Phemister epiphysiodesis in leg length inequality treatment--evaluative outcome analysis.

BACKGROUND: The aim of this study was to evaluate the efficacy of Phemister epiphysiodesis for leg length inequality (LLI) treatment in comparison with epiphysiodesis techniques currently in use and to determine factors that may influence outcomes. MATERIALS AND METHODS: Nineteen girls and sixteen boys with a mean skeletal age of 12.2 years and a mean functional LLI of 3.8 cm at the time of surgery were enrolled. Data were retrieved from hospital records and bone ages were determined using standard atlases. Mean follow-up duration was 7 years. RESULTS: Good results (final LLI < 1.5 cm) were

[Clinical-epidemiologic characteristics of large, invasive basal cell carcinomas of the scalp].

We studied a large, invasive basal cell carcinoma of the scalp in 42 operated patients (53 tumors) of which 26 patients with recurrent and 16 with no recurrence. This is a rare clinical form of this tumor that invades the skin in addition to some or all of the following structure: soft tissue scalp, skull, brain and brain membranes. The literature is known as the Giant basal cell carcinoma according to TNM classification of BCC in pT4 stage. Caused by aggressive subtypes of BCC. Treatment is surgical and involves only large, mutilate surgery and extensive reconstruction. Relapse often occurs.

Titanium mesh strut: a novel instrument for firm elevation of the reconstructed auricle.

UNLABELLED: Nagata's two-stage method for ear reconstruction of congenital microtia is widely used. The support materials used for the reconstructed cephaloauricular angle in the second stage have been studied by several surgeons. The cartilage wedge block has been popular for firm elevation of ear reconstruction. However, the costal cartilage harvested from the chest wall might be insufficient or unavailable and the fixed cartilage wedge might be absorbed or slip forward or backward resulting in an unstable cephaloauricular angle. To develop a more perfect strut for the reconstructed ear, we

Safety and feasibility of balloon sinuplasty for treatment of chronic rhinosinusitis in children.

OBJECTIVES: Balloon sinuplasty is a new technique that was recently introduced for the treatment of chronic rhinosinusitis (CRS). The initial experience in adults has been promising. The technique allows for restoring ventilation to the sinuses with minimal risk and trauma to the tissues. I present our initial experience of its use for treatment of CRS in children. METHODS: I performed a prospective study of 30 children in whom medical therapy failed and who were scheduled for surgery. They were offered treatment with balloon sinuplasty of selected sinuses. The data collected included age, co

Relationship between anteroposterior cranial vault deformation and mandibular morphology in a pre-Columbian population.

INTRODUCTION: An externally applied force to the cranial vault has been reported to affect the growth of the facial skeleton. However, the effect on the mandible is unclear. The purpose of this study was to investigate the relationship between anteroposterior cranial vault deformation and mandibular morphology. METHODS: The study sample included 44 women's crania with intact faces and bases that were excavated from archaeological sites in the Azapa Valley in northern Chile. The crania were divided into anteroposterior deformation (AP) and undeformed (U) groups according to frontal, parietal,

A positioning device for computed tomography: a clinical report.

This clinical report describes a device (Centrascan) used to assist in the correct alignment of the patient's head during computed tomography (CT) assessment of a proposed implant site. To obtain the desired anatomic detail, CT requires precise alignment of the axial images at right angles to the long axis of the proposed implant. This clinical report compared the anatomic morphology of a projected implant site derived from axial images provided by DentaScan software analysis of the CT scan acquisition. Images from a conventional scan, with the patient's head aligned along the frontal plane (p

Surgical treatment of fibrous dysplasia of the skull in children.

INTRODUCTION: We evaluate the role of surgery in the treatment of fibrous dysplasia of the skull in children. METHODS: We identified 48 consecutive cases of fibrous dysplasia of the skull that were surgically treated at a single institution over a 23-year interval. The 28 patients that initially presented during the first two decades of life were selected for further analysis. Presenting symptoms, signs, surgical treatment, surgical outcome and the state of the disease at the extended follow-up interval were recorded. RESULTS: Fibrous dysplasia of the skull in children most often involved th

Assessment and treatment of facial deformity resulting from radiation to the orbital area in childhood.

Children who receive radiation for malignant tumors in the orbital area frequently develop widespread craniofacial deformities. These affect the skull, orbit, maxilla, and mandible. When these patients seek treatment at a later age, they require careful assessment using cephalometrics and three-dimensional imaging. It is recommended that the four levels of skeletal deformity be corrected in a single procedure, that is frontotemporal expansion with repositioning of the skull base area, orbital expansion and repositioning together with maxillary and mandibular surgery. Bone grafts should be inla

Effects on sound localization of configuration and type of hearing impairment.

