Pulmonary Complications after Vertebral Body Tethering: Incidence, Treatment, Outcomes and Risk Factor Analysis.

INTRODUCTION: Vertebral body tethering (VBT) is gaining popularity for the management of selected AIS patients. The most frequent non-mechanical complications after VBT are pulmonary complications, with a reported incidence of up to 8% for recurrent pleural effusion. However, only trace data have been published on this topic. We aimed to analyze the incidence, timing, treatment, outcomes and risk factors of pulmonary complications after VBT. MATERIALS AND METHODS: All patients who underwent VBT between September 2018 and September 2022 were retrospectively reviewed. The rate of pulmonary comp…

Multimodal Approach to Vertebral Body Tethering With Erector Spinae Plane Blocks and Cryoablation.

Multimodal analgesia that combines around-the-clock medications and regional techniques can be especially effective for postoperative pain control. We describe a pediatric patient who underwent vertebral body tethering via an open thoracolumbar approach to treat juvenile idiopathic scoliosis. Erector spinae plane blocks (ESPBs), cryoablation to the intercostal nerves, and multimodal medications helped control our patient's pain well enough for her to be discharged home on postoperative day 2. To the best of our knowledge, this is the first report of this combination of regional techniques used…

Anterior Vertebral Body Tethering for Adolescent Idiopathic Scoliosis: Early Results and Future Directions.

Anterior vertebral body tether (AVBT) is a nonfusion surgical procedure for correction of scoliosis in skeletally immature individuals. With US Food and Drug Administration approval in 2019, AVBT technology is spreading and early to midterm reports are being published. Early clinical reports are promising while precise indications, outcomes, complication profiles, and best practices are being established. Patients who are skeletally immature and wish to avoid a fusion surgery may benefit from this procedure. This article highlights the translational science foundation, early to midterm clinica…

Trapezoidal Vertebral Body and Spine-Pelvis Sagittal Alignment in Patients with Lumber Spondylolisthesis.

BACKGROUND Trapezoidal changes of the vertebral body are more common in patients with lumbar spondylolisthesis than in others. However, we lack an understanding of factors predisposing to the development of a marked trapezoidal deformity. Also, no associations between a trapezoidal vertebrae (TV) and spine-pelvis sagittal parameters have been previously reported. MATERIAL AND METHODS A total of 73 subjects with lumbar spondylolisthesis were enrolled and we collected their clinical data. Vertebral body parameters and spine-pelvis sagittal alignment parameters were measured via lumbar spine X-ra…

Vertebral body stent augmentation to reconstruct the anterior column in neoplastic extreme osteolysis.

BACKGROUND: Extensive lytic lesions of the vertebral body (VB) increase risk of fracture and instability and require stabilization of the anterior column. Vertebral augmentation is an accepted treatment option, but when osteolysis has extensively destroyed the VB cortical boundaries (a condition herein defined as 'extreme osteolysis'), the risk of cement leakage and/or insufficient filling is high. Vertebral body stents (VBSs) might allow partial restoration of VB height, cement containment, and reinforcement, but their use in extreme osteolysis has not been investigated. OBJECTIVE: To assess…

Refracture of osteoporotic vertebral body after treatment by balloon kyphoplasty: Three cases report.

RATIONALE: Balloon kyphoplasty is a widely accepted treatment of osteoporotic vertebral compression fractures (OVCFs) with good results and a low risk for complications. A refracture of previously treated vertebra is a relatively rare condition. PATIENT CONCERNS: We reported our 3 cases and reviewed all relevant literatures of 11 cases with refracture of osteoporotic vertebral body after kyphoplasty. DIAGNOSES: Follow-up radiographs or magnetic resonance imaging examination confirmed refractures of previously treated vertebrae after kyphoplasty. INTERVENTIONS: One patient with 1 refracture …

Vertebral artery dissection due to an esophageal foreign body migration: a case report.

Unintentional foreign bodies' swallowing is a fairly common occurrence in ENT consultation especially among children. They usually pass through the gastrointestinal tract without complications. Migration of a foreign body through the esophageal wall is rare. It represents about 1% to 4% of all cases of foreign bodies' ingestion. A 16 year's old female patient has presented to ENT emergency with a painful dysphagia following an accidental ingestion of a metallic pin. Cervical X ray confirmed the presence of the pin while endoscopic investigations have shown no foreign body. Cervical CT scan rev

[Multilevel segmental interbody fusion versus vertebral body replacement: comparison of two operative methods].

