Portal vein thrombosis and liver transplant survival benefit.

Portal vein thrombosis (PVT) complicates the liver transplant operation and potentially affects waiting list survival. The implications on calculations of survival benefit, which balance both waiting list and posttransplant survival effects of PVT, have not been determined. The objective of this study is to describe the effect of PVT on the survival benefit of liver transplantation. Using Scientific Registry of Transplant Recipients data on adult liver transplant candidates wait-listed between September 2001 and December 2007, Cox proportional hazard models were fitted to estimate the covariat

Periodic limb movements in sleep are associated with stroke and cardiovascular risk factors in patients with renal failure.

Periodic limb movements in sleep (PLMS) is prevalent among dialysed patients and is associated with increased risk of mortality. Our study aimed to determine the prevalence of this disease in a sample of transplanted and waiting-list haemodialysed patients. One hundred transplanted and 50 waiting-list patients underwent polysomnography. Moderate and severe diseases were defined as periodic limb movements in sleep index (PLMSI) higher than 15 and 25 events h(-1), respectively. The 10-year coronary heart disease risk was estimated for all patients using the Framingham Score. Moreover, the 10-yea

Impact of organ prioritization for immunologic sensitization and waiting times for heart transplantation.

BACKGROUND: The Canadian status 4S category prioritizes highly sensitized patients with a calculated panel reactive antibody (CPRA) > 80% awaiting heart transplantation. We examined the effect of sensitization and status 4S and developed a predictive model to estimate waiting time in Canada. METHODS: A retrospective review was performed of patients listed for heart transplant at the Ottawa Heart Institute and Toronto General Hospital (Ontario, Canada). We evaluated the association of CPRA and priority listing status on waiting time and post-transplant outcomes. Waiting time risk factor analys

Access to the kidney donor pool for racial minority population is maximized by a variance of the UNOS point system--a regional experience. Regional Organ Bank of Illinois.

1. A local variance of the UNOS point system that awards no points to lesser HLA matches and gives slightly more weight to PRA and waiting time was evaluated by examining waiting times to transplant for 1,342 transplant candidates listed during a 3-year period (1989 through 1991). 2. Patients experienced long waiting times attributed to a large waiting list and donor shortage, but the differences in median waiting time between Blacks (22 months) and non-Blacks (17 months) was smaller than what has been reported for patients nationwide. 3. The longer waiting times for Blacks and fewer Blacks be

Liver Transplantation in Patients With Cystic Fibrosis: 30 Years of Experience in Australia and New Zealand.

OBJECTIVES: To review the waiting list data and outcomes after liver transplantation (LT) in patients with cystic fibrosis (CF) in Australia and New Zealand. METHODS: The Australia and New Zealand Liver and Intestinal Transplant Registry was utilised to identify patients with CF listed for LT, including as a component of multi-organ transplantation. Outcomes were compared to non-CF patients. RESULTS: Between January 1989 and July 2022, 55 patients with CF underwent LT at a median age of 18.9 (13.5-30.0) years. Twenty-five (45%) were less than 18 years of age at the time of LT. Thirty-seven (

Factors associated with transition to a nursing home in older adults living in naturally occurring retirement communities.

BACKGROUND: Naturally occurring retirement communities (NORCs) are geographic areas (generally high-rise buildings or neighborhoods) that have a high concentration of individuals 65 years and older. Supportive service programs in NORCs can address resident needs and delay nursing home (NH) admission but understanding what factors are associated with NORC residents requiring NH admission is needed to tailor such programs. Our aim was to examine individual- and neighborhood-level factors associated with NH wait-list status in NORC residents in Ontario. METHODS: We conducted a population-based,

An Analysis of Free-Text Refusals as an Indicator of Readiness to Accept Organ Offers in Liver Transplantation.

Racial/ethnic minorities experience higher rates of wait-list mortality and longer waiting times on the liver transplant wait list. We hypothesized that racial/ethnic minorities may encounter greater logistical barriers to maintaining "readiness" on the wait list, as reflected in offer nonacceptance. We identified all candidates who received an organ offer between 2009 and 2018 and investigated candidates who did not accept an organ offer using a free-text refusal reason associated with refusal code 801. We isolated patients who did not accept an organ offer due to "candidate-related logistica

Gamete donation in a system of need-adjusted reciprocity.

