Absent superior vena cava in tetralogy of fallot.

Absent superior vena cava (SVC) is an asymptomatic congenital systemic venous anomaly which is rarely detected and compatible with normal life. Undiagnosed absent SVC may cause problems during cardiac catheterization or cardiac surgery. We present our surgical experience in a patient with tetralogy of Fallot who had undiagnosed absent SVC.

Case 257: Leiomyosarcoma of the Inferior Vena Cava.

History A 65-year-old woman presented to her primary care physician with a history of progressive abdominal pain mainly in the upper quadrants, nausea, and edema in the bilateral symmetric lower extremities. Other symptoms and use of medication or related drugs were denied. Physical examination findings were normal. The serum lactate dehydrogenase level was 302 U/L (5.04 μkat/L) (reference range, <247 U/L [4.12 μkat/L]), and all other laboratory data were within normal ranges. Electrocardiography and chest radiography revealed no abnormalities. The patient underwent contrast material-enhanced

A STUDY OF SUPERIOR VENA CAVA OBSTRUCTION SYNDROME.

Superior vena cava obstruction syndrome was studied in 22 patients with a view to assess the etiological pattern and clinical profile. It was found that 4 patients (18.1%) had benign disease and the remaining 18 (81.9%) patients had malignancy. In 13 patients (59.9%) obstruction occurred due to bronchogenic carinoma. No complications occurred with invasive diagnostic procedures. All heavy smokers (59.9%) were found to have malignancy. Normal lung fields on chest radiography and CT scan with superior vena obstruction syndrome suggested benign disease in 2 cases.

Lipoma of superior vena cava: a rare occurrence.

Most benign primary cardiac tumours are myxomas; non-myxomatous tumours are less common but comprise a wide variety. Cardiac lipoma is a rare non-myxomatous variety. A 70-year-old Caucasian woman with right breast cancer status postpartial mastectomy underwent surveillance MRI of the breast and was found to have a possible right atrial (RA) mass. She also reported frequent headaches and palpitations. She underwent a transoesophageal echocardiogram which showed a 2.6×1.6×1.6 cm echogenic mass at the superior vena cava (SVC) and RA junction. She was anticoagulated for a possible thrombus without

Expectoration of an inferior vena cava filter strut.

Inferior vena cava (IVC) filter use is widespread in patients with venous thromboembolism (VTE) and temporary contraindication to anticoagulation, though timely removal is often not performed. We report the case of an expectoration of an IVC filter strut. Review of the patient's prior imaging confirmed an infrarenal Bard G2 filter with an absent strut, which was visualised in the left lung base. The strut was presumed to have embolised to a pulmonary artery branch and eroded into an adjacent bronchus. Subsequent fluoroscopically guided filter retrieval was successful. The incidence of IVC filt

Inferior Vena Cava Stent-Graft Sepsis.

We present the case of a patient with a complicated medical history, which included stent grafting as a life-saving measure for an iatrogenic inferior vena cava (IVC) injury. For persistent sepsis secondary to stent-graft infection, the patient underwent extraction of 2 IVC stent grafts, primary repair of a duodenal-caval fistula, and repair of the IVC with an allograft vein patch. Discussion of this case sought to shed light on the intricacies involved in medical decision making in an era of advanced medical technology.

Inferior vena cava filters: indications and management.

PURPOSE OF REVIEW: The purpose of this review was to examine recent studies concerning the use of inferior vena cava (IVC) filters. RECENT FINDINGS: In the past 18 months, the American College of Chest Physicians released the 9th edition of their guideline for the prevention and treatment of venous thromboembolism. There have also been a number of studies reviewing the use of IVC filters in select populations for the prophylactic prevention of pulmonary embolism. Trauma continues to be the leading indication for prophylactic filters in a number of series, but further studies have demonstrated

A case report of superior vena cava obstruction.

