The Price of Exposure: Xeroderma Pigmentosum and Skin Cancer.

Xeroderma pigmentosum (XP) is a rare autosomal recessive genodermatosis characterized by a defect in the nucleotide excision repair pathway of DNA. This condition leads to extreme photosensitivity, pigmentary changes, premature aging, and increased risk of UV-induced skin and mucous membrane neoplasms. We report a case of squamous cell carcinoma in a 55-year-old female with XP. A 55-year-old female presented with a single asymptomatic skin lesion on the left cheek that had developed over the past two years. Examination revealed a well-defined, round to oval black raised nodulo-plaque, with a c…

Impression cytology of ocular surface in xeroderma pigmentosum.

PURPOSE: To describe cellular alterations detected by impression cytology of the ocular surface in patients with xeroderma pigmentosum. The secondary objective was to assess the reliability of impression cytology in diagnosing ocular surface squamous neoplasia. METHODS: Patients with xeroderma pigmentosum underwent a single-day complete ophthalmological examination and impression cytology for ocular surface evaluation using 13 mm diameter mixed cellulose esters membrane filters and combined staining with Periodic Acid Schiff, Hematoxylin and Eosin, and Papanicolaou stains followed by microsco…

Sarcomatoid Carcinoma of Orbit in a Patient With Xeroderma Pigmentosum.

Spindle cell carcinoma (SpCC), also referred to as sarcomatoid carcinoma, makes up less than 3% of all head and neck squamous cell carcinomas (SCCs). It is an uncommon and unusual biphasic malignant tumor primarily affecting the upper aero-digestive tract. SpCC consists of spindled or pleomorphic tumor cells. Mostly, these tumors are seen in the fifth or sixth decades of life and are strongly associated with smoking and alcohol. We hereby report an infrequently encountered case of SpCC in a patient with xeroderma pigmentosum (XP), who was young, nonsmoker, and did not consume alcohol. The mass…

Xeroderma Pigmentosum: A Model for Human Premature Aging.

Aging results from intrinsic changes (chronologic) and damage from external exposures (extrinsic) on the human body. The skin is ideal to visually differentiate their unique features. Inherited diseases of DNA repair, such as xeroderma pigmentosum (XP), provide an excellent model for human aging due to the accelerated accumulation of DNA damage. Poikiloderma, atypical lentigines, and skin cancers, the primary cutaneous features of XP, occur in the general population but at a much older age. Patients with XP also exhibit ocular changes secondary to premature photoaging, including ocular surface…

Corneal changes in xeroderma pigmentosum: a clinicopathologic report.

PURPOSE: To report the clinicopathologic features of corneal involvement in patients with xeroderma pigmentosum. DESIGN: Retrospective review of corneal histopathology. METHODS: Thirteen corneal specimens of 11 patients with xeroderma pigmentosum who underwent keratoplasty (lamellar/full-thickness) for corneal involvement were studied. Five-micrometer-thick sections were made from all the samples and stained using hematoxylin-eosin and periodic acid-Schiff stains. A light microscopic examination was performed to study the histopathologic changes. RESULTS: The corneal findings on clinical ex…

[Conjunctival melanoma and xeroderma pigmentosum, a rare association].

Ocular involvement in the xeroderma pigmentosum (XP) can be located on the eyelids, conjunctiva or cornea. Malignant melanoma of the conjunctiva is a rare tumor, and even more so in children. Only five cases associated with XP have been published since 1960. We report a sixth case occurring in a girl aged 14. Surgical treatment consisted of an orbital exenteration extended to the eyelids and the evolution is marked by a good healing of the orbital cavity without local recurrence after four months.

Multiple cutaneous malignancies in a patient of xeroderma pigmentosum.

Xeroderma pigmentosum is a genodermatosis characterized by photosensitivity and the development of cutaneous and internal malignancies at an early age. The basic defect underlying the clinical manifestations is a nucleotide excision repair defect, leading to defective repair of DNA damaged by ultraviolet radiation. These patients exhibit enhanced sensitivity to ionizing radiation. Patients with xeroderma pigmentosum who are younger than 20 years of age have a greater than 1000-fold increased risk of developing skin cancer. Early detection of these malignancies is necessary because they are fas…

Xeroderma pigmentosum: a case report and review of the literature.

Inherited molecular defects in nucleotide excision repair genes cause the autosomal recessive condition xeroderma pigmentosum. Xeroderma pigmentosum is characterized by photo-hypersensitivity of sun-exposed tissues, and by a several thousand-fold increase in the risk of developing malignant neoplasms of the skin and of the eyes. Mutations in xeroderma pigmentosum genes that regulate nucleotide excision repair, not only predispose persons with xeroderma pigmentosum to multiple malignancies, but also promote premature cutaneous and ocular ageing, and in some cases promote progressive neurodegene…

Unexpected occurrence of xeroderma pigmentosum in an uncle and nephew.

BACKGROUND: Xeroderma pigmentosum (XP) is a rare autosomal recessive disorder characterized by a decreased ability to repair DNA damaged by UV radiation and the early development of cutaneous and ocular malignant neoplasms. Approximately 20% of patients with XP also develop progressive neurologic degeneration. OBSERVATIONS: We describe a boy who was found to have XP after a severe burn following minimal sun exposure. His maternal uncle, now age 20 years, had been diagnosed with XP after a similar sunburn in infancy. The uncle has the typical skin pigmentary findings of XP along with severe pr…

Corneal involvement in xeroderma pigmentosum;a histopathologic report.

PURPOSE: To report the histopathologic features of corneal involvement in a patient with xeroderma pigmentosum (XP). CASE REPORT: A 19-year-old man with XP presented with bilateral corneal leukoma and decreased visual acuity predominatly in his right eye. Penetrating keratoplasty was performed in the right eye due to severe corneal opacity, vascularization and lipid deposition. The corneal button underwent histopathologic evaluation which disclosed chronic interstitial lipogranulomatous keratitis. CONCLUSION: To our knowledge, this is the first report of corneal involvement in xeroderma pigm…

Neurological symptoms and natural course of xeroderma pigmentosum.

We have prospectively followed 16 Finnish xeroderma pigmentosum (XP) patients for up to 23 years. Seven patients were assigned by complementation analysis to the group XP-A, two patients to the XP-C group and one patient to the XP-G group. Six of the seven XP-A patients had the identical mutation (Arg228Ter) and the seventh patient had a different mutation (G283A). Further patients were assigned to complementation groups on the basis of their consanguinity to an XP patient with a known complementation group. The first sign of the disease in all the cases was severe sunburn with minimal sun exp…
