Acquired right ventricular outflow tract obstruction in twin-to-twin transfusion syndrome; a prospective longitudinal study.

OBJECTIVE: The pathophysiology of right ventricular outflow tract obstruction (RVOTO) in twin-to-twin transfusion syndrome (TTTS) recipients is incompletely understood. We aimed to investigate the development and spectrum of RVOTO in TTTS recipients. METHODS: A prospective longitudinal cohort study was conducted between 2015 and 2017. Echocardiographic assessment was performed in recipients from TTTS diagnosis until the neonatal period. RESULTS: Prenatal RVOTO, defined as abnormal flow velocity waveforms across the pulmonary valve (PV), was diagnosed in 12.9% (16/124) of recipients at TTTS d

Hybrid stenting for left ventricular outflow tract obstruction in congenitally corrected transposition of the great arteries.

Congenitally corrected transposition of the great arteries is commonly associated with left ventricular outflow tract obstruction. We describe a case of congenitally corrected transposition of the great arteries and previous surgical ventricular septal defect repair with recurrent left ventricular outflow tract obstruction. The patient underwent a hybrid procedure to stent the left ventricular outflow tract, which was successful with no re-intervention through 3 years of follow-up.

Surgical treatment of right ventricular outflow tract obstruction after Tetralogy of Fallot {S,D,I} repair.

Situs solitus of viscera and atria, D-looped ventricle with atrioventricular concordance, and inverted normally related great arteries with ventriculoarterial concordance characterize the segmental anatomy of an extremely rare {S,D,I} variant of Tetralogy of Fallot. The main pulmonary artery is rightward of the aorta and the right coronary artery crosses over the right ventricular outflow tract from left to right to reach the right atrioventricular groove. Our patient had undergone a neonatal repair due to persistent cyanosis with the implantation of a pericardial infundibular patch after the

Effect of amiodarone-induced hyperthyroidism on left ventricular outflow obstruction after septal myectomy for hypertrophic cardiomyopathy.

Patients with obstructive hypertrophic cardiomyopathy who undergo septal myectomy are at risk for developing postoperative atrial fibrillation. Amiodarone is effective in treating this arrhythmia but is associated with multiple adverse effects, often with delayed onset. A novel case is described of a patient who developed type 2 amiodarone-induced hyperthyroidism that presented as recurrence of outflow obstruction after septal myectomy. The patient's symptoms and echocardiographic findings of outflow obstruction resolved substantially with the treatment of the amiodarone-induced hyperthyroidis

Surgical management of left ventricular outflow tract obstruction after biventricular repair of double outlet right ventricle.

Regardless of the preoperative morphology and the type of operation, left ventricular outflow tract obstruction (LVOTO) after biventricular repair of double outlet right ventricle (DORV) may develop. This report presents our 10-yr experience with surgical management of LVOTO after biventricular repair of DORV. Between 1996 and 2006, 15 patients underwent reoperation for subaortic stenosis after biventricular repair of DORV. The mean age at biventricular repair was 23.3+/-18.3 months (1.1-64.2). Biventricular repairs included tunnel constructions from the left ventricle to the aorta in 14 cases

Apical ballooning syndrome complicated by acute severe mitral regurgitation with left ventricular outflow obstruction--case report.

BACKGROUND: Apical ballooning syndrome (or Takotsubo cardiomyopathy) is a syndrome of transient left ventricular apical ballooning. Although … he had a mechanical mitral valve replacement with a 29 mm St Jude valve. Post-operatively, her left ventricular outflow obstruction resolved and ventricular function returned to normal over the subsequent 10 days. She recovered well. CONCLUSION: This case represents a serious and poorly understood association of Takotsubo cardiomyopathy with acute pulmonary oedema, severe mitra …

Inheritance analysis of congenital left ventricular outflow tract obstruction malformations: Segregation, multiplex relative risk, and heritability.

The left ventricular outflow tract (LVOTO) malformations, aortic valve stenosis (AVS), coarctation of the aorta (COA), and hypoplastic left heart (HLH) constitute a mechanistically defined subgroup of congenital heart defects that have substantial evidence for a genetic component. Evidence from echocardiography studies has shown that bicuspid aortic valve (BAV) is found frequently in relatives of children with LVOTO defects. However, formal inheritance analysis has not been performed. We ascertained 124 families by an index case with AVS, COA, or HLH. A total of 413 relatives were enrolled in

Dynamic left ventricular outflow tract obstruction: an unusual mechanism mimicking anterior myocardial infarction with cardiogenic shock.

Cardiogenic shock is a frequent and threatening complication in the course of acute myocardial infarction. Besides the well known causes (left ventricular failure, acquired interventricular defect, papillary muscle rupture, free wall rupture) other less frequent mechanisms recognize a functional substrate. The recognition of such mechanisms makes us to revert to the treatments with completely different prognostic implications. In our Coronary Care Unit we encountered, in a period of 12 months, 4 patients who presented clinical, electrocardiographic and/or echographic signs and symptoms of acut

[Ventricular outflow tract obstruction by cardiac tumors during ACTH therapy of an infant with tuberous sclerosis].

