Leukaemia and smoking habits among United States veterans.

The relation between leukaemia and smoking habits was examined in data from the veterans' smoking study, a prospective study of mortality among 248,000 United States veterans, of whom 723 died of leukaemia during 1954-69. A significant increase in mortality from leukaemia among cigarette smokers (relative risk 1.53) was found, together with a dose-response relation with amount smoked (trend p less than 0.001). The relation was strongest (relative risk 1.72) for monocytic and chronic and unspecified myeloid leukaemias (ICD (7th revision) codes 204.1 and 204.2). For these leukaemias the increase…

Medical Debt, Credit Card Debt, and Other Financial Debt Among Veterans Who Receive Homeless Assistance in the Veterans Affairs Healthcare System.

The US Department of Veterans Affairs (VA) healthcare system is dedicated to preventing veteran homelessness, but there is limited research on the specific sources of financial debt among homeless veterans. We investigated the proportion of veterans seeking VA homeless services reporting any debt and specific types of outstanding debt, associations between homelessness experiences and debt, and whether these associations vary by sex. Using VA administrative data, we cross-sectionally evaluated various types of outstanding debt among 118 594 homeless and at-risk veterans in the VA healthcare sy…

Colorectal cancer screening, incidence, and mortality among aging veterans in the U.S. Department of Veterans Affairs healthcare system by housing status.

The objectives of this study are to examine colorectal cancer (CRC) screening, incidence, and mortality among veterans 45 to 75 years who are enrolled in the U.S. Department of Veterans Affairs (VA) healthcare system by housing instability or homelessness (HUH) and determine whether receiving primary care via Homeless Patient Aligned Care Teams (HPACTs) affects CRC screening, incidence, and mortality among veterans experiencing HUH. A retrospective cohort study was conducted among > 3,000,000 veterans who sought VA healthcare services (1 January 2019 to 31 December 2021) using three linked VA …

Changes in Reproductive and Sexual Health Diagnoses of Women Veterans Using Department of Veterans Affairs Health Care Between FY2010 and FY2018.

BACKGROUND: Women veterans are the fastest growing group of new Veterans Health Administration (VA) users, with more than half a million women veterans using VA care in 2018. VA has implemented policies and programs to meet the reproductive and sexual health (RSH) needs of women veterans who increasingly rely on VA to provide comprehensive care. OBJECTIVE: We aimed to describe changes in the prevalence of RSH diagnosis categories among women veterans using VA care in fiscal year (FY) 2010 and FY2018. METHODS: Repeated cross-sectional VA administrative data were used to identify women veteran…

Resilience to Suicidal Ideation Among U.S. Military Veterans with Posttraumatic Stress: Results from the National Health and Resilience in Veterans Study.

Veterans with combat exposure experience high rates of posttraumatic stress symptoms (PTSS) and associated suicidal ideation. The current study examined whether social support (i.e., social connectedness and social engagement) and protective psychological factors (i.e., resilience and altruism) moderated the relation between PTSS and suicidal ideation severity in a sample of 149 U.S. military combat veterans who served in the Vietnam War or Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn (OEF/OIF/OND). Consistent with expectations, initial PTSS were positively associated …

Comparison of opioid use disorder among male veterans and non-veterans: Disorder rates, socio-demographics, co-morbidities, and quality of life.

BACKGROUND AND OBJECTIVES: Amidst a surging national crisis of opioid use, concern has been expressed about its impact on veterans, but no study has presented a population-based comparison of opioid use disorder (OUD) among veterans and non-veterans. We analyzed national epidemiologic data to compare rates, correlates and impacts of the opioid crisis on male veterans and non-veterans. METHODS: Restricted data from 2012 to 2013 National Epidemiologic Survey on Alcohol and Related Conditions-III (NESARC-III) were used to compare veteran and non-veteran men on rates of OUD, as well as correlates…

Timing of evidence-based psychotherapy for posttraumatic stress disorder initiation among Iraq and Afghanistan war veterans in the Veterans Health Administration.

OBJECTIVE: Cognitive processing therapy (CPT) and prolonged exposure therapy (PE) were widely disseminated to treat posttraumatic stress disorder (PTSD) in the Veterans Health Administration (VHA). However, few Iraq and Afghanistan war veterans (Operation Enduring Freedom [OEF], Operation Iraqi Freedom [OIF], Operation New Dawn [OND]) diagnosed with PTSD have received CPT/PE and many initiate CPT/PE after substantial delay. Veterans who do not initiate CPT/PE or initiate CPT/PE after delay may have poorer treatment outcomes. This study aimed to identify predictors of CPT/PE initiation and timi…

Using Self-Identified Gender Identity Data to Advance Health Equity Among Transgender and Gender Diverse Veterans in the Veterans Health Administration.

