Correlation of right ventricular outflow tract residual disease on post-op transesophageal echocardiography and discharge transthoracic echocardiography after tetralogy of fallot repair.

Post-operative transesophageal echocardiography (TEE) is standard of care to evaluate for residual disease and quality of surgical repair. The residual lesion score (RLS) as defined by the pediatric heart network explored the impact of residual lesions on outcomes using discharge transthoracic echocardiogram (TTE). In tetralogy of fallot with pulmonary stenosis (TOF/PS), the residual right ventricular outflow tract (RVOT) gradient is one marker of quality of repair. Given peri-operative differences in hemodynamics, this study aims to define the correlation between TEE and discharge TTE RVOT gr

Twenty-year experience with the Konno operation: Konno incision does not impair LV function.

BACKGROUND: There has been no report observing the LV function of Konno incision. We examined the outcomes of the Konno operations over 20 years. METHODS: We investigated 63 Konno operations with mechanical valves performed from 1984. This study aimed to evaluate the effects of Konno incision including LV function changes and the long-term outcomes of patient survival and reoperation rate (RVOT and LVOT complications). RESULTS: The postoperative follow-up period was 20.6 ± 6.2 years. Early death occurred in 1 case and late death occurred in 9 cases. The cumulative survival rate was 89.6, 87.

Pulmonary vascular protective mechanisms in adult patients with an isolated large ventricular septal defect: a 21-year experience.

BACKGROUND: Ventricular septal defects (VSDs) are one of the most common congenital heart defects in adults. In adult patients with an anatomically large VSD and relatively preserved pulmonary vascular system, several pulmonary flow-limiting cardiac morphologic alterations (PFMA) are encountered. PATIENTS: Ninety-eight male patients (mean age 22.5 +/- 2 years) operated for an anatomically large VSD in our institution were retrospectively reviewed. PFMA in patients with an anatomically large but functionally mild-to-moderate VSD (when ratio of pulmonary to systemic flow (Q(p)/Q(s)) < 2.2 and r

Intermediate results with correction of tetralogy of Fallot with absent pulmonary valve using a new approach.

OBJECTIVE: The intermediate results achieved with a new technique for primary repair of tetralogy of Fallot with absent pulmonary valve syndrome are reported. METHOD: Apart from correction of tetralogy of Fallot, this approach includes translocation of the pulmonary artery anterior to the aorta and away from the tracheobronchial tree. Since November 1998 this technique has been employed in three symptomatic newborns and three infants with a diagnosis of tetralogy of Fallot with absent pulmonary valve. The median age at surgery was 37 days (range 14-256 days). The median weight at operation wa

Underestimation of the severity of pulmonary outflow tract obstruction in the first day of life--Doppler echocardiographic study.

To find out whether there is an underestimated severity of narrowing in obstructive lesions of the pulmonary outflow tract in the first day of life, Doppler measurement of the pulmonary outflow tract gradient and estimation of the pulmonary artery systolic pressure in 15 neonates with pulmonary outflow tract obstruction with a variety of associated lesions were studied in the first twenty-four hours of life (mean thirteen hours, range six to twenty-four) and repeated at the age of twenty-six to seventy-two hours (mean forty-nine). The maximal instantaneous gradient across the pulmonary outflow

Repeated repair of tetralogy of Fallot. Report of 11 cases and review of the literature.

Eleven patients underwent late repeated correction of tetralogy of Fallot in 1991-1993. The previous operation was repair of simple Fallot's tetralogy in seven cases, repair plus transannular patch in one case and repair of tetralogy and pulmonic atresia in three cases. The indications for reoperation were residual ventricular septal defect, right ventricular outflow tract (R.V.O.T.) obstraction, residual branch pulmonary artery stenosis, aneurysmal dilatation of R.V.O.T. Patch or combination of any of the above. At reoperation these defects were corrected. The post operative course was uneven

A Nomogram for Predicting Residual Mitral Regurgitation in Patients With Obstructive Hypertrohpic Cardiomyopathy Undergoing Transapical Beating-Heart Septal Myecotmy.

