Endoscopic endonasal management of skull base defects in pediatric patients.

PURPOSE: Skull base defects in children may be the result of congenital anomalies or trauma. They often present as cerebrospinal fluid (CSF) rhinorrhea, meningitis, brain abscess or nasal obstruction. Surgical intervention is predominantly the treatment of choice. Our goal is to assess the efficacy of endoscopic endonasal approach in treating skull base defects in pediatric patients. MATERIAL AND METHODS: In this retrospective study we identified 38 patients (mean age 8.7 ± 5.6 years old, ranging 2 months-18 years) who underwent endoscopic endonasal repair of skull base defects, between March…

Frontoethmoidal mucocele causing proptosis and visual loss.

Mucoceles are benign mucus-containing cysts formed due to the obliteration of sinus ostium. They are most commonly found in the frontal sinus. Mucoceles can spread both intraorbitally and intracranially causing complications such as meningitis, brain abscess, and loss of vision. Radiological investigations are required for diagnosis, with both computed tomography (CT) and magnetic resonance imaging (MRI) being useful. Surgical approaches for management have changed from external to endonasal these days. We report a case where a 54-year-old patient presented to us with a 14 days' history of los…

Oral flora meningoencephalitis diagnosis by next-generation DNA sequencing.

INTRODUCTION: Standard culture methods may fail to detect the causative agents of bacterial infection for various reasons including specimen … Prevotella species and Parvimonas micra . CONCLUSION: Oral flora can cause meningoencephalitis and brain abscess formation if translocation occurs by injury or surgical procedures. Next-generation sequencing is often not available at healthcare facilities, or when available may not have been validated for a wide spectrum of specimen sources, but is available a …

Transcatheter therapy of anomalous systemic venous drainage.

Anomalous drainage of the right superior caval vein into the left atrium is a rare congenital anomaly that causes cyanosis and occult infection owing to right-to-left shunting. Transcatheter management of this anomaly is unique and rarely reported. We report a 32-year-old man with a history of brain abscess, who was diagnosed with an anomalous right superior caval vein draining to the left atrium; right upper pulmonary vein and right middle pulmonary vein draining into the inferior portion of the right superior caval vein; and a left superior caval vein draining into the right atrium through t…

Myelosuppression-sparing treatment of central nervous system nocardiosis in a multiple myeloma patient utilizing a tedizolid-based regimen: a case report.

Central nervous system (CNS) nocardiosis is a recognised opportunistic infection in immunocompromised patients. Treatment involves prolonged institution of antibiotics, making oral agents a convenient and desired option. Unfortunately, devising an effective, well-tolerated antimicrobial for the duration required to treat CNS nocardiosis is challenging owing to treatment intolerance and toxicities. This report highlights myelosuppression-sparing treatment with an oral tedizolid-based regimen following a complicated course with standard agents. A 68-year-old female from Florida (USA) with low-ri

Clinical and MRI findings in three dogs with polycystic meningiomas.

One spayed female Labrador retriever and two castrated male golden retrievers were evaluated for chronic (i.e., ranging from 3 wk to 24 wk) neurologic signs localizable to the prosencephalon. Signs included seizures, circling, and behavior changes. MRI demonstrated extra-axial, contrast-enhancing, multiloculated, fluid-filled, cyst-like lesions with a mass effect, causing compression and displacement of brain parenchyma. Differential diagnoses included cystic neoplasm, abscess or other infectious cyst (e.g., alveolar hydatid cyst), or fluid-filled anomaly (e.g., arachnoid cyst). The cyst-like

Clinical characteristics and epidemiology of pulmonary pseudallescheriasis.

BACKGROUND: Some members of the Pseudallescheria (anamorph Scedosporium) have emerged as an important cause of life-threatening infections in humans. These fungi may reach the lungs and bronchial tree causing a wide range of manifestations, from colonization of airways to deep pulmonary infections. Frequently, they may also disseminate to other organs, with a predilection for the brain. In otherwise healthy patients, the infection is characterized by non-invasive type involvement, while invasive and/or disseminated infections were mostly seen in immunocompromised patients. AIMS: We reviewed a

[A case of the syndrome of the sinking skin flap: case report].

A case of "the syndrome of the sinking skin flap" was presented. A 40-year-old-man had suffered from severe SAH 9 months before. An aneurysm of the anterior communicating artery was successfully clipped and the bone flap was removed for the purpose of the external decompression. Cranioplasty and V-P shunt were performed 1 month after SAH, but both were removed because of postoperative wound infection, viz. epidural and subdural abscess 4 months after SAH. Following this, L-P shunt was performed, and the patient was discharged with mild dementia. A concave deformity of the skin flap developed a

Disseminated Nocardia farcinica Infection Presenting as Multiple Intramuscular Abscesses Mimicking Skeletal Muscle Metastases in a Patient With Hepatocellular Carcinoma.

A 67-year-old man with hepatocellular carcinoma and a previously resected pulmonary metastasis was receiving prednisolone for immune-related interstitial nephritis. Two months after pulmonary metastasectomy, multiple pulmonary nodules and painful intramuscular masses developed in the right forearm, left gluteus medius, and left obturator internus. The lesions were initially interpreted as metastases, and palliative radiotherapy was initiated. At the initial radiation oncology consultation, the patient was afebrile but had right forearm edema, neutrophilic leukocytosis, and an elevated C-reacti

Hidradenitis Suppurativa Burdens on Mental Health: A Literature Review of Associated Psychiatric Disorders and Their Pathogenesis.

Skin, mental health and the central nervous system (CNS) are connected by a deep link. It is not only the aesthetic and sometimes the disfiguring aspects of dermatological conditions that can cause a severe psychological burden; also, different studies have shown how chronic skin-inflammatory diseases may influence the activity of the CNS and vice versa. Moreover, the skin and brain share a common embryogenic origin. Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease affecting the hair follicles of the apocrine regions. The main clinical features are nodules, abscesses, cysts

Chapter 2: mesopotamia.

No treatise of ancient Mesopotamian clinical neurology has yet been written, although much groundbreaking work on medical or medically-related texts from this area (modern Iraq) has been performed in recent years. All through Mesopotamian civilization (c. 3000-100 BC), literary and erudite knowledge, recorded on clay tablets in cuneiform script, thrived; within the scholarly collections of medical texts, signs and symptoms of many afflictions were referred back to external agents entering in contact with the patient's body, albeit invisibly. This external etiological sphere pertained to the su

Progressive multifocal leukoencephalopathy (PML) and cerebral toxoplasmosis in an adult patient, with no symptoms of underlying immunosuppressing illness.

We present a case of the coincidence of progressive multifocal leukoencephalopathy (PML) and central nervous system (CNS) toxoplasmosis in an adult patient, without a detectable cause of cell-mediated immunity impairment. The proper diagnosis was made postmortem on the basis of histological changes typical of both pathological processes. PML was characterized by the presence of subcortical focal demyelination, containing enlarged, densely basophilic oligodendrocyte nuclei, often with intranuclear inclusion, and bizarre astrocytes, mimicking neoplastic cells. PML was confirmed by detecting nume
