Value and mechanisms of EEG reactivity in the prognosis of patients with impaired consciousness: a systematic review.

BACKGROUND: Electroencephalography (EEG) is a well-established tool for assessing brain function that is available at the bedside in the intensive care unit (ICU). This review aims to discuss the relevance of electroencephalographic reactivity (EEG-R) in patients with impaired consciousness and to describe the neurophysiological mechanisms involved. METHODS: We conducted a systematic search of the term "EEG reactivity and coma" using the PubMed database. The search encompassed articles published from inception to March 2018 and produced 202 articles, of which 42 were deemed relevant, assessin

Estrogen Modulates the Sensitivity of Lung Vagal C Fibers in Female Rats Exposed to Intermittent Hypoxia.

Obstructive sleep apnea is mainly characterized by intermittent hypoxia (IH), which is associated with hyperreactive airway diseases and lung inflammation. Sensitization of lung vagal C fibers (LVCFs) induced by inflammatory mediators may play a central role in the pathogenesis of airway hypersensitivity. In females, estrogen interferes with inflammatory signaling pathways that may modulate airway hyperreactivity. In this study, we investigated the effects of IH on the reflex and afferent responses of LVCFs to chemical stimulants and lung inflammation in adult female rats, as well as the role

Peripheral chemoreceptor responsiveness and hypoxic pulmonary vasoconstriction in humans.

OBJECTIVE: Studies in animals have shown that interruption of carotid body afferent hypoxic signaling or efferent CNS activity to the lung enhances hypoxic pulmonary vasoconstriction (HPV). Whether a similar influence of the CNS on HPV strength is present in humans has never been studied, owing to the invasive nature of physical neural ablation or nonspecific systemic effects of pharmacological blockade of putative neural pathways. In order to demonstrate a peripheral chemoreceptor-mediated modulation of HPV in man, we hypothesized that individuals with high hypoxic ventilatory responsiveness,

Role of calcitonin gene-related peptide in inhibitory neurotransmission to the pig bladder neck.

PURPOSE: We studied the role of calcitonin gene-related peptide in nonadrenergic, noncholinergic neurotransmission to the pig bladder neck. MATERIALS AND METHODS: We used immunohistochemical techniques to determine the distribution of calcitonin gene-related peptide immunoreactive fibers as well as organ baths for isometric force recording. We investigated relaxation due to endogenously released or exogenously applied calcitonin gene-related peptide in urothelium denuded phenylephrine precontracted strips treated with guanethidine, atropine and NG-nitro-L-arginine to block noradrenergic neuro

Genetic ablation of delta opioid receptors in nociceptive sensory neurons increases chronic pain and abolishes opioid analgesia.

Opioid receptors are major actors in pain control and are broadly distributed throughout the nervous system. A major challenge in pain research is the identification of key opioid receptor populations within nociceptive pathways, which control physiological and pathological pain. In particular, the respective contribution of peripheral vs. central receptors remains unclear, and it has not been addressed by genetic approaches. To investigate the contribution of peripheral delta opioid receptors in pain control, we created conditional knockout mice where delta receptors are deleted specifically

Synaptic profile of nucleus tractus solitarius neurons involved with the peripheral chemoreflex pathways.

The glomus cells in the carotid bodies (CB) detect alterations in pH and pCO₂ and low pO₂ level in arterial blood. The carotid sinus nerve conveys the information related to the oxygen level to 2nd-order neurons in the nucleus tractus solitarius (NTS) via tractus solitarius (TS), which is part of the chemoreflex pathways. It has been demonstrated that in 2nd-order NTS neurons receiving inputs from the aortic depressor nerve (ADN), the TS stimulation presents high temporal fidelity. However, the temporal properties of synaptic activity in NTS neurons receiving inputs from CB were not yet fully

[Neurogenic bladder: pathophysiology of the disorder of compliance].

