Biceps long head tendon revisited: a case report of split tendon arising from single origin.

A 27-year-old, right-hand-dominant woman with a posttraumatic anterior shoulder dislocation 3 months earlier after traffic accident presented because of pain and limited range of motion in the right shoulder. On physical examination, the patient had negative instability tests and a sulcus sign. On arthroscopic examination, a bifurcate long biceps tendon with two limbs was observed about 1 cm distal to the origin in the supraglenoid tubercle. We found no evidence of a tear in the long biceps tendon on probing, and the margin of each limb was smooth and round. Although this anatomic variant may

Platelet-rich therapies for musculoskeletal soft tissue injuries.

BACKGROUND: Platelet-rich therapies are being used increasingly in the treatment of musculoskeletal soft tissue injuries such as ligament, muscle and tendon tears and tendinopathies. These therapies can be used as the principal treatment or as an augmentation procedure (application after surgical repair or reconstruction). Platelet-rich therapies are produced by centrifuging a quantity of the patient's own blood and extracting the active, platelet-rich, fraction. The platelet-rich fraction is applied to the injured tissue; for example, by injection. Platelets have the ability to produce severa

Evaluation of the lag sign tests for external rotator function of the shoulder.

Rotator cuff lesions pose a serious clinical challenge. The objective of this study was to verify the biomechanical basis for the lag sign clinical tests for rotator cuff dysfunction. The lag sign tests were simulated in vitro by a sensor-guided robotic simulator configured to reproduce in vivo testing conditions. The ability of the test to isolate supraspinatus and/or infraspinatus dysfunction was investigated from 20 degrees to 90 degrees of scapular plane arm elevation. The test was 100% sensitive for lack of all infraspinatus-teres minor force at all elevations tested (6/6 specimens at 20

Redislocation of the shoulder during the first six weeks after a primary anterior dislocation: risk factors and results of treatment.

BACKGROUND: After an anterior dislocation, shoulder instability may occur with disruption of the soft-tissue or osseous restraints, leading to early redislocation. The aim of the present study was to clarify the risk factors for this complication within the first six weeks after a first-time anterior traumatic dislocation and to assess the outcome of treatment with immediate operative stabilization. METHODS: A three-year, prospective, observational cohort study of 538 consecutive patients with a first-time anterior dislocation of the shoulder was carried out. Reassessment of shoulder function

Implant-specific rates of post-operative displacement and reoperation in isolated greater tuberosity fracture open reduction and internal fixation.

BACKGROUND: Isolated fractures of the greater tuberosity (GT) comprise nearly 20% of the fractures of the proximal humerus. The Mutch classification categorizes these injuries by morphology into avulsion (Type I), split (Type 2), and depression (Type 3) patterns. While the efficacy of arthroscopic repair vs. open reduction and internal fixation is well-studied, there is less-convincing data on the comparative effectiveness of different open reduction and internal fixation constructs in GT fracture care. METHODS: A retrospective review analyzed patients with isolated GT fractures of the humeru

Axillary nerve injuries in contact sports: recommendations for treatment and rehabilitation.

Axillary nerve injuries are some of the most common peripheral nerve injuries in athletes who participate in contact sports. Resulting deltoid muscle paralysis is secondary to nerve trauma which occurs following shoulder dislocation or a direct blow to the deltoid muscle. Compression neuropathy has been reported to occur in quadrilateral space syndrome as the axillary nerve exits this anatomic compartment. The axillary nerve is also extremely vulnerable during any operative procedure involving the inferior aspect of the shoulder, and iatrogenic injury to the axillary nerve remains a serious co

Anabolic Androgenic Steroids in Orthopaedic Surgery: Current Concepts and Clinical Applications.

Despite the well-documented effects of testosterone and its synthetic derivatives-collectively termed anabolic androgenic steroids (AASs)-on the musculoskeletal system, the therapeutic use of these agents has received limited investigation within the field of orthopaedic surgery. In the last 2 decades, preclinical and clinical research has started to identify promising applications of the short-term use of AASs in the perioperative period. There is evidence to suggest that AASs may improve postoperative recovery after anterior cruciate ligament reconstruction and total joint arthroplasty. In a

Parsonage-Turner Syndrome of Unclear Causation: A Case Report.

Parsonage-Turner syndrome (PTS), also referred to as idiopathic brachial plexopathy or neuralgic amyotrophy, is a rare disorder that classically presents with abrupt, patchy, unilateral shoulder pain followed by varying degrees of weakness and atrophy of the upper extremity musculature. PTS is a serious diagnosis that can result in irreversible atrophy with delayed diagnosis and treatment. Since there currently is no gold standard for diagnosis and the syndrome can present as many other possible pathologies, careful clinical examination and thorough review of relevant imaging and diagnostic st

Explainable machine learning to predict prolonged post-operative opioid use in rotator cuff patients.

