Speech and oromotor deficits of epileptic origin in benign partial epilepsy of childhood with rolandic spikes (BPERS). Relationship to the acquired aphasia-epilepsy syndrome.

The authors report three children who suffered temporary oromotor or speech disturbances as focal epileptic manifestations within the frame of benign partial epilepsy of childhood with rolandic spikes and review similar cases described in the literature. The deficit can occur as an initial symptom of the disorder without visible epileptic seizures and interferes in a variable way with simple voluntary oromotor functions or complex movements including speech production, depending on the exact location and spread of the discharging epileptic focus around the perisylvian region. The most severe d

Error analysis of limb and orofacial praxis in children with developmental motor deficits.

Two models have been suggested to depict the relationship between disorders of limb and orofacial praxis. The first views apraxia as a unitary disorder in which the underlying mechanisms for each type are similar, while the second model suggests that there are two separate praxis systems: one for planning and controlling limb gestures and a second one for planning and controlling orofacial movements. The purpose of this study was to investigate whether a common mechanism may underlie deficits in limb and orofacial praxis in children. This was done by analyzing the types of praxis errors demons

Women's views of second-stage labor as assessed by interviews and videotapes.

Twenty women were selected from public and private maternity services and interviewed by nurses and nurse-midwives on a research team. The women were shown videotapes of their second-stage labors that for 15 of them had been recorded by 2 other members of the research team. Four videos were provided by mothers who had had home births and one from a mother whose birth was filmed at a birth center. The interviews were analyzed for major themes; the theme reported here is women's experiences of pushing. Women reported wide variations in sensations during the second stage. Whereas 9 of 16 women ex

A computer-supported method for analyzing behavioral observations: studies with stereotypy.

The present report describes an observational method for quantifying behavior including drug-induced stereotypy, and employs an electronic data-collecting device with microcomputer hardware and software support. This method generates interval scale data, thus permitting the application of powerful parametric statistics, and also allows examination of discrete response topographies. For example, the method permits parametric comparisons among drug doses, treatment groups, as well as over time. The effects of the atypical antipsychotic drug thioridazine on apomorphine-induced stereotypy were use

Analysis of durations and frequencies of mouthing behaviors in children 6 months to 6 years: an advanced videotaping and translation approach.

BACKGROUND: Children are susceptible to harmful substances found in indoor dust through frequent hand-to-mouth and object-to-mouth contacts. Despite the importance of this exposure route, few studies have quantified the frequency, duration, or patterns of mouthing behaviors in indoor residential settings, creating uncertainty in exposure and risk models. OBJECTIVE: This study quantified the frequency and duration of indoor mouthing contacts for children 6 months to 6 years across sociodemographic variables including age, sex, ethnicity, race and geographic region using advanced videotaping an

Video feedback: A novel application to enhance person-centred dementia communication.

AIM: A discussion of the use of video feedback as an effective and feasible method to promote person-centred communication approaches within dementia care and long-term care. BACKGROUND: Effective strategies to integrate person-centred approaches into health care settings have attracted global attention and research in the past two decades. Video feedback has emerged as technique to enhance reflective learning and person-centred practice change in some care settings; however, it has not been tested in the context of person-centred dementia communication in long-term care. DESIGN: Discussion

Endoscopic assessment of the oesophageal features of eosinophilic oesophagitis: validation of a novel classification and grading system.

OBJECTIVE: Abnormalities are commonly identified during endoscopy in eosinophilic oesophagitis (EoE). There is no standardised classification to describe these features. This study aimed to evaluate the interobserver agreement of a grading system for the oesophageal features of EoE. METHOD: The proposed system incorporated the grading of four major oesophageal features (rings, furrows, exudates, oedema) and the presence of additional features of narrow calibre oesophagus, feline oesophagus, stricture and crepe paper oesophagus. Endoscopic videos from 25 patients with EoE and controls were rev

Detection of orbicularis oris muscle defects in first degree relatives of cleft lip children using ultrasound.

OBJECTIVE: The severity of cleft lip (CL) varies considerably from complete bilateral CL and palate at one end of the spectrum to a minimal CL at the other. In some cases of microform clefting, there may be no visible manifestation of the defect on the lip surface (i.e., the defect is occult) and no residual functional deficit. This study used high resolution ultrasonography to detect subclinical anomalies of orbicularis oris muscle (OOM) in first degree relatives of CL +- cleft palate children and compared it with controls. MATERIALS AND METHODS: Thirty relatives of 25 children with non-synd

Self-induced seizures presumably by peri-orbital somatosensory self-stimulation: a report of two cases.

