of lung malignancy. A man in his early 50s complained of cough, low-grade fever and dyspnoea. Chest radiograph was suggestive of a right hilar mass. Contrast-enhanced CT of the thorax revealed right perihilar consolidation and mediastinal lymphadenopathy. The lesion was encasing the major vessels, causing luminal narrowing. Flexible bronchoscopy showed multiple mucosal nodules in the right lobar bronchi. Histopathology of the endobronchial

[Update: Diagnostics and treatment of pulmonary tuberculosis].

Molecular diagnostic tools have changed the approach to the detection of Mycobacterium tuberculosis and associated drug-resistance substantially. PCR-based technologies allow a more rapid detection with higher diagnostic sensitivity in pulmonary and extrapulmonary specimens. However, a real point of care test, which needs minimal technical resources remains missing. Genome sequencing technologies are currently changing tuberculosis drug resistance testing, and for some questions are replacing phenotypic drug resistance testing, based on culture.New evidence on treatment for drug-sensitive tube

Co-existence of pulmonary tuberculosis with sarcoidosis.

Tuberculosis (TB) and sarcoidosis are multisystem, chronic granulomatous diseases. Although characterized by similar clinical manifestations, these disease entities vary significantly in etiologies and management. Sarcoidosis is an immunological disorder of unknown etiology, characterized by the presence of noncaseating granulomas in the tissues involved. TB is the infectious disease caused by Mycobacterium tuberculosis, characterized by granulomas with caseous necrosis. It is rare to have both the diseases concomitantly. We present the case of a 38-year-old male with microbiological confirmat

Case of melioidosis misdiagnosed as pulmonary tuberculosis.

Melioidosis is an endemic infection caused by Burkholderia pseudomallei predominantly reported in the coastal parts of India. A 19-year-old male student with no comorbidities presented with features suggestive of pneumonia. He was initiated on antitubercular treatment empirically elsewhere. However, due to lack of response to therapy diagnosis was revisited. Microbiological investigations were unyielding initially. Despite antitubercular treatment, he presented with complications of pneumonia and was diagnosed to have melioidosis. He was initiated on appropriate antibiotics for the intensive a

[Coinfection with hydatid cyst and pulmonary tuberculosis].

Morocco is an endemic area for both hydatid cyst and tuberculosis (TB). The co-occurrence of these two diseases in lung is unusual, especially when localized in the same area. We describe the case of a patient with pulmonary hydatid cyst associated with tuberculosis infection where the diagnosis was unknown prior to surgery because TB tests were negative. The surgical resection was very difficult because of inflammation in the surrounding tissue and its fragility. However, the postoperative course was uneventful. The goal of this case report is to keep in mind that such co-infection is possibl

[Diagnosis delay of pleural and pulmonary tuberculosis].

Tuberculosis (TB) is still being endemic in our country. Time until management determines both evolution and prognosis of this condition. The aim of this work is to evaluate the delay in diagnosis of TB in a respiratory unit from a university hospital series. The authors conducted a cross-sectional study including patients with pulmonary TBC and/or pleural. An evaluation of time management was conducted from the beginning of symptoms and various consultations with reference to the date of hospitalization and treatment set up. One hundred patients were included (pulmonary TB: 68 cases, pleural

Pulmonary tuberculosis outbreak in a pediatric population.

Community-based outbreaks of Mycobacterium tuberculosis are uncommon in the United States but represent a dramatic type of epidemic that can lead to considerable investigations. Most of our knowledge regarding spread of tuberculosis (TB) has accumulated from the study of outbreaks. We describe the most recent outbreak of TB in Genesee County, Michigan. In February 2007, isoniazid-sensitive infectious pulmonary TB was identified in a 45-year-old African American grandmother who frequently provided care for her grandchildren and other children. The source case was reported to the Genesee County

Pulmonary tuberculoma and miliary tuberculosis in silicosis.

Tuberculosis is a disease with protean manifestations. We present a case which was initially suspected as bronchogenic carcinoma with lymphangitic carcinomatosis, based on radiological appearance but later diagnosed as pulmonary tuberculoma with military tuberculosis and silicosis after thoracotomy and open lung biopsy. The patient was treated successfully with Antituberculosis Therapy (ATT). Rarity of presentation in form of pulmonary tuberculoma co-existing with histological features of miliary tuberculosis and silicosis, led us to report this case.

[Sparfloxacin in combined therapy for pulmonary tuberculosis].

Efficacy and safety of the difluoroquinolone sparfloxacine used in combined therapy for drug-succeptible and drug-resistant pulmonary tuberculosis were studied. Clinical trials enrolled 60 patients with severe pulmonary tuberculosis. The most effective combinations of sparfloxacine and other antituberculous drugs were determined. Data on satisfactory tolerability of sparfloxacine used in various combinations of the main and reserve agents are presented.

Nasopharyngeal aspiration for diagnosis of pulmonary tuberculosis.

BACKGROUND: Confirmation of pulmonary tuberculosis (PTB) in young children is difficult as they seldom expectorate sputum. AIM: To compare sputa obtained by nasopharyngeal aspiration and by sputum induction for staining and culture of Mycobacterium tuberculosis. PATIENTS AND METHODS: Patients from Mulago Hospital, Kampala with symptoms suggestive of PTB were considered for inclusion in the study. Those with a positive tuberculin test and/or a chest radiograph compatible with tuberculosis were recruited. Infection with human immunodeficiency virus (HIV) was confirmed by duplicate enzyme-label

Factors changing the manifestation of pulmonary tuberculosis.

OBJECTIVE: To investigate factors altering the manifestation and outcome of pulmonary tuberculosis (PTB). DESIGN: The medical records of culture-proven TB patients from July 2001 through December 2002 were reviewed. RESULTS: A total of 461 patients were identified. Diabetes (21.5%) and malignancy (15.2%) were the most common underlying comorbidities. Sixteen cancer patients were initially misdiagnosed as pulmonary metastasis or recurrence. Younger patients frequently had constitutional symptoms and haemoptysis, while older patients frequently had respiratory symptoms and pleural effusion. Ma

Pulmonary pseudallescheriasis in a patient with healed tuberculosis.

Pulmonary pseudallescheriasis in an immunocompetent patient without a pre-existing cavity or cyst is a rare phenomenon. We report a case of invasive pulmonary pseudallescheriasis in a lobectomised patient treated for tuberculosis. Filamentous fungi with pyriform conidia were seen in the bronchoalveolar lavage fluid . The fungus was identified as Pseudallescheria boydii on culture.

Pulmonary tuberculosis in children with Hodgkin's lymphoma.

The clinical presentation of tuberculosis (TB) and Hodgkin's lymphoma (HL) with pulmonary involvement is similar and raises problems of differential diagnosis. It may also be difficult to distinguish TB from relapsed lymphoma. The purpose of this study was to evaluate the association of HL and pulmonary TB and to discuss differential diagnosis. Medical records of 70 children were reviewed retrospectively. A total of 27 patients (38%) had mediastinal-pulmonary involvement initially. Systemic symptoms were present in 37 (52%) patients. In all, 14 patients (20%) had pulmonary TB; three of them we