BACKGROUND: Opioid overuse is a costly and significant problem in the United States. Medical specialties including surgery are a contributor to opioid prescriptions while having few clear prescribing guidelines. Machine learning predictive tools can assist surgeons with evaluating patients for opioid prescriptions and help prevent prescriptions for patients at-risk for prolonged post-operative opioid use. This project aims to develop accurate and transparent machine learning models to predict prolonged opioid use by individuals undergoing rotator cuff surgery. METHODS: Six machine learning mo

Effects of early exercise and immobilization after arthroscopic rotator cuff repair surgery: a systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: Early exercise is a physical adjuvant therapy that begins on day 1 postoperatively. It prevents postoperative stiffness, fatty infiltration, muscle atrophy and loss of range of motion. Usually, use of a brace fixation that immobilizes the shoulder in 30° of abduction during the postoperative rehabilitation period reduces tension on the repaired tendon, which improves tendon-bone healing. To investigate the effect of early exercise and brace fixation on postoperative recovery after arthroscopic rotator cuff repair by systematic review, thereby providing evidence-based evidence for cl

Effectiveness of high intensity laser treatment in partial supraspinatus tendon tears: a randomised controlled trial.

The most common cause of rotator cuff injury is supraspinatus tendon tears (STTs). High-intensity laser therapy (HILT) has recently emerged as an important conservative treatment option. This study was designed as a randomised controlled trial in patients with partial STTs to compare the effects of HILT with those of ultrasound (US) therapy. Fifty patients between the ages of 18-75, who were diagnosed with partial STT were randomised into two groups using the closed envelope method. They were blinded to group assignment. Group-1 received 3-week HILT program. Group-2 received 3-week US therapy

A comprehensive scoring system for the diagnosis and staging of adhesive capsulitis: development, application, and implications.

INTRODUCTION: Adhesive capsulitis (AC), often referred to as frozen shoulder, presents a diagnostic challenge due to its insidious onset and progressive nature. The condition is characterized by pain and restricted motion in the shoulder, with a predilection for individuals between 40 and 60 years of age. A novel scoring system was developed to enhance the accuracy of diagnosing AC and distinguishing between its stages, aiming to streamline clinical decision-making and treatment planning. METHODS: A cohort of patients with symptoms suggestive of AC was assessed using the new scoring system, w

Early Postoperative MRI Evaluation of a Fascia Lata Autograft With and Without Polypropylene Mesh Augmentation After Superior Capsular Reconstruction.

BACKGROUND: Recently, a polypropylene mesh has been introduced and reported to improve clinical outcomes after superior capsular reconstruction (SCR) using a fascia lata autograft (FLA). However, mesh-related events such as a foreign body response may trigger inflammation, which might affect graft healing and remodeling. PURPOSE/HYPOTHESIS: The aim was to investigate whether the healing and remodeling of an FLA were affected by the use of a mesh by comparing the signal intensity of an FLA-alone group vs an FLA + Mesh group on postoperative magnetic resonance imaging (MRI). The hypothesis was

Outcomes of reverse shoulder arthroplasty following failed superior capsular reconstruction.

BACKGROUND: Superior capsular reconstruction (SCR) can be used for massive irreparable rotator cuff tears in the absence of significant degenerative changes; however, those who fail an SCR may require reverse shoulder arthroplasty (RSA). The effect of a previously performed SCR on outcomes following RSA remains unknown. METHODS: Subjects who underwent RSA from May 2015 to January 2021 at 2 separate institutions were retrospectively identified through prospectively collected databases. Patients who underwent RSA after failed SCR were matched to those who underwent RSA after failed rotator cuff

Graft healing does not influence subjective outcomes and shoulder kinematics after superior capsule reconstruction: a prospective in vivo kinematic study.

BACKGROUND: A viable treatment option for young patients with massive, irreparable rotator cuff tears is arthroscopic superior capsule reconstruction (SCR). SCR theoretically improves shoulder stability and function and decreases pain. However, no prospective studies to date have correlated magnetic resonance imaging (MRI) healing with in vivo kinematic data. The purpose of this study was to evaluate the association between graft healing and in vivo kinematics, range of motion (ROM), strength, and patient-reported outcomes (PROs). METHODS: Ten patients (8 men and 2 women; mean age, 63 ± 7 yea
