Intussusception in children under 2 years of age in the State of Qatar : analysis of 67 cases.

Intussusception is one of the leading causes of bowel obstruction in early infancy and childhood. From 1984-1989, 67 patients under 2 years of age with intussusception were diagnosed and treated in our institution. There were 48 boys and 19 girls ranging in age from 2 months to 2 years with a mean of 7.4 months. Presenting symptoms and signs included abdominal pain (96%), vomiting (93%), rectal bleeding (60%) and a palpable mass (67%). Symptoms and signs were present for less than 24 hours in about 80% of cases. Most of the intussusceptions were of the ileocolic type (75%). The overall success

Clinical application of wireless capsule endoscopy in pediatric patients for suspected small bowel diseases.

Capsule endoscopy (CE) has been demonstrated to be safe and well tolerated in adults with suspicion of small intestinal diseases with negati … CE was positive in 12 patients, including Crohn's disease (4), hemangioma (2), angiodysplasia (2), Meckel diverticulum (1), polyp (1), aphthous ulcer in ascending colon (1), and cobblestone appearance of ileal mucosa. CE has been performed safely in a small series of pediatric patients after ingestion or endoscopic placement of the capsule. The high yield of abnorma …

Problems in family practice. Rectal bleeding.

The diagnosis and management of rectal bleeding problems varies with the urgency of the situation, the age of the patient, and the applicability of available diagnostic methods. Every instance of rectal bleeding is a problem that demands investigation by endoscopic, radiographic, and laboratory means. A physician can be misled by the patient's understatement or underobservance of bleeding. A good history obtained as quickly as possible in urgent circumstances and in great detail under more relaxed circumstances is of immeasurable diagnostic value.

Management of Littre Hernia-Case Report and Systematic Review of Case Reports.

Littre hernia is a rare type of hernia in which a Meckel diverticulum is found in the hernia sac. Given the rare nature of this disease, little data on demographics and surgical management exists. In this article, we provide a case report of a strangulated inguinal Littre hernia and perform a systematic review of the literature. The PubMed database was searched on 5 March 2022, and all cases of Littre hernia in adults that had English abstracts or full-text were analyzed. Our primary objective was to evaluate the surgical management and outcomes of this particular type of hernia, and our secon…

Giant lipoma of the small bowel associated with perforated ileal diverticulum.

The authors present a rare case of the middle aged man who presented with a typical history and clinical findings suggestive of an acute appendicitis. After initial assessment, he required a surgical intervention. Operation findings revealed a perforated ileal diverticulum into a giant lipoma of the small bowel. The patient made a good postoperative recovery.

[Application of double balloon enteroscopy in pediatric patients].

OBJECTIVE: Double balloon enteroscopy (DBE) is already an established modality of investigation in adults though its use in children has not yet been widely practiced. This study aimed to explore the method and safety of pediatric DBE and to evaluate its clinical value and applicability in the pediatric age group. METHOD: During the period from December, 2006 to October, 2009 DBE was performed on a total of 22 patients (age 4 to 16 years) at the hospital. The indications included: gastrointestinal bleeding, chronic abdominal pain, suspected Crohn's disease and hypoalbuminemia. With the aid of

Pneumatosis intestinalis. Appearance on MR examination.

We describe the magnetic resonance (MR) appearance of pneumatosis intestinalis in two patients. Pneumatosis is suggested by the finding of circumferential collections of air adherent or within the bowel wall. This air is more apparent on gradient-echo images due to "blooming" associated with magnetic field inhomogeneities at air-tissue interfaces. While MR is not routinely indicated as a diagnostic tool for the detection of pneumatosis, the radiologist should nevertheless be familiar with its appearance on MR examination.

Specialist investigation of obscure gastrointestinal bleeding.

The investigation and treatment of 131 patients with 'obscure' gastrointestinal bleeding has been reviewed. One hundred and six patients were assessed electively for recurrent haemorrhage, 25 presented as emergencies. The major presenting feature was melaena (55 patients), anaemia (35), rectal bleeding (34), haematemesis (six) and ileostomy bleeding (one). The lesions responsible for haemorrhage were colonic angiodysplasia (52 patients), small bowel vascular anomalies (16), Meckel's diverticula (nine), small bowel smooth muscle tumours (seven), gastric vascular anomalies (four), chronic pancre

Technetium-99m-labelled red blood cell scanning to diagnose occult gastrointestinal bleeding.

The authors, from their experience in 60 patients, demonstrate that technetium-99m-labelled red blood cell scintigraphy is a sensitive (91%) and specific (76%) diagnostic procedure with a high predictive value, when both positive (81%) and negative (92%). It is preferable to and more cost-effective than selective angiography as a first-line diagnostic test for occult gastrointestinal bleeding.

Obscure gastrointestinal haemorrhage of small-bowel origin.

The records of 37 patients investigated for obscure gastrointestinal haemorrhage originating from the small bowel were reviewed retrospectively. Bleeding was caused by Meckel's diverticula in eight cases, smooth muscle tumours in seven cases, vascular anomalies in 14 cases, and other single lesions in eight cases. The lesions were identified by angiography in 18 patients, operation in 17, and barium follow through examination in two. Only one patient under 50 years of age had a lesion that would not have been found by careful laparotomy. Early laparotomy is advisable in patients aged under 50

Visualization of a Y-configuration and peristalsis on prenatal sonography: The pathognomonic sign of an ileal duplication cyst.

OBJECTIVE: Enteric duplication cysts are a rare congenital malformation that consists of a cystic formation adjacent to the native gastrointestinal tract. A double wall appearance on fetal abdominal sonography is highly suggestive of its diagnosis. However, this sign also can be seen in an ovarian cyst, a mesenteric cyst, and a Meckel's diverticulum. To the best of our knowledge, this case is the first report of accurate prenatal diagnosis based upon the presence of a Y-configuration and peristalsis on prenatal sonography. We herein present prenatal and postnatal images including ultrasonogram

CT Changed the Diagnosis: Temporal Trends in Recognition and Management of Fish Bone-Induced Gastrointestinal Perforation.

Fish bone perforation of the gastrointestinal tract is rare and frequently misdiagnosed-not because it is inherently difficult to recognize, but because it is not considered. Symptoms are nonspecific, patients may not recall ingestion, and the condition can mimic acute abdomen. We performed a systematic review of published case reports and case series with individual patient-level data extraction; 220 patients from 199 publications were included. The small bowel, including Meckel's diverticulum, was the most frequent perforation site (46.3%), followed by the colorectum (22.9%) and upper gastro

Ultrasound guided hydrostatic enema reduction of ileocolic intussusception: a safe and effective technique.

BACKGROUND: Currently, the primary management of ileocolic intussusception in children is usually by non-operative image-guided enema reduction. In most centres around the world especially in Australasia the predominant technique is the pneumatic reduction under fluoroscopic guidance. At our institution, we have been performing ultrasound-guided hydrostatic reduction since 2012.This is an audit to determine the efficacy and safety of ultrasound-guided hydrostatic reduction for intussusception. METHODS: Following ethics approval, a retrospective review of all patients presenting to our institu