OBJECTIVES: Residual mitral regurgitation (MR) after septal myectomy increases the risk of further mitral interventions. This study aimed to identify predictors of residual MR (grade ≥ 2+) following transapical beating-heart septal myectomy (TA-BSM) in hypertrophic obstructive cardiomyopathy (HOCM) patients and to develop a predictive nomogram. METHODS: We prospectively enrolled 200 HOCM patients undergoing TA-BSM from April 2023 to March 2024, all with resting or provoked left ventricular outflow tract gradient ≥ 50 mmHg and MR ≥ 2+. Preoperative clinical and echocardiographic MV parameters

Pseudo-Anterior ST-Elevation Myocardial Infarction Due to Hypertrophic Cardiomyopathy.

We report the case of a 43-year-old man with cardiovascular risk factors such as hypertension, hypertriglyceridemia and non-alcoholic fatty liver disease. He was admitted with a complaint of exertional and persistent chest pain, accompanied by electrocardiographic changes suggestive of anterior ST-elevation myocardial infarction. Initial investigations, including coronary angiography and optical coherence tomography, revealed unobstructed coronary arteries and no evidence of plaque disruption. Transthoracic echocardiography demonstrated marked asymmetric septal hypertrophy with mild systolic a

Deceptive Anteroseptal ST-segment Elevation and Brugada Pattern Caused by Isolated Conus Artery Occlusion.

The conus artery (CA) supplies the right ventricular outflow tract (RVOT). ST-segment elevation in leads V1-3, which can resemble Brugada electrocardiogram (EKG) patterns, has been reported due to occlusion of the CA. A 68-year-old male was admitted to the hospital with a diagnosis of non-ST-elevation myocardial infarction. A coronary angiogram revealed a dissection in the conus artery, most likely caused by the catheter. Due to the small caliber of the CA, medical therapy was chosen as the course of action. However, after the procedure, an EKG showed changes consistent with features of both t

Systolic Anterior Motion (SAM) Complicating Mitral Valve Repair: Current Concepts of Intraoperative and Postoperative Management.

Mitral valve repair is the gold standard for treatment of degenerative mitral regurgitation, such as that caused by leaflet prolapse, flail or annular dilatation. A variety of surgical techniques allow surgeons to achieve a high rate of success with mitral valve repair, even in complex cases, and mitral valve repair is associated with better long-term outcomes than valve replacement. However, in the setting of mitral valve surgical repair, systolic anterior motion (SAM) is a complication that determines the dynamic anterior movement of the mitral valve towards the interventricular septum durin

Outcome of patients ≥ 60 years of age after alcohol septal ablation for hypertrophic obstructive cardiomyopathy.

INTRODUCTION: The outcome of patients ≥ 60 years of age after alcohol septal ablation (ASA) for obstructive hypertrophic cardiomyopathy (HCM) remains unresolved. We sought to determine the long-term survival and the causes of death in this population. MATERIAL AND METHODS: We enrolled 156 consecutive patients (69 ±6 years, 69% women, follow-up: 4.8 ±3.5 years) who underwent ASA at ≥ 60 years of age. RESULTS: The 30-day mortality rate was 1.3%. At the last check-up, 81% of patients were in New York Heart Association class ≤ 2 and 76% had a left ventricular outflow tract gradient (LVOG) ≤ 30 m

The clinical outcomes of damus-kaye-stansel procedure according to surgical technique.

BACKGROUND: The Damus-Kaye-Stansel (DKS) procedure is a method for mitigating the risk of systemic ventricular outflow tract obstruction (SVOTO). However, there have been few reports on which surgical technique shows a better outcome. The objective of this study was to compare the outcome of the DKS procedure according to the surgical technique used. METHODS: We retrospectively reviewed 12 consecutive patients who underwent the DKS procedure from March 2004 to April 2013. When the relationship of the great arteries was anterior-posterior, the double-barrel technique (group A) was performed. I

Early postoperative interventional ASD-closure for severe atrial right to left shunt in a neonate with common arterial trunk.