A new paradigm to collect and allocate donor gametes is proposed. In the system, called indirect mirror exchange, the partner of the person who needs donor gametes, donates in exchange for bonus points that are awarded to the infertile person. All candidate recipients of donor gametes are ranked on a waiting list and receive points according to waiting time, medical urgency, phenotypic resemblance and contribution. According to the principle of fairness, persons who benefit from a system are obliged to contribute their share. However, strict reciprocity is rejected as unfair. A contribution by

Impact of the pandemic on surgical activity in colorectal cancer in Spain. Results of a national survey.

INTRODUCTION: The pandemic has had an impact on colorectal cancer surgery in hospitals. In 2020, up to 75% of colorectal cancer patients are estimated to require surgery. No objective data on the impact of the pandemic on the management of surgical waiting lists is available. We conducted a survey in colorectal surgery units to assess the impact on colorectal cancer surgery waiting lists. METHOD: All personnel in charge of colorectal surgery units nationwide received a survey (from February to April, 2020) with eight questions divided into three sections-cessation date of colorectal cancer su…

Trends in patient selection for heart transplantation.

OBJECTIVES: The purpose of this study was to review specific outcomes of patient referrals and the utility of selection criteria for heart transplantation at a single transplant center and to assess important trends over a 5-year period. BACKGROUND: Although patient selection criteria are important for the clinical success of heart transplantation and the optimal utilization of the limited supply of donor organs, there are few data regarding actual outcomes and whether selection criteria are facilitating the identification of the most appropriate patients. METHODS: We retrospectively reviewe

Waiting Time Prioritisation for Public and Private Providers: Evidence From Hip Replacement Surgery.

Waiting times for planned health care treatments have been increasing in England and other OECD countries. One policy to reduce the impact of waiting times on patients' health is for doctors to prioritise patients according to their health status. This study tests the extent to which publicly-funded patients in poorer health are prioritised on the list, and whether prioritisation differs between public and private providers. We use hospital administrative data in 2015-2021 on hip replacements in England. Our results provide evidence of inpatient waiting-time prioritisation (from specialist add

Waiting for elective orthopaedic surgery: A qualitative scoping review of the patient experience.

BACKGROUND: Public health waiting lists for elective or planned orthopaedic surgery are increasing to the point where demand far exceeds our ability to treat. This is largely due to ageing populations, rising obesity and the continued impact of delays caused by the COVID-19 pandemic. While waiting, patients suffer debilitating physical pain, functional limitation, social isolation and associated mental health impacts while some are unable to work. This qualitative scoping review explored and mapped the experiences of patients of waiting for elective or planned orthopaedic surgery. METHODS: Th…

Impact of recipient body mass index on organ allocation and mortality in orthotopic heart transplantation.

BACKGROUND: It is unknown whether obesity affects organ allocation in orthotopic heart transplantation (OHT). The United Network for Organ Sharing (UNOS) database provides an opportunity to examine this issue. METHODS: We reviewed UNOS data to identify 27,002 OHT candidates placed on the heart transplantation wait list (1998 to 2007). Patients were stratified by body mass index (BMI) at listing. Multivariate Cox proportional hazards model estimated the chance of receiving OHT, adjusting for factors that might affect allocation. Mortality on the wait list and post-OHT mortality were estimated

Do Patients Supported With Continuous-flow Left Ventricular Assist Device Have a Sufficient Risk of Death to Justify a Priority Allocation? A Propensity Score Matched Analysis of Patients Listed in UNOS Status 2.

BACKGROUND: Outcomes of continuous flow left ventricular assist devices (CF-LVADs) as bridge to transplant have significantly improved. The question has arisen whether patients on CF-LVADs have an increased risk of death on the waiting list as to justify a priority allocation (status 1). The aim of this study was to compare the survival after implantation of CF-LVADs with the survival on the waiting list for patients initially listed in United Network for Organ Sharing (UNOS) status 2. METHODS: All patients 18 years or older listed for heart transplantation (HT) in the United States between 2

[Innovations in transplantation medicine].

More than 70 years after the first successful organ transplant between identical twins, transplantation medicine still faces significant challenges today: How can the large number of people waiting for a new kidney, liver, lung, or heart receive a suitable organ more quickly? How can survival on the waiting list be extended? How can long-term graft survival with high quality of life be ensured after transplantation? The various disciplines of transplantation medicine are working daily to address these challenges. The goal of this overview article is to provide practicing physicians with an ove