Bladder compliance is defined by the ratio of the increase of intravesical pressures to the increase of volume (_V/_P). The pathophysiology of disorders of compliance in neurogenic bladder is still poorly elucidated. It can be evaluated in terms of three elements: 1) The natural history of the appearance of these disorders in neurogenic bladders. Clinical experience shows the existence of prognostic factors that determine the development of these disorders, such as the voiding mode adopted (self-catheterization/hetero-catheterization versus indwelling catheter), the level of the spinal cord le

Regulator of G protein signaling 5 marks peripheral arterial smooth muscle cells and is downregulated in atherosclerotic plaque.

OBJECTIVE: Regulator of G protein signaling 5 (RGS5), an inhibitor of Galpha(q) and Galpha(i) activation, was recently identified among genes highly expressed in smooth muscle cells (SMCs) of aorta but not vena cava. This finding prompted the hypothesis that RGS5 provides long-term G protein inhibition specific to normal arterial SMC populations and that loss of expression may in turn contribute to arterial disease. METHODS: To test this hypothesis we characterized RGS5 gene expression throughout the vasculature of nonhuman primates to determine whether RGS5 was restricted to arteries in othe

Afferent fibers of the pudendal nerve modulate sympathetic neurons controlling the bladder neck.

AIMS: Pudendal nerve stimulation is known to have a potential modulative effect on bladder function. However, even if its efficiency has been established for various neurogenic and non-neurogenic bladder dysfunctions, the underlying neuronal mechanism, and the involved pathways in humans remain unknown. In this prospective study we focused on the effects of pudendal nerve stimulation in complete spinal cord injured patients to identify neuromodulative processes that occur on spinal level. METHODS: Twenty complete spinal male presenting with upper motor neuron lesion and neurogenic incontinenc

Contralateral suppression of transient evoked otoacoustic emissions in patients with cerebello-pontine angle tumor.

OBJECTIVE: During measurement of transient evoked otoacoustic emissions (TEOAEs), acoustic stimulation of the contralateral ear reduces or suppresses TEOAE amplitude. This is thought to be due to the inhibitory control that the medial efferent auditory nerve exerts on outer hair cell (OHC) function. The main aim of this study was to investigate the effect of cerebello-pontine angle (CPA) tumor on the medial efferent nerve pathways to both tumor and non-tumor ears by examining alterations in TEOAE amplitude that result from contralateral acoustic stimulation. DESIGN: Contralateral suppression

[Dopamine as a regulator of natriuresis. Experimental bases and their implications].

Renal effects of dopamine (DA) infused i.v. in a subpressor dose (0.1 microgram. kg.-1 min-1) were investigated in healthy human subjects during steady hypotonic polyuria. In each experiment four clearance (cl.) periods of 15 min were performed; DA was administered during the second and the third cl. periods. The glomerular filtration rate and renal effective plasma flow were estimated as endogenous creatinine and PAH clearances. Moreover the following variables were evaluated: a) sodium total and sodium isosmotic reabsorption as a % of sodium filtered load (s.f.l.); b) sodium anisosmotic reab

[Cerebellar atrophies].

The concept of cerebellar atrophy was first introduced by Pierre Marie in 1893 in his description of hereditary cerebellar ataxia. Subsequent criticism of this notion was refuted by the report of new clinicopathological entities which are compatible with it. The contributions of the Clinic for Diseases of the Nervous System are discussed in this paper. Etiological classification is difficult, in spite of progress made in the genetic, epidemiologic, and biochemical approaches to cerebellar atrophies. Pathologic findings appear to supply the most secure basis for presentation of these affections

Arachidonic Acid as Mechanotransducer of Renin Cell Baroreceptor.

For normal maintenance of blood pressure and blood volume a well-balanced renin-angiotensin-aldosterone system (RAS) is necessary. For this purpose, renin is secreted as the situation demands by the juxtaglomerular cells (also called as granular cells) that are in the walls of the afferent arterioles. Juxtaglomerular cells can sense minute changes in the blood pressure and blood volume and accordingly synthesize, store, and secrete appropriate amounts of renin. Thus, when the blood pressure and blood volume are decreased JGA cells synthesize and secrete higher amounts of renin and when the blo

Towards a nonlinear "neuroimmune gate-model": beyond stellate ganglion block and vagus nerve stimulation.

