AIM: A subgroup of children who present with motor stereotypies in the context of episodes of intense imagery have recently been described in the literature,(1) termed Intense Imagery Movements (IIM). All children report conscious engagement in acts of imagery or imagination, with stereotyped movements occurring simultaneously with limited conscious awareness. This article reports preliminary cognitive data to inform clinical management and guide future research.
METHOD: Intellectual functioning was assessed for ten children with IIM (7 boys, 3 girls; mean age = 10;01, age range = 6;06 to 14;

BACKGROUND: In euthymic bipolar disorder patients, scores on the Mini-Mental State Examination (MMSE) are not abnormal but general functioning remains impaired. Recent studies provide ample evidence that euthymic patients show significant impairment on more comprehensive neuropsychological test batteries. There is no definitive relationship between performance on neuropsychological test batteries and the ability to cope in everyday life. Ecologically valid tests of cognitive function aim to bridge this gap as they use everyday tasks to explore cognitive function. The aims of the study were to

An international, prospective, multicentre trial (AIDA) was undertaken from October 1995 to March 1997. The object was to compare the diagnosis of atrial arrhythmias (AA) by the automatic interpretation of the memory functions of dual chamber pacemakers with that of 24 hour Holter monitoring at day 1. The second objective was to assess the incidence and symptomatology of the AA during follow-up at Day 28. In France, 226 patients implanted with Chorus, Chorus II and Chorus RM pacemakers were included in the study (148 men, 70.5 +/- 10.8 years) for the following indications: AVB (atrioventricula

A new type of seafood toxicity, called amnesic shellfish poisoning, was described from 107 human cases after individuals consumed mussels containing domoic acid harvested from Prince Edward Island, Canada, in 1987. Most of these cases experienced gastroenteritis, and many older persons or others with underlying chronic illnesses developed neurologic symptoms including memory loss. Standard treatment procedures for the neurologic condition were not effective and three patients died. Domoic acid is a known neurototoxin, and it is believed that in these cases enough toxin was absorbed through the

INTRODUCTION: RNU4ATAC-opathy is a rare autosomal recessive disorder characterized by a wide clinical spectrum, including brain anomalies, seizures, strokes, immunodeficiency, and cardiac defects. Other manifestations include ophthalmologic, dermatologic, renal, gastrointestinal, auditory, and endocrine involvement. Here, we report the identification of pathogenic RNU4ATAC variants through whole exome sequencing in an infant presenting with microcephaly, growth failure, and immunodeficiency.
CASE PRESENTATION: We describe an 11-month-old infant who had been followed since 4 months of age for

T cells have been shown to maintain a lower percentage (heteroplasmy) of the pathogenic m.3243A>G variant (MT-TL1, associated with maternally inherited diabetes and deafness [MIDD] and mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes [MELAS]). The mechanism(s) underlying this purifying selection, however, remain unknown. Here we report that purified patient memory CD4+ T cells have lower bulk m.3243A>G heteroplasmy compared to naïve CD4+ T cells. In vitro activation of naïve CD4+ m.3243A>G patient T cells results in lower bulk m.3243A>G heteroplasmy after prolifera

BACKGROUND: Colloid cysts of the posterior third ventricle are exceedingly rare. This location is a high-risk zone for colloid cysts because of potential obstruction of the cerebral aqueduct.
OBSERVATIONS: The authors report a case of a 57-year-old man who presented with a 6-month history of progressive headache, short-term memory loss, visual blurring, and an episode of double vision. Magnetic resonance imaging (MRI) revealed a colloid cyst, 22 mm in diameter, attached to the roof of the posterior third ventricle. The posterior third ventricle was obstructed, but both foramina of Monro were

Skin is a relatively uncommon site of metastasis in lung cancer and is associated with a poor prognosis. Although, lung cancer does not uncommonly metastasise to the brain, haemorrhagic brain metastases are rarely reported. In this report, we present a dramatic presentation of a female smoker with a 3-week history of numerous cutaneous lesions over her body and two episodes of transient memory loss. Work-up demonstrated widely metastatic, poorly differentiated lung adenocarcinoma with haemorrhagic brain metastases. She proceeded with whole brain radiotherapy, but her performance status quickly

BACKGROUND: Gamma Knife ventral anterior capsulotomy is an effective option to treat refractory obsessive-compulsive disorder. Although well tolerated, complications can develop years after radiosurgery. We describe a case in which abnormal complications induced by very high doses of radiation evolved.
CASE DESCRIPTION: A 55-year-old man with refractory obsessive-compulsive disorder was treated with Gamma Knife ventral anterior capsulotomy using a dose of 180 Gy. His obsessive-compulsive symptoms improved, but his condition evolved with a manic episode, cognitive memory changes, visual halluc

Transient epileptic amnesia (TEA) is a distinct syndrome affecting middle-aged persons without concurrent brain disease or disposition to epileptic seizures. Seizures are characterized by amnesia, usually lasting less than one hour, and interictal memory deficits that are common. Effective antiseizure treatment is usually rapid in patients with TEA, which underlines the need for prompt diagnosis. Here, we report a 58-year-old male patient with recurrent episodes of antero- and retrograde amnesia. MRI was normal and diagnosis was made using long-term EEG (27 hours), revealing 10 right-sided tem