In an infant with tuberous sclerosis and West syndrome, ACTH treatment was interrupted because of augmentation of the biventricular outflow tract obstruction. A 5-month-old boy, who had been diagnosed to have multiple cardiac tumors since in utero, manifested West syndrome with typical hypsarhythmia in EEG. Several days after starting low dose ACTH treatment, a systolic murmur due to biventricular outflow tract obstruction intensified with ventricular arrhythmia. A tumor in the left ventricular outflow tract was enlarged. Discontinuation of daily administration of ACTH and administration of a

Homograft valve insertion for pulmonary regurgitation late after valveless repair of right ventricular outflow tract obstruction.

OBJECTIVE: Pulmonary regurgitation after valveless repair of right ventricular outflow tract obstruction (RVOTO) results in progressive right ventricular (RV) dilatation and dysfunction in an increasing number of patients. Since 1989, we have exclusively used cryopreserved homografts to restore pulmonary valve competence in these patients. Our 9-year-experience with pulmonary valve insertion (PVI) in such cases has been reviewed to evaluate the indications for this procedure and its benefits. METHODS: From 1989 to 1998, 49 patients (original diagnosis: tetralogy of Fallot in 42 patients and p

Dynamic left ventricular outflow tract obstruction after anterior myocardial infarction. A potential mechanism of myocardial rupture.

This study describes a dynamic left ventricular outflow tract gradient complicating an acute anterior myocardial infarction in two patients. In both cases, hyperdynamic contraction of the non-infarcted segments were noted in addition to large antero-apical a- or dyskinesia. Both patients had evidence of mitral annular calcifications, and the occurrence of dynamic outflow tract obstruction preceded sudden death. Early recognition of the underlying pathophysiological mechanism might have major therapeutic implications and could decrease the risk of myocardial rupture.

Reduction of the left-ventricular outflow-tract obstruction by aortoventriculoplasty--long-term results of 96 patients.

The aortoventriculoplasty (AVP) is an established surgical procedure for enlargement of different types of congenital and acquired obstruction of the left-ventricular outflow tract (LVOTO), especially with narrowing of the aortic valvular and subvalvular segment. Between 1974 and 1992, 96 patients underwent AVP. There were 32 female and 64 male patients, aged 3 to 32 years (mean 11.2 +/- 2.7 years) at operation. Indications for the procedure were as follows: 1. narrowing of the aortic annulus (19 patients), 2. multi-level stenosis (26 patients), 3. "outgrown"-prosthesis (8 patients), 4. recurr

Assessment of residual right ventricular outflow tract obstruction following surgery using the response to intravenous propranolol.

The response to intravenous administration of propranolol hydrochloride was studied in 24 children undergoing operation for isolated or complex right ventricular outflow tract obstruction. Ten had pulmonary valve stenosis, 4 had pulmonary valve stenosis plus ventricular septal defect, 8 had tetralogy of Fallot, and 2 had complex lesions. Propranolol (0.01 to 0.2 mg per kilogram of body weight) was given to patients with a residual right ventricular to left ventricular systolic pressure ratio greater than 0.75. Four children did not respond to propranolol and required further surgical intervent

Invasive and noninvasive phonocardiography and orifice-view aortography in the diagnosis of left ventricular outflow obstruction.

Phonocardiography and orifice-view aortography for the detection of valvular and subvalvular stenosis is reviewed. Intracardiac phonocardiography may be useful in detecting a left ventricular outflow tract obstruction, and in distinguishing it from other conditions that can produce an apparent pressure gradient during cardiac catheterization. The frequency analysis of heart sounds on noninvasive phonocardiograms may be useful in identifying subclinical aortic stenosis. Orifice-view aortography can show the anatomy of deformed aortic valves and is useful in measuring the orifice area. In patien

Right ventricular outflow tract stenting for obstructive lesions in adult congenital heart disease.

Currently, the use of stents in the right ventricular outflow tract (RVOT) has been described as alternative for palliation in neonates with Tetralogy of Fallot (ToF) and critical infundibular stenosis, who are not candidates for total correction in a single stage. However, there is limited evidence of the benefit of this intervention in other clinical scenarios, and just a few reports in adults. A retrospective study was conducted on 23 patients with significant RVOT obstruction who underwent RVOT stent placement and were not considered candidates for surgery. Patients 17 years old at the tim

Intracardiac Glomus Tumor Obstructing the Right Ventricular Outflow Tract: A Rare Case Report.

Intracardiac glomus tumors are very rare neoplasms arising from mesenchymal cells. Solitary glomus tumors are usually benign and mostly seen in the skin of limbs. Still, exceptional cases of noncutaneous visceral glomus tumors have been reported at sites other than the limbs. We present a case of glomangioma that was surgically removed from the right ventricle and briefly pointed to the importance of differential diagnosis of cardiac tumors. A 57-year-old woman was referred to our hospital for dyspnea and recurrent arrhythmia attacks. We performed a three-dimensional transthoracic echocardiogr
