The most widely accepted mechanism of male urethral development proposes that the urethral plate is elevated by urethral folds which fuse ventrally in a proximal-to-distal sequence. Unlike its proximal counterpart, the urethra which forms within the glans is lined by a stratified squamous epithelium and has a more controversial development. One theory supports the idea that fusion of the urethral folds extends all the way to the tip of the glans. Primary localized amyloidosis of the penile urethra is rare. We report a case in which the urethra was obstructed by amyloid tissue without evidence of systemic amyloidosis. After urethral dilatation, the patient reported a markedly improved urinary stream with minimal dysuria.

We report a case of leiomyoma of the female urethra and review the literature. A mass protruding from the urethral meatus during voiding caused dysuria. Urodynamic study showed an obstructive pattern with low maximum urine flow and high detrusor pressure in initiating voiding. Cystourethroscopy revealed a mass extending from the proximal segment of the left lateral urethral wall into the bladder. At transurethral resection of the tumor 12.2 gm. of tissue were excised. Histopathological studies confirmed urethral leiomyoma. Surgery completely resolved the original symptoms of dysuria.

We present a rare case of inverted papilloma of the prostatic ureter whose presenting features were gross hematuria and variable dysuria. Endoscopy revealed a polypoid appearance with a fine pedicle arising from the prostatic urethra. Treatment was by transurethral resection of the neoformation. The diagnosis was made on the histopathological findings. The different histopathogenic considerations published in the literature are discussed.

A total of 310 consecutive sexually active men attending sexually transmitted disease clinic of the Nehru Hospital, Chandigarh, were studied to assess the prevalence of G vaginalis. Non-gonococcal urethritis (NGU) was diagnosed in 53, acute gonococcal urethritis (AGU) in 20, psychosomatic disorders in 50 and genital ulcer disease in 187 patients. A case of condyloma acuminatum in female urethra is reported. A 53-year-old woman was referred to our clinic because of urethral bleeding. A tumor was noted around the external urethral meatus and the size of the tumor was 20 mm in diameter. Excision was performed on the tumor including the external urethral meatus. Pathological examination revealed condyloma acuminatum. Condyloma acuminatum in female urethra is very rare. We reviewed and discussed 6 cases of condyloma acuminatum in female urethra, including our case, in the Japanese literature.

We described 4 cases of polyps of the prostatic urethra. Three of them had adenomatous polyp with prostatic epithelium and 1 showed polypoid urethritis. These lesions were worth noting as a cause of hematuria, dysuria or hematospermia. Polyps or papillary lesions of the prostatic urethra appear to be more common than have been suspected. Careful attention should be paid to the prostatic urethra in patients with persistent painless hematuria.

A prospective study was done to evaluate the long-term effects of radical prostatectomy on the function of the bladder in filling and voiding. Preoperative urodynamic studies were done on 29 patients with a mean age of 62.9 +/- 5.2 years. The preoperative results show that 16 of the 29 patients demonstrated detrusor instability with maximum contractile pressures of 59 +/- 28 cm. water. Followup urodynamic assessment was done in 13 of these patients 22.9 +/- 1.1 months after surgery. Postoperatively, the maximum detrusor instability pressure did not decrease significantly (49 +/- 17 cm. water).

A rare case of congenital diverticulum of the urethra of a 14-year-old boy associated with numerous calculi is reported. The clinical symptoms were pain during micturition and dysuria. Physical examination was complemented by instrumental, of which most important for the diagnosis was retrograde urethrography and urethrocystoscopy. A case of urethral condyloma acuminatum is reported. A 77-year-old man was admitted to Konan Hospital to undergo surgery for benign prostatic hyperplasia. By urethrography and urethroscopy, a circumferential papillary lesion was disclosed in the posterior urethra extending from the membraneous urethra. Transurethral resection was performed on July 19, 1989. Pathohistological diagnosis was condyloma acuminatum/condyloma acuminatum in which the antigen of papilloma virus was detected by PAP stain.

Complete rupture of the female urethra is rare. We found only 30 such cases described in the literature, 19 of which were in children. In most of these patients, the proximal urethra and bladder neck were injured. Herein we report on the complete rupture of the distal part of the urethra in a female child with a pelvic fracture. This diagnosis should be considered in such cases with deep vaginal lacerations, inability to void, and with unsuccessful catheterization.

The urogenital tract has been examined in 35 patients who required 3 or more years of therapy. A range of studies revealed a short urethral stricture in 11, long urethral stricture in 26, megalocystis in 6, domicile urinary bladder in 2 and bladder diverticulosis in 6 patients. Delayed direct-vision internal urethrotomy is a reasonable first treatment of the traumatically obliterated posterior urethra. Postoperative continence is achieved if the bladder neck is competent on preoperative cystogram. Simultaneous suprapubic and transurethral instrumentation most accurately define the length of obliterated urethra, and rectal examination determines any prostatic displacement. Half the patients will require repeat endoscopic surgery for significant stricture, and all require long-term follow-up to assure urethral patency. Potency is not affected by the procedure. Urethrot
