Association between diabetes mellitus and fetal and maternal outcomes during and after pregnancy-a nationwide study.

INTRODUCTION: Diabetes Mellitus (DM) affects both the mother and fetus during and after pregnancy. Multiple studies have shown the prognostic impact of DM on maternal and fetal outcomes, but studies at the national level are limited. Therefore, we aimed to conduct this nationwide study. MATERIALS AND METHODS: This retrospective study used a Nationwide Inpatient Sample 2020 (NIS) using International Classification of Diseases (ICD-10) codes for adult pregnant patients. STATA version MP14.2 was used mainly for analysis. We used Fischer's exact test to compare proportions, the student's t-test t

The effect of lupus disease on the pregnant women and embryos: a retrospective study from 2010 to 2014.

BACKGROUND AND AIMS: Pregnancy in women with systemic lupus erythematosus (SLE) is one of the challenges of recent studies. Women should prevent the onset of relapses with medications before and after pregnancy, and on the other hand, the effect of these medicines considers the health and development of the fetus. In this retrospective study, the effects of anti-phospholipid syndrome and the use of common drugs such as methotrexate, cyclosporine, and azathioprine and their side effects on maternal health and ultimately the development of the fetus have been investigated. MATERIAL AND METHODS:

History of perinatal loss and maternal-fetal attachment behaviors.

BACKGROUND: Maternal-fetal attachment (MFA) is an important requirement for optimal maternal-infant adaptation. Current studies showed conflicting findings about whether a history of perinatal loss (fetal/neonatal death) affects maternal attachment in pregnancy. RESEARCH QUESTION: "Does a history of perinatal loss affect maternal-fetal attachment behaviors?" METHODS: One hundred women with and without a history of PL were recruited using a convenience method of sampling, from prenatal care services affiliated to Shahid Behesti University of Medical Sciences. Data collected by questionnaires

Preeclampsia: Long-term effects on pediatric disability.

BACKGROUND: Preeclampsia affects up to 10% of pregnancies worldwide and is one of the main causes of fetal morbidity and mortality. Although it has been linked to developmental delay, its long-term effects on neurologic development in children have yet to be sufficiently quantified. OBJECTIVES: To evaluate whether preeclampsia's severity and its obstetric management correlate to the degree of disability in these infants. MATERIALS AND METHOD: This is an observational and descriptive study performed on a population of 96 women who were diagnosed with preeclampsia at Hospital General Universit

Determinants of Birth Asphyxia Among Newborns at a Tertiary Care Hospital, Central Ethiopia: A Case-Control Study.

BACKGROUND: Birth asphyxia is a major cause of newborn deaths worldwide, especially in developing countries where access to skilled delivery care is limited. It is a significant health challenge in Ethiopia, contributing to many newborn deaths and long-term health issues. Despite efforts to improve maternal and newborn care, it remains a serious concern. Thus, this study was aimed at identifying the determinants of birth asphyxia among newborns at a tertiary care hospital in Central Ethiopia. METHODS: An institution-based unmatched case-control study was conducted among 345 participants with

Pre-labor rupture of membranes and associated factors among pregnant women admitted to the maternity ward, Northwest Ethiopia.

Pre-labor rupture of membranes (PROM) is the rupture of fetal membranes before the onset of labor. PROM is found in 3-15% of all pregnancies and 30-40% of preterm labor worldwide. The most serious complications are neonatal and prenatal mortality, which is higher in Africa, including Ethiopia. Despite a paucity of evidence on the magnitude and factors affecting PROM after 28 weeks of gestation but before the onset of labor (including both term and preterm PROM). Hence, the purpose of this study was to determine the magnitude and identify associated factors of the pre-labor rupture of membranes

Society for Maternal-Fetal Medicine Special Statement: Updated checklists for management of monochorionic twin pregnancy.

Approximately 20% of twin pregnancies are monochorionic-either monochorionic/diamniotic (MC/DA) or monochorionic/monoamniotic (MC/MA). The management of monochorionic twin pregnancy involves several additional interventions beyond the routine management of singletons or dichorionic twins. In 2020, the Society for Maternal-Fetal Medicine (SMFM) published checklists for the management of MC/DA and MC/MA twin pregnancies. SMFM now presents updated versions of these checklists reflecting recent changes in practice recommendations. Suggestions for implementing the use of the checklists into practic

Can promoting awareness of fetal movements and focusing interventions reduce fetal mortality? A stepped-wedge cluster randomised trial (AFFIRM).

