A hypothesis on paradoxical privileged portal vein metastasis of hepatocellular carcinoma. Can organ evolution shed light on patterns of human pathology, and vice versa?

Unlike other carcinomas, hepatocellular carcinoma (HCC) metastasizes to distant organs relatively rarely. In contrast, it routinely metastasizes to liver vasculature/liver, affecting portal veins 3-10 times more often than hepatic veins. This portal metastatic predominance is traditionally rationalized within the model of a reverse portal flow, due to accompanying liver cirrhosis. However, this intuitive model is not coherent with facts: 1) reverse portal flow occurs in fewer than 10% of cirrhotic patients, while portal metastasis occurs in 30-100% of HCC cases, and 2) portal vein prevalence o

The corrected donor age for hepatitis C virus-infected liver transplant recipients.

Donor age has become the dominant donor factor used to predict graft failure (GF) after liver transplantation (LT) in hepatitis C virus (HCV) recipients. The purpose of this study was to develop and validate a model of corrected donor age (CDA) for HCV LT recipients that transforms the risk of other donor factors into the scale of donor age. We analyzed all first LT recipients with HCV in the United Network for Organ Sharing (UNOS) registry from January 1998 to December 2007 (development cohort, n = 14,538) and January 2008 to December 2011 (validation cohort, n = 7502) using Cox regression, e

How much of the difference in life expectancy between Scottish cities does deprivation explain?

BACKGROUND: Glasgow's low life expectancy and high levels of deprivation are well documented. Studies comparing Glasgow to similarly deprived cities in England suggest an excess of deaths in Glasgow that cannot be accounted for by deprivation. Within Scotland comparisons are more equivocal suggesting deprivation could explain Glasgow's excess mortality. Few studies have used life expectancy, an intuitive measure that quantifies the between-city difference in years. This study aimed to use the most up-to-date data to compare Glasgow to other Scottish cities and to (i) evaluate whether deprivati

Meta-analysis of variables affecting mouse protection efficacy of whole organism Brucella vaccines and vaccine candidates.

BACKGROUND: Vaccine protection investigation includes three processes: vaccination, pathogen challenge, and vaccine protection efficacy assessment. Many variables can affect the results of vaccine protection. Brucella, a genus of facultative intracellular bacteria, is the etiologic agent of brucellosis in humans and multiple animal species. Extensive research has been conducted in developing effective live attenuated Brucella vaccines. We hypothesized that some variables play a more important role than others in determining vaccine protective efficacy. Using Brucella vaccines and vaccine candi

Back pain's association with vertebral end-plate signal changes in sciatica.

BACKGROUND CONTEXT: Patients with sciatica frequently experience disabling back pain. One of the proposed causes for back pain is vertebral end-plate signal changes (VESC) as visualized by magnetic resonance imaging (MRI). PURPOSE: To report on VESC findings, changes of VESC findings over time, and the correlation between VESC and disabling back pain in patients with sciatica. STUDY DESIGN/SETTING: A randomized clinical trial with 1 year of follow-up. PATIENTS SAMPLE: Patients with 6 to 12 weeks of sciatica who participated in a multicenter, randomized clinical trial comparing an early surg

Prospects of robotic thyroidectomy using a gasless, transaxillary approach for the management of thyroid carcinoma.

PURPOSE: Robotic surgical systems are among the most innovative surgical developments and have radically promoted the use of minimally invasive techniques. Robotic technologies using different approaches have also been applied to thyroid surgery. Recently, the authors described a novel robotic surgical method for thyroid surgery based on a gasless, transaxillary approach (TAA), and have since serially reported on its technical feasibility and safety. Here, the authors report their experience of a consecutive series of 1000 cases treated using robotic thyroidectomy, and demonstrate its use for

"Virtual flow cytometry" of immunostained lymphocytes on microscopic tissue slides: iHCFlow tissue cytometry.

BACKGROUND: A method and approach is developed for fully automated measurements of immunostained lymphocytes in tissue sections by means of digital color microscopy and patent pending advanced cell analysis. The validation data for population statistic measurements of immunostained lymphocytes in tissue sections using tissue cytometry (TC) is presented. The report is the first to describe the conversion of immunohistochemistry (IHC) data to a flow cytometry-like two parameter dot-plot display, hence the technique is also a virtual flow cytometry. We believe this approach is a paradigm shift, a

Phylogeny of Taenia: Species definitions and origins of human parasites.

Phylogeny is fundamental as it constrains explanations about history and forms our foundation for recognizing and diagnosing species. In the absence of such a framework taxonomists historically relied on intuitive processes, personal judgment and authority, often embracing a typological view of species that disregarded otherwise unequivocal historical and biological criteria. Species of Taenia are among the most characteristic tapeworms infecting carnivores and humans as definitive hosts and indeed Taeniidae is unique among the Eucestoda in requiring 2 obligate mammalian hosts for transmission

Development of a novel digestion chamber for human and porcine islet isolation.