Although closure of an atrial septal defect (ASD II) with an occluding device in the first year of life is not a routine procedure, it is a feasible treatment, even in neonates. Case reports on the off-label use of Amplatzer devices have been repeatedly published, but there are no reports on using the Amplatzer Duct Occluder (ADO) to close an atrial septal defect in a neonate. We report on a successful catheter closure of an ASD II with ADO in a severely cyanotic neonate, seven days after surgical repair of common arterial trunk. Due to progressive cyanosis and clinical signs of right ventricu

Medical, surgical and interventional management of hypertrophic cardiomyopathy with obstruction.

Patients with hypertrophic cardiomyopathy (HCM) are classified as having hypertrophic obstructive cardiomyopathy (HOCM) if a left ventricular outflow tract (LVOT) gradient is present at rest or during provocation, as with Valsalva maneuver or exercise. Management of HCM in general and HOCM in particular encompasses (1) activity restriction with avoidance of volume depletion, (2) prevention of sudden cardiac death, (3) control of symptoms, and (4) screening of relatives. Those patients at high risk of sudden cardiac death (SCD) should be offered an implantable cardioverter-defibrillator (ICD).

Considerations for prenatal counselling of patients with cardiac rhabdomyomas based on their cardiac and neurologic outcomes.

Cardiac rhabdomyomas are benign cardiac tumours with few cardiac complications, but with a known association to tuberous sclerosis that affects the neurologic outcome of the patients. We have analysed the long-term cardiac and neurological outcomes of patients with cardiac rhabdomyomas in order to allow comprehensive prenatal counselling, basing our findings on the records of all patients seen prenatally and postnatally with an echocardiographic diagnosis of cardiac rhabdomyoma encountered from August, 1982, to September, 2007. We analysed factors such as the number and the location of the tum

Lymphatic aneurysm of the heart.

The Annular Subvalvular Left Ventricular Aneurysm (ASLVA) is a rare entity, which occurs usually in young ethnic groups from Sub-Saharan Africa and South India. These aneurysms are situated immediately beneath the mitral and aortic valves and extend around in the substance of fibrous ring, from which valves arise. Their etiology is still obscure. Fifteen consecutive cases of ASLVA were treated surgically for this heart condition. Their clinical presentations, surgery, histopathology of aneurysmal wall were studied. Infective diseases with associated mediastinal lymphadenopathy were observed in

Hybrid Transcatheter Pulmonary Valve Replacement With SAPIEN S3 Following Right Ventricular Outflow Tract Reduction Using Tricuspid Annuloplasty Rings.

We describe a novel variation of hybrid TPVR using a tricuspid annuloplasty ring which can be considered for patients with very large RVOT, short pulmonary artery, and limited vascular access and to avoid the risk of cardiopulmonary bypass. The utilization of Carpentier-Edwards Physio Tricuspid annuloplasty ring can reduce the size of the RVOT and at the same time the ring served as an effective docking site for the implantation of the SAPIEN S3 (Edwards Lifesciences). All three reported cases showed satisfactory immediate hemodynamic and clinical outcomes as well as at mid-term follow up.

The Superior Cavopulmonary Connection: History and Current Perspectives.

The development of the superior cavopulmonary connection is a rich illustration of international influences in congenital cardiac surgery. The bidirectional Glenn and hemi-Fontan procedures have improved survival as both definitive and staged functional single ventricle palliation. The optimal timing of the second-stage superior cavopulmonary procedures varies by center but for low- and intermediate-risk patients, this may be within three to six months after the Norwood procedure. The list of risk factors continues to grow but the most frequently cited factors include atrioventricular valve re