BACKGROUND: The surgical techniques of interbody fusion and vertebral body replacement represent two concurrent options for multilevel anterior decompression and arthrodesis of the cervical spine. PATIENTS AND METHODS: In a retrospective study the data from 61 patients who received either interbody fusion (n = 38) as anterior cervical discectomy and fusion (ACDF) or vertebral body replacement (n = 23) (as anterior cervical corpectomy and fusion (ACCF) because of degenerative disc disease of the cervical spine were collected. RESULTS: The clinical outcome was better at all time points in the …

Thoracic vertebral body fracture after total hip replacement in diffuse idiopathic skeletal hyperostosis.

This article describes the critical clinical sequelae of a patient with diffuse idiopathic skeletal hyperostosis who sustained an acute iatrogenic thoracic vertebral body fracture with subsequent spinal cord injury after a total hip replacement, with a final lethal course.A 57-year-old woman was referred to the authors' institution after undergoing a total hip replacement in the supine position for secondary osteoarthritis. Postoperatively, the patient had symptoms of an incomplete paraplegia. Computed tomography scan and magnetic resonance imaging revealed diffuse idiopathic skeletal hyperost…

Early spondylodiscitis presenting with single vertebral body involvement: a report of two cases.

Infectious spondylodiscitis is an uncommon disease with increasing incidence that typically presents with abnormalities in two adjacent vertebral bodies and the intervening disk. We describe two cases that initially presented with imaging abnormalities in only a single vertebral body. Both patients had a history of lumbar back pain and elevated inflammatory markers, but the lack of classical spondylodiscitis imaging findings led to diagnostic delay and confusion. It is likely that the incidence of atypical presentations of spondylodiscitis will increase as the disease incidence increases and i…

Traumatic lateral expulsion of the L-4 vertebral body from the spinal column.

Traumatic lateral spondyloptosis is mostly a lateral shearing injury that must be tremendous enough to completely disrupt the strong musculoligamentous and bony structures. This injury has only been described at single levels in the lumbar spine. Lateral expulsion of a vertebral body from the spinal column due to 2-level adjacent spondyloptosis has not been previously reported. This 16-year-old girl was referred to our center for the management of an extremely unusual L2-5 fracture-dislocation. Motor deficits were incomplete and sacral sensation was spared. Three-dimensional reconstructed CT s…

Augmentation improves human cadaveric vertebral body compression mechanics for lumbar total disc replacement.

STUDY DESIGN: Cadaveric biomechanical study. OBJECTIVE: To quantify the effects of vertebral body augmentation on biomechanics under axial compression by a total disc replacement (TDR) implant. SUMMARY OF BACKGROUND DATA: TDR is a surgical alternative to lumbar spinal fusion to treat degenerative disc disease. Osteoporosis in the adjacent vertebrae to the interposed TDR may lead to implant subsidence or vertebral body fracture. Vertebral augmentation is used to treat osteoporotic compression fracture. This study sought to evaluate whether vertebral augmentation improves biomechanics under TD…

Combined posterior-anterior stabilisation of thoracolumbar injuries utilising a vertebral body replacing implant.

The authors report on a prospectively followed series of 35 patients with injuries of the thoracolumbar spine from T7 to L3. The radiological course after combined posterior-anterior surgery with anterior column reconstruction with a distractible vertebral body replacing implant demonstrated a stable reconstruction technique with almost no re-kyphosing. In 18/18 patients with CT follow-up intervertebral fusion was observed as bony bridging lateral to the VBR implant. The functional/clinical outcome of the patients was analysed with a set of eight validated outcome scales. After an average foll…

Antiresorptive Medications Prior to Stereotactic Body Radiotherapy for Spinal Metastasis are Associated with Reduced Incidence of Vertebral Body Compression Fracture.

STUDY DESIGN: Retrospective Cohort. OBJECTIVE: Antiresorptive drugs are often given to minimize fracture risk for bone metastases, but data regarding optimal time or ability to reduce stereotactic body radiotherapy (SBRT)-induced fracture risk is limited. This study examines the association between antiresorptive use surrounding spinal SBRT and vertebral compression fracture (VCF) incidence to provide information regarding effectiveness and optimal timing of use. METHODS: Patients treated with SBRT for spinal metastases at a single institution between 2009-2020 were included. Kaplan-Meier an

Intraoperative three-dimensional imaging-guided vertebral body stenting for cleft-associated very severe osteoporotic vertebral compression fractures: a surgical technique case series.

Very severe osteoporotic vertebral compression fractures (vsOVCF) associated with intravertebral clefts represent a technically challenging subgroup of vertebral augmentation because of severe vertebral collapse, distorted intravertebral anatomy, and increased risk of cement leakage. Conventional fluoroscopy alone may be insufficient for accurate device positioning in these cases. We describe the surgical technique and technical utility of intraoperative three-dimensional (3D) imaging during vertebral body stenting (VBS) in three consecutive patients with cleft-associated vsOVCF without neurol…