BACKGROUND: Identification of transgender and gender diverse (TGD) people has been limited to diagnoses and text rather than self-identified gender identity (SIGI), representing a subset of TGD people. In 2017, the Veterans Health Administration (VHA) implemented SIGI, allowing for precise identification of TGD veterans, including subgroups (transgender man, transgender woman, and nonbinary). OBJECTIVES: Health conditions, adverse social determinants of health (SDOH), and health care utilization were compared among veterans (1) identified by SIGI only, both SIGI and diagnosis/text, diagnosis/…

Short and mid-term research priorities for Veterans with multiple sclerosis: A modified Delphi process engaging Veterans, researchers, and operational partners.

BACKGROUND/OBJECTIVE: Identifying research priorities of Veterans, MS researchers, and key stakeholders is critical to advance high-quality, evidence-based, and Veteran-specific MS care. METHODS: We used a modified Delphi approach to identify research priorities for Veterans with MS. Electronic surveys were distributed to Veterans with MS (n = 50,975), MS researchers (n = 191), VA healthcare providers (1,337), and funding agency representatives (n = 6) asking about their 2-3 most important research questions that would benefit Veterans with MS for researchers to answer in the next 5-10 years.…

Determinants of new-onset alcohol use disorder in U.S. military veterans: Results from the National Health and Resilience in Veterans Study.

BACKGROUND: Although veterans are at increased risk of alcohol use disorder (AUD) relative to civilians, few longitudinal studies have examined both risk and protective factors that influence the development of AUD. This study aimed to identify risk and protective factors that contribute to incident AUD. METHODS: Data were analyzed from the National Health and Resilience in Veterans Study (NHRVS), a nationally representative, prospective cohort study of U.S. veterans. The sample included 1,770 veterans who did not meet criteria for lifetime AUD at Wave 1 and completed at least one follow-up a…

Ensuring Continuity of Transitional Housing for Homeless Veterans: Promoting Disaster Resilience among the Veterans Health Administration's Grant and Per Diem Providers.

The US Department of Veterans Affairs (VA) has committed significant resources toward eliminating homelessness among veterans as part of its health care mission. The VA Grant and Per Diem (GPD) program funds non-VA, community-based organizations to provide transitional housing and support services to veterans experiencing homelessness. During a disaster, GPD grantee organizations will be especially critical in ensuring the well-being of veterans residing in their programs. Recognizing the need to ensure continued access to this residential care, the VA GPD program implemented a disaster prepar…

Mental Health and Medical Health Disparities in 5135 Transgender Veterans Receiving Healthcare in the Veterans Health Administration: A Case-Control Study.

PURPOSE: There are no large controlled studies of health disparities in transgender (TG) or gender dysphoric patients. The Veterans Health Administration (VHA) is the largest healthcare system in the United States and was an early adopter of electronic health records. We sought to determine whether medical and/or mental health disparities exist in VHA for clinically diagnosed TG veterans compared to matched veterans without a clinical diagnosis consistent with TG status. METHODS: Using four ICD-9-CM codes consistent with TG identification, a cohort of 5135 TG veterans treated in VHA between 1…

Current perspectives on the pharmacotherapeutic management of insomnia in Veterans.

INTRODUCTION: Insomnia is a highly prevalent sleep disorder among Veterans and frequently co-occurs with post-traumatic stress disorder (PTSD), depression, chronic pain, and substance use disorders. AREAS COVERED: This review synthesizes the current evidence on pharmacological management of insomnia in Veterans with attention to guideline recommendations, veteran-specific safety considerations, and comorbidity-driven prescribing. We discuss the rationale for reducing long-term reliance on benzodiazepines and benzodiazepine receptor agonists (BZRAs) in favor of agents with more favorable long-…

Risk of dementia in older veterans with multiple sclerosis.

BackgroundWhile it is widely accepted that multiple sclerosis (MS) often causes cognitive dysfunction, it is thought that these cognitive symptoms rarely progress to dementia. However, this has not been thoroughly investigated. The objectives of this cohort study are to determine whether people with MS have an increased risk of dementia compared to the general population and to identify factors, such as geographic latitude, which may modify this association.MethodsWe studied data from a random sample of US veterans aged ≥ 55 years followed at Veterans Affairs Health Care Systems nationwide fro…
