Nocturnal enuresis, daytime urinary incontinence, lower urinary tract symptoms and faecal incontinence are more common in children with special needs than in typically developing children. Children with intellectual disability, which can be attributed to a range of causes, are particularly affected. Indeed, the epidemiological and clinical studies conducted to date show clear associations that children with special needs have higher rates of urinary (and faecal) incontinence than children without development, physical or cognitive impairments. For example, low intelligence quotient (IQ)-associ…

Most cases of urinary incontinence in women fall under one of three major subtypes: urge, stress, or mixed. A stepped-care approach that advances from least invasive (behavioral modification) to more invasive (surgery) interventions is recommended. Bladder retraining and pelvic floor muscle exercises are first-line treatments for persons without cognitive impairment who present with urge incontinence. Neuromodulation devices, such as posterior tibial nerve stimulators, are an option for urge incontinence that does not respond to behavioral therapy. Pharmacologic therapy with anticholinergic me…

AIMS: To evaluate cognitive processing in elderly women with urge incontinence as compared to continent cohorts by measuring performance on neuropsychological tests of working memory, attention and inhibition. To measure speed of information processing, intensity of bladder filling sensation, and urine volume at a first desire (FDV) and strong desire to void (SDV), and level of attentional demands.
METHODS: Test measures included Operation and Reading Span, Wisconsin Card-Sorting, Stroop Color Reading, Map Planning, and Maze Speed tests. A Likert scale measured bladder filling sensation inten

Urethral closure mechanisms under stress conditions consist of passive urethral closure involving connective tissues, fascia and/or ligament … ch as laughing, exercise or lifting heavy objects. There are over 200 million people worldwide with urinary incontinence, a condition that is associated with a significant social impact and reduced quality of life. Therefore, basic research for urinary continence mechanisms in response to different stress conditions can play an essential role in developing treatments …

BACKGROUND: ncontinence can have a devastating effect on the lives of sufferers with significant economic implications. Non-surgical treatments such as pelvic floor muscle training and the use of mechanical devices are usually the first line of management, particularly when a woman does not want surgery or when she is considered unfit for surgery. Mechanical devices are inexpensive and do not compromise future surgical treatment.
OBJECTIVES: To determine the effects of mechanical devices in the management of adult female urinary incontinence.
SEARCH STRATEGY: We searched the Cochrane Inconti…

Overall prevalence of urinary incontinence in men is 17%, rising to 31% in those men aged 85 years old or older (Anger et al, 2006). Penile sheaths, sometimes referred to as 'condom catheters', provide a reliable form of management for those men with intractable urinary incontinence. A penile sheath is a soft, flexible sleeve that fits over the penis with an anti-reflux bulbous end which culminates as a short tube that attaches to any standard urinary drainage system. Penile sheaths provide clinicians with a useful alternative to long-term urethral catheterization as they are non-invasive and …

AIM: To determine the characteristics of urinary incontinence and its impact on the quality of life in adult women with urinary incontinence who presented to a tertiary care clinic of Vojvodina from September 2008 to May 2009 for treatment
METHODS: We used a prospective case-control study. Cases were defined as patients (47) with urinary incontinence symptoms. Controls (50) were defined as patients without urinary incontinence who presented to a tertiary care gynecology clinic for other reasons. Both, cases and controls, completed two questionnaires recommended for the evaluation of symptoms,…

Urinary incontinence is a common symptom that can affect women of all ages. It has been estimated that there are more than 3.5 million sufferers in the UK alone. History taking guides the investigation and management of patients by evaluating symptoms, their progression and the impact of symptoms on lifestyle. The onset of urinary symptoms, their duration and their severity should be recorded. The predominant bother symptom, e.g., urgency, urge incontinence or stress incontinence, should be identified. The clinician should also enquire about colorectal symptoms and genitourinary prolapse. Acco…