Localization ability of 87 bilaterally hearing-impaired listeners was tested in the horizontal and vertical planes, frontally and laterally. In those with sensorineural hearing loss, it was found that deficits in localization accuracy in different regions of auditory space could be related to different configurations of hearing loss. For example, there were associations between vertical plane discrimination and high-frequency sensitivity; and front-rear discrimination and mid-to-high-frequency sensitivity. These results agree with theoretical expectations, while the outcome overall contrasts w

The terminal nerve of dolphins: gross structure, histology and luteinizing-hormone-releasing hormone immunocytochemistry.

The terminal nerve (TN) of several dolphins was examined using gross dissection aided by osmium staining, routine light and electron microscopy, and immunocytochemistry with antibodies to mammalian luteinizing-hormone-releasing hormone (LHRH). The TN consists of numerous pial strands which emanate from large paired ganglia located in the dura near the frontal lobe of the hemisphere. The strands are largely composed of myelinated axons which extend to basal forebrain areas including the anterior perforated substance. Peripheral branches of the ganglia run through foramina in the ethmoid bone in

Is bone turnover of jawbone and its possible over suppression by bisphosphonates of etiologic importance in pathogenesis of bisphosphonate-related osteonecrosis?

PURPOSE: The pathogenesis of bisphosphonate-related osteonecrosis of the jaw (BRONJ) is not completely understood. The most popular hypothesis has suggested that the bone turnover (BT) in the jawbone is greater than that in other sites and that this turnover will be overly suppressed by bisphosphonates. Using bone scintigraphy, a simple tool for the quantitative evaluation of bone metabolism and blood flow, the goals of the present study were to determine whether the rate of bone remodeling is greater in the jaw and whether the bone BT in the jaw is differentially altered after bisphosphonate

Association between hip abductor muscle strength and mobility limitation (locally defined as locomotive syndrome) in community-dwelling adults undergoing osteoporosis screening: a cross-sectional study.

BACKGROUND: Mobility limitation is associated with an increased risk of disability in older adults. In Japan, mobility limitation is commonly assessed using the concept of locomotive syndrome. Hip abductor muscle strength plays a key role in gait stability; however, its independent association with mobility limitation remains unclear, partly because previous studies have focused primarily on general muscle strength or muscle mass rather than frontal-plane hip stability. This study aimed to investigate the association between hip abductor muscle strength and mobility limitation in community-dwe

Predicting Knee Joint Instability Using a Tibio-Femoral Statistical Shape Model.

Statistical shape models (SSMs) are a well established computational technique to represent the morphological variability spread in a set of matching surfaces by means of compact descriptive quantities, traditionally called "modes of variation" (MoVs). SSMs of bony surfaces have been proposed in biomechanics and orthopedic clinics to investigate the relation between bone shape and joint biomechanics. In this work, an SSM of the tibio-femoral joint has been developed to elucidate the relation between MoVs and bone angular deformities causing knee instability. The SSM was built using 99 bony sha

Fragile and Brittle Bone Disease or Osteogenesis Imperfecta: A Case Report.

AIM AND OBJECTIVE: The aim and objective of this report is to describe the dental management of 11-year-old patient with type III osteogenesis imperfecta (OI). BACKGROUND: Osteogenesis imperfecta or brittle bone disease is caused by mutations in the collegen type I gene which is a heterogeneous rare connective tissue disorder. Dentinogenesis imperfecta, hearing impairment, scoliosis, sclera is blue, hyperlaxity of ligaments, and fragile skin are other common features. Individuals having positive family history suggest a straightforward diagnosis of OI but can be difficult in the absence of af

Application of the Bracing System in Reduction Malarplasty in Asian Population.

BACKGROUND: In the East, a broad and prominent malar is considered to express an aggressive and unpleasing impression; therefore, patients seek to improve their appearance through malar reduction. Although most of the techniques have been greatly improved, still there are some pitfalls in the form of cheek sagging or bone nonunion. In this study, we performed a reduction malarplasty using a firm bracing system to minimize major postoperative complications. METHOD: This was a retrospective study evaluating the results of a total of 157 patients (139 women and 18 men) who underwent reduction ma

Brain abscess in a patient with psoriatic arthritis treated with adalimumab: A case report.

RATIONALE: In patients receiving biological therapies, serious infections are a major concern. Infections associated with anti-tumor necrosis factor antibody therapy include tuberculosis, viral, fungal, and bacterial infections. Likewise, severe infections of the upper and lower respiratory tract, lung, skin and soft tissue, urinary tract, gastrointestinal tract, joint, and bone have also been reported previously. However, infections involving the central nervous system are rare, especially an intracranial infection caused by odontogenic infection. To date, only few cases have been reported of
