From 1967 to 1980, 65 patients underwent vagotomy and pyloroplasty for treatment of an acute perforated duodenal ulcer at the University of Oklahoma Health Sciences Center. Their age ranged from 15 to 82 years, with a mean age of 49.6 years. All vagotomies were truncal and pyloroplasties were of the Heineke-Mikulicz variety. The condition of each patient was classified as acute or chronic depending on the duration of symptoms before perforation. Vagotomy and gastric surgery have been implicated in gallstone formation, although the association remained unproven. Gallbladder function was investigated in 11 patients with a pyloroplasty and truncal vagotomy, 5 with a subtotal gastrectomy, and 16 healthy controls. The exocrine pancreatic and biliary secretion in response to vagal stimulation by insulin hypoglycemia was measured in preoperative patients with duodenal ulcer and in patients who underwent highly selective vagotomy, bilateral selective vagotomy with pyloroplasty, and truncal vagotomy with pyloroplasty. Significant stimulation of both biliary and pancreatic secretion occurred only in patients with an intact vagal nerve supply. No evidence of increased lithogenicity of human bile was found up to 2 years after truncal vagotomy.

Sixty-eight patients antrectomized for recurrent ulcer after different types of vagotomy were reexamined two to ten years after the operation. We report the use of a new surgical procedure following pancreaticoduodenectomy in nine patients with chronic relapsing pancreatitis and its complications. The modification of the Whipple reconstruction following resection of the head of the pancreas consists in selective proximal vagotomy, transposition of a jejunal loop for bile duct, and pancreatic anastomosis.