BACKGROUND: The current technique of human pancreas digestion for islet isolation relies on selective distribution of collagenase delivered via the pancreatic duct to produce digestion and removal of peri-acinar fibrous tissue. However, the collagenase has relatively little effect on the interlobular fibrous tissue, which must therefore be broken down by mechanical means within the digestion chamber so as to release the contained acini and islets. The current way of achieving this in the Ricordi chamber is to place five or six stainless steel balls within the chamber and shake vigorously. The

Does the combination of two BMD measurements improve fracture discrimination?

UNLABELLED: Combining information from different types of BMD measurement should improve the evaluation of patients' risk of fracture. This study used a bivariate gaussian model to examine the effect of combining two different BMD measurements. The results show that, in practice, there is little benefit unless the measurements are completely unrelated. INTRODUCTION: Intuitively, the combination of information from two or more different types of bone densitometry investigation should improve our ability to identify patients at high risk of fracture. However, the best way to combine measurement

In nosocomial pneumonia, optimizing antibiotics other than aminoglycosides is a more important determinant of successful clinical outcome, and a better means of avoiding resistance.

In in vitro and animal models, antibiotics show good relationships between concentration and response, when response is quantified as the rate of bacterial eradication. The strength of these in vitro relationships promises their utility for dosage regimen design and predictable cure of infections such as nosocomial pneumonia. In spite of their intuitive logic, close relationships between dosage and bacterial eradication have not been easy to show in clinical studies of nosocomial pneumonia. Presumably, a variety of patient, disease, bacterial, and pharmacokinetic variables cloud these relation

Cold Water Immersion: Simultaneous Assessment of Cerebral Oxygenation, Vascular Function, and Thermoregulatory Responses.

INTRODUCTION: Developing a greater understanding of how the body reacts following cold water immersion (CWI) and its impact on human performance is required for the development of future monitoring technologies and countermeasures to reduce cold-induced impairments. Currently, there are no field-deployable technologies for real-time monitoring of cold-induced neurochemical and metabolic changes, limiting physiological assessment tools for monitoring the impact of cold immersion on cognitive and physical performance. Functional near-infrared spectroscopy (fNIRS) may be an effective technique fo

Designing an mHealth App to Encourage Uptake of Muscle-Strengthening Exercise in Older Adults: Co-Design Focus Group Study.

BACKGROUND: Sarcopenia, the age-related decline in muscle mass and strength, poses a significant threat to functional independence in older adults. Despite strong evidence supporting resistance training as a preventive and therapeutic strategy, adherence to muscle-strengthening guidelines remains low. Mobile health (mHealth) technologies offer a promising avenue to bridge this gap; however, few apps are tailored to older adults or designed with their input. OBJECTIVE: This study aimed to identify key features that a muscle-strengthening exercise app should include to enhance engagement and up

An applied study on the assessment of concomitant pulmonary hypertension in patients with chronic obstructive pulmonary disease based on whole-lung CT imaging histologic features.

BACKGROUND: Pulmonary hypertension (PH) is a common complication in the progression of chronic obstructive pulmonary disease (COPD) and plays a pivotal role in the advancement of chronic pulmonary heart disease. This condition is closely associated with acute exacerbations and poor prognosis in COPD patients. Currently, methods used in clinical practice to assess PH each have certain limitations. This study aimed to investigate the diagnostic value of whole-lung computed tomography (CT)-based Radiomics features in identifying PH in patients with COPD. METHODS: A total of 171 patients diagnose

Assessing racial disparities in healthcare expenditure using generalized propensity score weighting.

PURPOSE: This paper extends current propensity score weighting methods for causal inference to better understand disparities in healthcare access across multiple racial groups. By treating each racial group as a distinct entity (or "treatment") in the causal inference framework, we can assess and evaluate heterogeneity in healthcare outcomes across various racial or ethnic categories. Furthermore, we leverage modern propensity score weighting techniques to address the challenges inherent to multiple group evaluations, such as violations of the positivity assumption, and compare the performance

Development and externally validated prediction model of individualization of FSH starting dose in the depot GnRH agonist protocol for the early follicular phase.

BACKGROUND: Each controlled ovarian hyperstimulation(COH) protocol has its own unique mechanism and hormone pattern. The depot GnRHa protocol has a deeper down-regulation effect and favorable clinical pregnancy rates. The predictive model of the optimal follicle-stimulating hormone (FSH) starting dose in the early follicular phase depot GnRH agonist (EFDGa) protocol has not been reported. Our study was made to explore predictive indicators for determining the optimal FSH starting dose in patients undergoing ovarian stimulation with the EFDGa protocol in assisted reproductive technology (ART),