Self-induced seizures by somatosensory stimulation are rare. We describe two epileptic patients with self-induced seizures presumably by peri-orbital somatosensory stimulation. Two infants with severely delayed psychomotor development and poor visual acuity after acute subdural hemorrhage in early infancy had been diagnosed as having West syndrome. They evolutionally became to show serial self-induced seizures preceded by rubbing eye with finger in one case and touching right eyebrow with the back of left hand in the other case. Video-electroencephalography (EEG) monitoring was useful to confi

Axis rotation and visually induced motion sickness: the role of combined roll, pitch, and yaw motion.

BACKGROUND: Motion sickness (MS) is a well-known phenomenon in aviation and in virtual environments such as simulators or computer games. The severity of MS is thought to be due to the amount of sensory conflict, which should increase with the complexity of the simulated motion. The present study focused on the direction and complexity of simulated body rotations in the genesis and severity of visually induced MS. METHODS: Three simulated rollercoaster rides including translational movement in the fore-aft axis and additional rotational motion either in pitch only, along the pitch and roll ax

[Application of video laryngoscope for vocal cord synechiotomy in a patient with recurrent laryngeal nerve paralysis].

Vocal cord synechia causes respiratory disturbance and severe pneumonia. A 63-year-old woman with recurrent laryngeal nerve paralysis caused by translaryngeal intubation after resection of acoustic tumor and by thyroid surgery in her history and progressive dyspnea, had received vocal cord synechiotomy under general anesthesia. Preoperative endoscopic examination revealed edematous larynx, immobility of left unilateral vocal fold, insufficient mobility of right vocal fold, left arytenoid cartilage dislocation and a posterior glottic adhesion. Anesthesia was induced by propofol administration,

Elevated blood lead concentrations in essential tremor: a case-control study in Mersin, Turkey.

BACKGROUND: Essential tremor (ET) is one of the most common neurologic disorders. Aside from underlying susceptibility genes, recent studies have also begun to focus on environmental toxic factors. Yet there remains a paucity of information on such factors, making studies of environmental factors important. A recent study in New York City found blood lead concentrations to be elevated in ET cases compared with matched controls. Chronic exposure to lead produces cerebellar damage, and this could predispose individuals to develop ET. OBJECTIVE: The aim of this study was to determine whether the

Clinical assessment of infant colour at delivery.

OBJECTIVE: Use of video recordings of newborn infants to determine: (1) if clinicians agreed whether infants were pink; and (2) the pulse oximeter oxygen saturation (Spo(2)) at which infants first looked pink. METHODS: Selected clips from video recordings of infants taken immediately after delivery were shown to medical and nursing staff. The infants received varying degrees of resuscitation (including none) and were monitored with pulse oximetry. The oximeter readings were obscured to observers but known to the investigators. A timer was visible and the sound was inaudible. The observers wer

Clinician agreement on gait pattern ratings in children with spastic hemiplegia.

The level of agreement between clinicians' ratings of gait patterns in children with spastic hemiplegia was investigated using the Winters, Gage, and Hicks (WGH) classification scale. Sixteen clinicians (nine physiotherapists, seven orthopaedic surgeons) from six gait laboratories in five different countries rated gait patterns in 34 children with spastic hemiplegia (22 males, 12 females; mean age 10y [SD 3y], range 6-18y). Archived three-dimensional gait reports (kinematic and video data) and videos (video clips only) were rated for each child. Agreement between clinicians was substantial for

Eshkol-Wachman movement notation in diagnosis: the early detection of Asperger's syndrome.

The diagnostic criteria of Asperger's syndrome (AS), considered a part of the autistic spectrum disorder, are still unclear. A critical marker, which distinguishes AS from autism, is the presence of language. The ability of a child with AS to acquire and use language early results in the fact that AS usually is diagnosed much later than autism. Autism is not usually diagnosed until around the age of 3, whereas AS usually is not diagnosed until the child is 6 or 7 years of age. In the present article, using Eshkol-Wachman movement notation, we present evidence that abnormal movement patterns ca

Video analysis of standing--an alternative footprint analysis to assess functional loss following injury to the rat sciatic nerve.

The rat sciatic nerve is a well-established animal model for the study of recovery from peripheral nerve injuries. Footprint analysis is the most widely used non-invasive method of measuring functional recovery after injury in this model. We describe a new alternative video analysis of standing (or static footprint video analysis) to assess functional loss following injury to the rat sciatic nerve, during animal standing or periodic rest on a flat transparent surface. We found good correlation between video recording during standing and dynamic ink track footprint parameter measurements for bo

Acute cocaine effects on stereotype and defense: an ethoexperimental approach.

Cocaine administration to laboratory animals may produce locomotor hyperactivity and stereotypies that include sniffing and rearing, in addition to anxiety-like effects. A time-sampling study of the effects of 3, 10 or 30 mg/kg cocaine (i.p.) over time following injection indicated early enhancement of locomotion and crouching, with the latter most increased in low- and intermediate-dose cocaine groups, with increased rearing and standing during the second hour of the test period. Additional analyses at 30-60 min post-injection suggested qualitative changes in rearing, with high dose animals s