We propose a three-level neuroimmune gate model to characterize the communication between the nervous system and the immune system. The first gate, Gate 1, is the peripheral neuroimmune gate. Here, immune cells, inflammatory mediators, sensory neurons, autonomic nervous system neurons, blood vessels, and specific tissue components interact. The second gate, Gate 2, is the spinal neuroimmune gate and includes spinal afferents, spinal neurons and circuits, preganglionic sympathetic neurons, and microglia. The third gate, Gate 3, comprises supraspinal structures, including the nucleus tractus sol

The role of gut microbiota in shaping and modulating dietary preferences in neurological diseases: implications for microbiota-targeted interventions.

A high percentage of patients with neurological diseases have significant dietary preference abnormalities (such as picky eating and texture sensitivity). Studies show Gut Microbiota (GM) mediates dietary preferences via the microbiota-gut-brain axis (MGBA), such as depression-related appetite changes linked to gut flora imbalance. GM directly or indirectly regulates dietary behavior through the vagal-mediated gut-brain signaling pathway. On the one hand, gut bacterial metabolites (such as butyrate) can directly activate vagal afferents, which project signals through the nucleus of the solitar

Pairing extinction training with vagus nerve stimulation reduces drug-seeking by altering activity in afferents to the medial prefrontal cortex.

INTRODUCTION: Relapse triggered by drug-associated cues remains a major challenge in treating substance use disorders, as extinction learning is often weak and context dependent. Vagus nerve stimulation (VNS) enhances learning-related plasticity and, when paired with extinction training, reduces cue-induced reinstatement of cocaine seeking; however, the underlying circuit mechanisms remain unclear. METHODS: We examined how VNS paired with extinction reshapes medial prefrontal cortex (mPFC) networks that regulate drug seeking, focusing on afferent inputs from the anterior (aPVT) and posterior

Beyond motor control: The subthalamic nucleus as a central hub for pain in Parkinson's disease.

The subthalamic nucleus (STN), considered a primarily motor structure within the basal ganglia, is recognized as a key contributor to a wider set of behaviors. Among these, nociceptive processing has gained particular attention, especially given the high prevalence and early emergence of pain in Parkinson's disease (PD). This review brings together anatomical, physiological and translational evidence to examine how the STN integrates motor and nociceptive information. The STN receives ascending afferents from brainstem nociceptive pathways and its activity tracks the intensity and salience of

Cholecystokinin Induces Ca2+ Transients in Brainstem Neurons, Astrocytes and Central Vagal Terminals in Jugular/Nodose Ganglia-Brainstem Slice Cultures.

Cholecystokinin is a well-established neuromodulator of brainstem homeostatic circuits, including circuits implicated in satiety and visceral sensation. To better understand the cellular basis of brainstem cholecystokinin signalling, we developed a novel jugular/nodose ganglion-brainstem slice culture, where microdissection isolated bilateral jugular/nodose ganglia attached to a slice of the brainstem via intact vagal rootlets. This preparation preserves the functional vagal afferent pathway to the nucleus of the tractus solitarius and allows for targeted viral transduction of genetically enco

Neuroinflammatory Remodeling by Type 2 Immune Pathways Links Allergic Signaling to Neurodegenerative Disease.

The hallmarks of allergic diseases are Type 2 immunity, including IL-4 and IL-13 production, IgE antibody generation, mast cell and basophil activation, histamine release, and eosinophil activation. There are many routes by which such mediators can influence CNS biology, including cytokine entry or signaling via brain barrier receptors; leukocyte trafficking across activated barriers; cytokine signaling via circumventricular organ sites or dural immune compartments; vagus nerve afferent signaling; mast cell degranulation; and histamine neuromodulation. Neuroinflammation is a common hallmark of