Stress urinary incontinence is one of the most prevalent and costly problems encountered by the international medical community. The exact mechanism of stress incontinence remains elusive. Early management relied on behavioral modification but, as more advanced anatomic and urodynamic research surfaced, the focus shifted to surgical correction. Initial innovations provided a compressive force/hammock to support the urethra and bladder neck. For almost a century, the pubovaginal sling provided this support, with 70-90% cure rates at the expense of significant voiding dysfunction. Later work has…

BACKGROUND: Urinary incontinence is a common problem in dementia. Almost invariably, the person with dementia will develop incontinence as the disease progresses. However, the primary reasons for incontinence are often not because of any significant pathology in the urinary system. Rather, it is due to factors outside the urinary system. The term 'functional incontinence' has hence been applied. Maintenance of continence requires mobility, manual dexterity, mental capacit and motivation. Clearly, the person with dementia is vulnerable to developing problems in these domains.
OBJECTIVE: This a…

BACKGROUND: Laparoscopic colposuspension was one of the first minimal access operations for the treatment of women with stress urinary incontinence, with the presumed advantages over traditional Burch colposuspension of avoiding major incisions, shorter hospital stay, and quicker return to normal activities. A variety of approaches and methods are used.
OBJECTIVES: To determine the effects of laparoscopic colposuspension for urinary incontinence.
SEARCH STRATEGY: We searched the Cochrane Incontinence Group Specialised Trials Register (searched 21 September 2005). Additional trials were sough…

Urinary incontinence is a common and distressing condition. The two main types of incontinence in the developed world are urodynamic stress incontinence and detrusor overactivity. Recent advances have focussed on the development of a drug for stress incontinence and on the production of newer more M3 specific anticholinergics. Duloxetine, a relatively balanced and potent serotonin noradrenaline reuptake inhibitor, is the first drug to be licensed for stress incontinence. Until recently, the pharmacological treatment options for stress urinary incontinence (SUI) have been limited to the off-the…

Urinary and faecal incontinence are a growing problem in the Dutch elderly population. Recent figures from general practice suggest an average prevalence in the over-60-year-olds of 19 and 6%, respectively. Due to an accurate system of continuous monitoring and a very high response on a non-validated but clearly-worded and short questionnaire, the reliability of these figures is most probably high. The research article is one in a series on incontinence from the same research group that studied the practical implications of urinary incontinence in the elderly. Topics published so far concerned…

It is estimated that 10 million women in the UK suffer with urinary incontinence (Hunskarr et al, 2004). Many women consider bladder weakness to be an inevitable consequence of childbirth or the menopause. Haslam (2004) reported on a telephone survey that examined whether women were aware of stress urinary incontinence, their attitude to it and the impact it had on their daily lives. In the UK, 505 women were interviewed, 206 of whom responded positively to a question about symptoms of stress urinary incontinence, giving a prevalence rate of 41 per cent. There is also evidence that most patien…

The increasing prevalence and incidence of male urinary incontinence in senium-necessitate a specific as well as symptom and etiologically-oriented therapy. In addition to the conservative care with absorbent products, catheterization methods are available. Moreover, the problem can be successfully controlled in the majority of the patients through very specific drug and surgical therapeutic concepts.

The aim of this study is to verify the prevalence of urinary incontinence in hospitalized patients and identify the statistical associations between the figures gathered and some of the patients' demographic and clinical variables. The data were obtained with 77 inpatients of three different units from the University of São Paulo's Hospital Universitário. The results showed a total prevalence of 35% and partial rates of 48%, 37% and 22% in the Surgical, Obstetric and Clinical Units respectively. Significant statistical correlation were observed between the prevalence and dysuria (r = 0.19 and …

To date limited data exists that addresses the impact of urinary incontinence on sexual function. In the present study, we assessed sexual functions in patients with urinary incontinence and compared with healthy continent subjects by means of Female Sexual Function Index (FSFI) questionnaire. A total of 21 premenopausal incontinent women (three stress incontinence, nine overactive bladder and nine mixed incontinence) were enrolled in the study, and 18 healthy continent subjects served as controls. All subjects were asked to complete FSFI questionnaire and each FSFI domain scores including des…