We report herein an 83-year-old gentleman with lung cancer who presented with nausea, complete atrioventricular (AV) block and presyncope. Despite a present temporary pacemaker, which had been inserted through the femoral vein 5 days previously, the patient had asystole attacks that resolved with atropine administration. Coronary angiography demonstrated no critical stenosis. Sick sinus syndrome was diagnosed, and permanent pacemaker implantation was decided. However, the guidewire could not be advanced into the superior vena cava (SVC). Right jugular venogram showed complete obstruction of th

Retrievable vena cava filters: a clinical review.

Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism (PE), is a major cause of morbidity and mortality. Parenteral anticoagulant treatment with full-dose unfractioned heparin, low-molecular-weight-heparin, or fondaparinux, followed by oral treatment with the vitamin K antagonists, is recommended for the majority of patients. However, in the presence of contraindications to anticoagulant treatment, bleeding complications during antithrombotic treatment, or VTE recurrences despite optimal anticoagulation, interruption of the inferior vena cava with a filter is a po

Catheter-induced thrombosis of the superior vena cava.

There has been an increase in the use of central venous catheters (CVCs) in clinical practice. One of the most dangerous complications associated with their use is symptomatic or asymptomatic thrombosis (T), sometimes associated with superior vena cava (SVC) syndrome, resulting from impaired venous drainage. The right heart clots can induce an increased risk of mortality due the potential pulmonary embolism (PE). We report a case of asymptomatic 83-year-old woman in whom the thrombosis was detected after an echocardiogram. Echocardiography demonstrated a cardiac mass, and the T was confirmed b

Suprarenal inferior vena cava filter implantation.

PURPOSE: This study evaluated the feasibility, safety, and efficacy of the suprarenal implantation of a retrievable filter in patients with renal cell carcinoma (RCC) and renal vein thrombosis (RVT) [extending or not extending to the inferior vena cava (IVC)] undergoing surgery. MATERIALS AND METHODS: Between March 2005 and May 2010, 13 patients (eight men and five women; mean age 67.08 years, range 38-95) with RCC and RVT associated or not with IVC thrombosis underwent implantation of a retrievable suprarenal IVC filter. All patients underwent computed tomography angiography (CTA), which doc

Inferior vena cava filters for the cerebrovascular patient.

Acute cerebrovascular disease is often complicated by deep venous thrombosis and pulmonary embolism. Many of these patients are at high risk of intracranial hemorrhage with therapeutic anticoagulation. These patients may benefit from insertion of inferior vena cava filters. Studies specifically dealing with stroke patients are lacking, but it is the authors' opinion that filters reduce the incidence of pulmonary embolism. There is little evidence to support the use of these devices prophylactically in patients who do not have venous thromboembolism. Retrievable filters are an attractive option

Retrievable vena cava filters: a clinical review.

Vena cava filters were developed as a method of preventing pulmonary embolism (PE) in patients with venous thromboembolism (VTE) at risk for bleeding from therapeutic anticoagulation. However, the long-term complications of filter placement, such as caval thrombosis, have mitigated some of the benefits, particularly in those patients with only a temporary contraindication to anticoagulation. Retrievable filters were designed to avoid the long-term risks of a permanent filter while still providing short-term protection against PE. As a result, their use has expanded from patients with known thr

Incidence of vena cava thrombosis in the United States.

From 1979 through 2005, vena cava thrombosis (either superior or inferior) was diagnosed in 99,000 hospitalized patients. Most, 78%, had isolated vena cava thrombosis. From 2000 to 2005, 5,000 patients were diagnosed yearly with vena cava thrombosis (1.5% of patients hospitalized with deep venous thrombosis). The population-based incidence of diagnosis of vena cava thrombosis from 2001 to 2005 was 1.7 in 100,000. The incidence increased with age. It was rare in Asian Americans. Pulmonary embolism occurred in 12% of patients with isolated vena cava thrombosis. Cancer was frequently associated w