BACKGROUND: In 2013, the stillbirth rate in the UK was 4.2 per 1000 live births, ranking 24th out of 49 high-income countries, with an annual rate of reduction of only 1.4% per year. The majority of stillbirths occur in normally formed infants, with (retrospective) evidence of placental insufficiency the most common clinical finding. Maternal perception of reduced fetal movements (RFM) is associated with placental insufficiency and increased risk of subsequent stillbirth. This study will test the hypothesis that the introduction of a package of care to increase women's awareness of the need for

Recombinant alpha-1-microglobulin: a potential treatment for preeclampsia.

Preeclampsia is a serious pregnancy-specific condition, affecting 10 million women annually worldwide. No specific treatment is currently available. Recent studies have demonstrated abnormal production and accumulation of free fetal hemoglobin in the preeclamptic placenta, and identified subsequent leakage into the maternal circulation as an important factor in the development of preeclampsia. A recombinant version of alpha-1-microglobulin, an endogenous well-characterized heme and radical scavenger, has been developed. Intravenous administration of recombinant alpha-1-microglobulin in animal

Prenatal Surgery for Open Fetal Spina Bifida in Patients with Obesity: A Review of Current Evidence and Future Directions.

Background: Obesity rates have significantly increased globally, affecting up to 40% of women of childbearing age in the United States. While prenatal repair of open fetal spina bifida has shown improved outcomes, most fetal surgery centers exclude patients with a body mass index (BMI) ≥ 35 kg/m2 based on criteria from the Management of Myelomeningocele Study (MOMS) trial. This exclusion raises concerns about healthcare equity and highlights a significant knowledge gap regarding the safety and efficacy of fetal spina bifida repair in patients with obesity. Objective: To review the current stat

Lowering the cesarean rate at a small USAF hospital.

From July 1, 1988 to June 30, 1989, programs were initiated at the 93d Strategic Hospital at Castle AFB, California, to lower the cesarean (C/S) rate. Programs included vaginal birth after cesarean, external cephalic version, adequate labor documentation, and peer review of all C/S for fetal distress. One year prior (N = 467) was compared to the year after the start of these programs (N = 430). A significant decrease of 21.2% to 10.2% (p < 0.0001) was documented. We conclude that aggressive management of obstetrical patients can reduce the rate of C/S in a small USAF hospital without increasin

Association Between Attempted External Cephalic Version and Perinatal Morbidity and Mortality.

OBJECTIVE: To examine whether, with fetal malpresentation at term, perinatal morbidity and mortality differ between women who undergo an external cephalic version (ECV) attempt and those who do not and are expectantly managed. METHODS: We conducted a retrospective cohort study of women with nonanomalous singleton gestations in nonvertex presentation delivering at a tertiary care institution from 2006 to 2016. Women undergoing an ECV attempt at 37 weeks of gestation or greater were compared with those with nonvertex fetuses who did not undergo an ECV attempt and delivered at 37 weeks of gestat

Termination of Pregnancy for Ultrasound diagnosed Major Fetal Anomalies at a Nigerian Tertiary Hospital: An Initial Experience.

BACKGOUND: Major Congenital anomalies are conditions that are severe enough to reduce life expectancy or compromise normal function. The introduction of prenatal screening for fetal anomalies to mid-trimester ultrasonography allows for early detection of many anomalies including the major ones. Once the diagnosis is made, the parents have the options of either terminating or continuing with the pregnancy after proper counselling. The purpose of this study is to determine the pattern of pregnancy termination for major congenital anomalies in our hospital after seven years of commencement of the

Customized growth charts: rationale, validation and clinical benefits.

Appropriate standards for the assessment of fetal growth and birthweight are central to good clinical care, and have become even more important with increasing evidence that growth-related adverse outcomes are potentially avoidable. Standards need to be evidence based and validated against pregnancy outcome and able to demonstrate utility and effectiveness. A review of proposals by the Intergrowth consortium to adopt their single international standard finds little support for the claim that the cases that it identifies as small are due to malnutrition or stunting, and substantial evidence tha

Novel histone deacetylase 6 gene variant is associated with fetal hydrocephalus and lateral ventriculomegaly.

OBJECTIVES: Congenital hydrocephalus (CH) is a neurological disease that significantly impacts fetal nervous system development and subsequent quality of life. CH pathogenesis may be linked to genetic factors such as defects in the histone deacetylase 6 (HDAC6) gene. However, only one CH family has been reported to date, limiting our comprehension of the relationship between HDAC6 and CH. Here, we investigated a family with an HDAC6-related fetal case of CH and ventriculomegaly, and propose a potential pathogenic mechanism involving the HDAC6 gene in CH development. METHODS: Skin tissue from