Cognitive dysfunction is a symptomatic domain identified across many mental disorders. Cognitive deficits in individuals with major depressive disorder (MDD) contribute significantly to occupational and functional disability. Notably, cognitive subdomains such as learning and memory, executive functioning, processing speed, and attention and concentration are significantly impaired during, and between, episodes in individuals with MDD. Most antidepressants have not been developed and/or evaluated for their ability to directly and independently ameliorate cognitive deficits. Multiple interactin

Obstructive Sleep Apnea (OSA) is characterized by repetitive episodes of airflow reduction (hypopnea) or cessation (apnea), which, as a prevalent sleep disorder, can cause people to stop breathing for 10 to 30 seconds at a time and lead to serious problems such as daytime fatigue, impaired memory, and depression. This work intends to explore automatic detection of OSA events with 1-second annotation based on blood oxygen saturation, oronasal airflow, and ribcage and abdomen movements. Deep Learning (DL) technology, specifically, Convolutional Neural Network (CNN), is employed as a feature dete

Rhythmic theta frequency (~5-12 Hz) oscillations coordinate neuronal synchrony and higher frequency oscillations across the cortex. Spatial navigation and context-dependent episodic memories are represented in several interconnected regions including the hippocampal and entorhinal cortices, but the cellular mechanisms for their dynamic coupling remain to be defined. Using monosynaptically-restricted retrograde viral tracing in mice, we identified a subcortical GABAergic input from the medial septum that terminated in the entorhinal cortex, with collaterals innervating the dorsal presubiculum.

We report a case of increased values of entropy parameters Response Entropy (RE) and State Entropy (SE) during intravenous general anesthesia in a sleepwalking patient. An ASA class II, 64-year-old woman with stress incontinence underwent mid-urethral sling surgery. Prior to surgery, the patient had been administered paroxetine, valproic acid and clonazepam for the treatment of sleepwalking disorder. After 10 min of target-controlled infusion of propofol and remifentanil, entropy values increased up to 94 (RE) and 88 (SE) for 10 min. The target effect-site concentrations of anesthetics increas

A 62-year-old man presented with a history suggesting both dissociative fugue and a distinct fugue-like hallucination. The dissociative fugues included unplanned travel, loss of personal identity, inability to recall his past and amnesia for the fugue interval. The subjective fugues consisted of a stereotyped hallucination wherein he would travel to a social gathering place, meet his 'imaginary friends' and engage with them in conversation. He experienced the subjective fugues as if they were real, recognised them as hallucinations when he was normally conscious, and remembered them in great d

Obstructive sleep apnea (OSA) is a common and debilitating condition characterized by recurrent episodes of upper airway obstruction, resulting in intermittent occurrence of apnea-hypopnea. Clinical features include snoring or disturbed sleep, reduced concentration and memory, mood disorders, and excessive sleepiness (ES). Left undiagnosed and untreated, OSA may have detrimental consequences, including cardiovascular (CV) morbidity and mortality, decreased health-related quality of life, and increased incidence of motor vehicle accidents. As most individuals affected by OSA will initially pres

The default-mode network (DMN) is a set of specific brain regions whose activity, predominant in the resting-state, is attenuated during cognitively demanding, externally-cued tasks. The cognitive correlates of this network have proven difficult to interrogate, but one hypothesis is that regions in the network process episodic memories and semantic knowledge integral to internally-generated mental activity. Here, we compare default-mode functional connectivity in the same group of subjects during rest and conscious sedation with midazolam, a state characterized by anterograde amnesia and a red

BACKGROUND: Transient global amnesia is characterized by the sudden development of dense anterograde amnesia, without alteration in level of consciousness and in the absence of focal neurologic deficits or seizure activity. Various precipitating causes have been reported in the medical literature.
OBJECTIVE: To present a literature-guided approach to the diagnosis and management of transient global amnesia in the Emergency Department (ED).
CASE REPORT: We report the case of a 54-year-old woman who presented to the ED with an episode of acute memory loss.
CONCLUSIONS: Although rare, transien

Bacteraemia can be life-threatening, particularly in the neutropenic cancer patient, and any episode of pyrexia is treated empirically with antibiotics, and a blood culture taken. Over an 18-month period, 3025 (934 patients) blood culture sets were received by the microbiology department of the Shaukat Khanum Memorial Cancer Hospital and Research Centre, Lahore, Pakistan. The blood culture sets used a blood/tryptone soya broth (1:5 ratio) mixture and were manually subcultured onto appropriate media following incubation at 37 degrees C. Of these, 406 (13.4%) were positive for bacteria or yeasts

A 36-year old male with a three year history of HIV infection and more recently, CMV retinitis, had several episodes of polyradiculitis with severe bilateral leg pain and urinary retention which resolved slowly over several months. He then presented with high fevers and severe dysphagia with dehydration. Examination showed oral thrush, dyarthric speech and mild memory impairment. Fundoscopic exam showed CMV retinitis and HIV retinopathy. Further examination revealed other cranial nerve signs and leg weakness. MRI scans showed several contrast enhancing abnormalities of cranial nerve roots. The
